    Why: Pilot overshot the runway. The pilot was not hurt but the plane landed in a sewage pond and had to swim (through the sewage pond) to safety.

    August 16: West Caribbean Flight 708

    For most people August 16 was just a day. But there are still some people who remember this day as the day in 2005 that one hundred fifty-two people died aboard a MD-82 near Machiques Venezuela. First one engine failed, then the second. After both engines flamed out, Flight 708 requested an emergency landing from Machiques ATC, but pilots lost control and within three minutes, crashed in a swampy area in a cattle ranch. The Bureau d’Enquêtes et d’Analyses pour la Sécurité de l’Aviation Civile (BEA) Investigation found the cause was human error due to in-flight problems from poorly paid, stressed Caribbean Airways crew working with poor communication.

    One hundred and fifty-two French citizens from Martinique, and Colombian crew of eight were lost. It has gone down in the record books as the biggest crash in Venezuelan history, and the worst accident in 2005. In the distant photo of this accident, we cannot help but see how small and frail the craft looks. Like a toy of broken matchsticks lying broken on the ground. Sometimes we should remember that we are creatures of land, and have given ourself wings. The tragedy that we sometimes fail does overshadow sometimes that itt is a marvel that we fly.

    It is the job of the investigation to find the cause to help make future flight safer; but when we remember a date such as this, it is a time to remember the passengers and crew. It is time to remember and console the families who survive them.

    Whatever the cause, tragedies wear the same face of irreparable loss. It is not only France and Colombia that mourned the loss of their citizens. When tragedies happen, all the nations of the world mourn. So let us pause in our day and remember those who are lost to us; and take the time to remember John Donne “Any man’s death diminishes me, because I am involved in Mankind; And therefore, never send to know for whom the bell tolls; it tolls for thee.”

    South African Express Flight Makes Emergency Landing due to Engine Issue

    South African Express flight SA-1412 had to return and make an emergency landing in East London, South Africa, on May 11th.

    The plane took off for Johannesburg, South Africa, but had to turn back after the crew needed to shut down one of the engines.

    The plane landed safely. All passengers and crew members remained unharmed.

    Virgin Australia Boeing Engine Flames On LA Runway

    What: Virgin Australia Boeing 777-300 en route from Los Angeles to Sydney
    Where: Los Angeles
    When: April 12, 2011
    Who: 305 passengers
    Why: On making its takeoff run from Los Angeles, the plane emitted a loud “bang!” and stopped on the runway, they believed for a nose gear. It turned out to be the fireball erupting from engine one. Needless to say, the jet was pulled for inspection, and the passengers were given hotel accommodations for the night. They were provided an alternative flight the next day.

    Air Ambulance Emergency Landing

    What: ORNGE AugustaWestland air ambulance en route from Billy Bishop island airport to a patient west of Toronto
    Where: Colonel Samuel Smith Park south of Lake Shore Boulevard West.
    When: March 23, 2012, 6:00 pm
    Who: 2 pilots, 2 medics
    Why: While en route to pick up a Toronto patient, the air ambulance door opened and a window blew out. The pilots had to make an immediate emergency landing near Humber College.

    No one was injured, but they had been in jeopardy. Sn ORNGE insider said if the door or window hit the tail rotor “it could have been catastrophic.”

    Two aircraft engineers removed the faulty door. The flight crew flew the chopper back to the airport for examination.

    A known problem of the AW139 is the helicopters tail rotors. An EASA directive of Aug. 19 2011 ordered helicopter owners to replace the tail rotors after 600 hours of flight following a fatal accident “possibly caused by cracks in a TR (tail rotor) blade.”

    5 Injured in Kit Plane Crash through Publix Roof


    What: Seawind 3000 kit plane built by Larry E. Sapp
    Where: Northgate Shopping Center in DeLand Florida
    When: April 2, 2012, 7:20 pm
    Who: 2 aboard, 5 injured
    Why: Taking off from Deland Municipal-Sidney H. Taylor Field Airport, the small kit plane developed engine problems and crashed into a Publix market in the Northgate Shopping Center on International Speedway Boulevard on Monday. The pilot and passenger are in critical condition. Brendan Beitler, a Stetson University student is in stable condition. Cordova and Morris Beitler were also injured.

    A fire occurred when the fuel tanks burst. Three individuals in the store were injured. The store has been closed, and 175 employees are working at other Publix. The city has issued a demolition permit.

    The plane is made of composite material and was amateur-built in 2002.

    The pilot was Kim Presbrey, an attorney from Chicago who is in the intensive care unit at Orlando Regional Medical Center. Presbry is managing partner of Presbrey & Associates Thomas Rhoades from Illinois was the passenger.

    What remains of the melted aircraft has been loaded on a flatbed trailer.
