Swiftair Plane Makes Emergency Landing at Shannon Airport

swiftairSwiftair flight QY-2939 had to return and make an emergency landing at Shannon Airport, Ireland, on November 17th.

The cargo plane had just departed for Leipzig, Germany, when the crew reported that they had lost all instruments and declared an emergency.

The crew were able to re-gain most of the instruments before the plane landed back safely.

Everyone aboard remained unharmed.

Pinging for Egyptair MS 804 as the Clock Runs Out

When a plane goes down in the ocean, the black boxes aboard have enough juice to ping for thirty days. The pingers on flight data recorders AKA “black boxes” last a minimum of 30 days. After 30 days, the devices are still active, but the sound on which searchers hone is expected to die out. The pinger is located by a “pinger locater,” a device that listens for the sound of the black box. It is towed within the search area but it’s listening radius is usually around 2 miles. The pinger’s sound is not very powerful, and the pinger is towed at 3 knots.

After AirFrance 447, legislation was underway to increase the battery life to 90 days. The technology exists, but because implementation of that transition has been slow, EgyptAir MS 804’s pinger battery is expected to expire at around 30 days.

EgyptAir: 3 Alerts Before the Plane Left Radar. Fire on board.

Several different alerts on this plane went off immediately prior to the plane disappearing from the radar, including 1) issues with the windows on the co-pilot’s side of the plane; 2) two smoke alerts, the first one “SMOKE LAVATORY SMOKE” at 00:26 a.m. local time; 3) the second one “AVIONICS SMOKE” at 00:27 a.m. The second alert refers smoke in the avionics bay, which is located below the cockpit, and is where the electronics are housed.

This is also the same plane that was vandalised two years ago at Cairo Airport. In Arabic someone, presumably Cairo Airport workers, wrote on its belly, “We will bring this plane down.”

It is a special concern that the plane had flown to Eritrea.

The US State Department has a standing travel warning for Eritrea, and is quoted as saying “security at the Asmara airport ‘can be unpredictable,’ and had a ‘lack of efficiency and consistency” in screening passengers.'”

The search for Egyptair is still underway.

On the search for Egyptair Flight MS804

According to AV:

on 2016/06/01, Egypt’s Civil Aviation Authority reported “Laplace” located pings presumably from one of MS804’s black boxes. The vessel “John Lethbridge” of DOS will be joining the search team to retrieve the black boxes. The BEA (FR)confirmed that Egyptian Authorities have confirmed a “signal that may come from one of the recorders” of flight MS-804. The BEA did NOT confirm that a black box has been located.

Debris has been found.

MH370: One Ping Does Not A Discovery Make. Or Does it?

An underwater locator beacon (ULB) such as the one on the black boxes (CVR) Cockpit Voice Recorder and FDR (Flight Data Recorder) of Malaysia Airlines Flight 370 on 37.5 kHz for about 30 days at 4°C temperature. They run on lithium-ion batteries, and “mileage” may vary; 30 days is the minimum expectation. This is all relevant to today’s news because the Chinese patrol ship Haixun 01 which is one of the ships equipped with a pinger locator, has heard a ping in the South Indian Ocean.

The particular frequency was selected because it is not one that occurs in nature.

Although Malaysia Airlines told the public that “This battery is not replaceable,” the ULB batteries had been scheduled for battery replacement in 2012, but were not replaced by Dukane Seacom, the original equipment manufacturer of the beacons. (Dukane Seacom either replaces the entire pinger or installed new batteries.) If replacement was not performed by toe OEM or other parties, the actual ping time may be less than 30 days.

One ping in an ocean does not a discovery make. The wreckage has not been located, nor the ping confirmed. But we can still hope this is a step in the right direction.

New computation of location is based on faster speeds, faster usage of fuel based on Kuala Lumpar data. Termed the most credible lead.

Prime Minister Endorses Australian-based search

On March 24, Najib Razak, Malaysia’s Prime Minister, announced that new computations by Inmarsat and the AAIB, flight MH-370 ended in the South Indian Ocean west of Perth (Australia). At this point, Malaysia endorsed the new search search parameters.

The full statement:

“This evening I was briefed by representatives from the UK Air Accidents Investigation Branch — or AAIB. They informed me that Inmarsat, the UK company that provided the satellite data which indicated the northern and southern corridors, has been performing further calculations on the data. Using a type of analysis never before used in an investigation of this sort, they have been able to shed more light on MH370’s flight path.

Based on their new analysis, Inmarsat and the AAIB have concluded that MH370 flew along the southern corridor, and that its last position was in the middle of the Indian Ocean, west of Perth. This is a remote location, far from any possible landing sites.

It is therefore with deep sadness and regret that I must inform you that, according to this new data, flight MH370 ended in the southern Indian Ocean.

We will be holding a press conference tomorrow with further details. In the meantime, we wanted to inform you of this new development at the earliest opportunity. We share this information out of a commitment to openness and respect for the families, two principles which have guided this investigation.

Malaysia Airlines have already spoken to the families of the passengers and crew to inform them of this development. For them, the past few weeks have been heartbreaking. I know this news must be harder still. I urge the media to respect their privacy, and to allow them the space they need at this difficult time.”

