    Redwings Moscow Crash Has Three Survivors


    The Red Wings Tupolev jet that crashed on December 29 had three survivors, two men and a woman.

    Their conditions are improving.

    The five fatalities included the captain, first officer, flight engineer, and two flight attendants. One of the now-deceased flight attendants had survived a while with traumatic brain injury, but succumbed.

    Russian investigators have started analysing the flight recorders.

    Reports are that the runway was inspected an hour and a half before the flight landed and overrun it. Speculation is that there may have been a braking (thrust reverser) issue.

    On Dec 31st 2012 Rosaviatsia released a modification to the aircraft operations manual requiring flight crew to apply reverse thrust only if the engine is operating

    Application of reverse thrust is prohibited on a malfunctioning engine.

    To apply reverse thrust the engine should be pulled to idle for 1-2 seconds, then command reverse thrust at minimum thrust, verify yellow “thrust reverser unlocked” followed by green “REV” indications appear, only then apply high reverse thrust. Rosaviatsia reported there have been a number of cases where the green “REV” indication did not appear (editorial note: there have been rumours since the Dec 20th overrun, that the crew had applied reverse thrust, the reverser however had not opened and at least one of the engine was delivering full forward thrust instead).

    Four die in Red Wings Ferry Flight Crash in Russia


    On December 29, 2012, at 16:33, a Red Wings Tupelev 204-100V was on a ferry flight from Pardubice Airport Czech Republic to Moscow-Vnukova Airport, Russia when it crashed-landed in snow, overrunnning the runway. Weather conditions showed heavy gusts accompanying a light snowfall. The Federal Air Transport Agency said that the plane touched down in the proper landing area but for some reason was unable to stop on the runway.

    Initially traffic controllers refused to authorise landing, and the plane had to circle the airport. Fire trucks were not on standby; they were undergoing maintenance and were nearly an hour late.

    The captain, copilot, flight engineer and one flight attendant were killed. Two died on the scene, one died en route in the ambulance, and one died at the hospital. Four crew survived with serious injuries. Pieces of the plane caught fire.

    The plane overran the runway and struck the M3 highway embankment, breaking into three parts. The fuselage landed on the highway 320 meters from the end of the runway. A BBC report said the aircraft was in several pieces, with the cockpit on the highway. Kiev Highway and Vnukovo Airport were both shut down temporarily.

    Banner Flight Ends in San Diego Bay

    What: Cessna 150
    Where: San Diego bay
    When: May 26, 2012, 4:20 p.m
    Who: 2 aboard, no fatalities
    Why: A Cessna towing a banner lost engine power. Pilots informed ATC, cut free the banner they were towing and ended up in San Diego Bay, Missing the Hilton Hotel. The plane sank; the pilot and passenger aboard were recovered uninjured, by passing boaters.

    A salvage company is going to recover the plane from the bay.

    Balloon Crashes Due to Wind Shear, Pilot lost License

    Where: Ljubljana Slovenia
    When: August 23, 2012
    Who: 30 passengers, 2 crew, 4 fatalities, 28 injured
    Why: A hot air balloon with 32 aboard caught fire and crashed into a tree, burning four beyond recognition. At least six children were injured. Then people at the scene were reported resuscitated; eight in the crash were in critical condition. One passenger was quoted as saying “we were landing, but I think the speed was too high for landing so we hit the ground, bounced off once and hit it again. We held on, but five of us fell out“. Twenty-one were admitted in the hospital Ljubljana, and seven to other hospitals. The injured included one brit and three Italians.

    When the balloon hit the ground and and bounced back in the air, some passengers jumped or fell out, scattering the injured across four locations and making it more difficult for rescuers. Nine teams with ambulances and twelve emergency medical service teams responded as well as 79 firefighters, 20 fire fighting vehicles and three helicopters.

    The balloon takes off and lands in uncontrolled airspace and does not need air traffic control clearance.

    Prime Minister Janez Janša, Minister for Health Tomaz Gantar, Minister of Defense Aleš Hojs and Interior Minister Vinko Gorenak expressed condolences.

    The cause is being attributed to ground level wind shear. The pilot needed to retake a health exam to renew his license in June.

    Five are unidentified.

    The on board extinguishers apparently had cracked cylinders.

    New computation of location is based on faster speeds, faster usage of fuel based on Kuala Lumpar data. Termed the most credible lead.

    Mercy Flight Engine Shut Down During Evac

    What: Mercy Flight enroute Buffalo to ECMC from Orleans County
    Where: A mile from Buffalo Niagara
    When: Dec 2 2009
    Who: Pilot Shawn Hayes, 2 flight crew nurses and a patient
    Why: After Pilot Shawn Hayes reported “a drop” in one of his engine, he headed for the airport. ATC halted takeoffs for 5 minutes allowing Hayes to land anywhere but he didn’t make it to the airport. With one engine running, the pilot made a safe emergency landing.

    Flight nurses transported patient by ambulance.

    Hayes is a 23 year former military pilot.