Malaysia Airlines Flight Update

Although the Malaysia Airlines has denied reports of a beacon, the Malaysian navy has said publicly they have tracked the wreckage of the missing Malaysia Flight MH370, and has dispatched a plane, two helicopters and four vessels i addition to the Philippines three navy patrol boats and one surveillance plane. Chinese ships and aircraft are on standby with eight ships belonging to the Nanhai Rescue Bureau and the Hainan Maritime Safety Administration, and a supporting air fleet. Boeing expressed their concern and are sending a team to assist the investigation.

Meanwhile, in spite of the airline telling the public the whereabouts are unknown, families are being told to take their valid passports to KLIA airport before 6 pm for MAS travel arrangements to the crash site.

If the plane is down (and even if it is magic thinking, we are still hoping it landed safely somewhere), search and rescue is still within the “24-hour golden window” in which it is more likely to find survivors. Eighty% of the family members have been contacted by Malaysia Airlines, according to Malaysia Airlines at the conference at Sama Sama Hotel, Kuala Lumpur, March 8, 2014.

The plane that is missing is registration number 9M-MRO. In September 2012, the plane was on the ground and suffered an airfield incursion when it collided with a China Eastern Airlines’ Airbus.

Malaysia Airlines Flight Missing

Saturday at 12:40 a.m. a Malaysia Airlines B777-200 (registration #9M-MRO) took off from Kuala Lumpur and disappeared.

Subang Air Traffic said radio and radar contact was lost from flight MH370 at 01:22, forty minutes into the flight, contact was never made with Ho Chi Minh Control Center in Vietnam at the expected time. Chinese reports deny the plane ever entered their airspace. Subang ATC did not inform the airline until 02:40, which may have caused a catastrophic delay in the information stream, preventing the possibility of rescue.

A search is underway for the missing plane somewhere along the route between the Gulf of Thailand and the China Sea, northeast of Kota Bharu (Malaysia).

Radar suggests a descent of the aircraft.

The plane’s first flight was May 14, 2002. It is 11 years old and flew on Rolls Royce Trent 892 engines.

The flight’s intended destination was Beijing.

Our prayers are with the twelve crew and 227 passengers. Reports are that two infants were aboard.

Sepang, 8 March 2014: Malaysia Airlines confirms that flight MH370 has lost contact with Subang Air Traffic Control at 2.40am, today (8 March 2014).

Flight MH370, operated on the B777-200 aircraft, departed Kuala Lumpur at 12.41am on 8 March 2014. MH370 was expected to land in Beijing at 6.30am the same day. The flight was carrying a total number of 227 passengers (including 2 infants), 12 crew members.

Malaysia Airlines is currently working with the authorities who have activated their Search and Rescue team to locate the aircraft.

Saturday, March 08, 09:05 AM MYT +0800 Malaysia Airlines MH370 Flight Incident – 2nd Media Statement

We deeply regret that we have lost all contacts with flight MH370 which departed Kuala Lumpur at 12.41 am earlier this morning bound for Beijing. The aircraft was scheduled to land at Beijing International Airport at 6.30am local Beijing time. Subang Air Traffic Control reported that it lost contact at 2.40am (local Malaysia time) today.

Flight MH370 was operated on a Boeing B777-200 aircraft. The flight was carrying a total number of 239 passengers and crew – comprising 227 passengers (including 2 infants), 12 crew members. The passengers were of 13 different nationalities. Malaysia Airlines is currently working with the authorities who have activated their Search and Rescue team to locate the aircraft. Our team is currently calling the next-of-kin of passengers and crew.

Focus of the airline is to work with the emergency responders and authorities and mobilize its full support. Our thoughts and prayers are with all affected passengers and crew and their family members. The airline will provide regular updates on the situation.

Nepal Airlines Flight Missing

Eighteen people are missing over the Himalayan mountains of central Nepal on a Nepal Airlines Twin Otter en route from Pokhara to Jumla. The plane had stopped in Pokhara to refuel. The crew lost contact with Bhairawa ATC a quarter hour after taking off. The plane never arrived in Jumla.

The plane’s first flight was in 1971.

The route from Pokhara to Jumla flies over Dhorpatan and Rukum, and zig zags over tall mountains.

The last radio contact was at 13:13.

A fire was spotted in a forested area near Khidim but the search was called off due to weather conditions. Locals also reported hearing a loud noise. The Fishtail Air search helicopter was unable to land due to poor visibility and the air search was called off. On foot, the search continues with the Nepal Army, a team from Palpa and Mujhum Police.

The wreckage has not been located but the crash site is presumed to be on the border of Argakhanchi, Gulmi and Palpa districts, one and half hour walking uphill from Khidim village in Argakhanchi district.

There were three crew and fifteen passengers, all but one of whom were from Nepal. The foreigner is Dutch.

The crew was made up of Captain Shankar Shrestha, co-pilot Rabinda Banjara and Air hostess Muna Maharjan. The names of the passengers have not been released.

One child was also aboard.

Experimental Crash in Brazil kills Deputy Mayor

A small plane, an INPAER Explorer#PR-ZAL with three aboard was en route from Campos dos Amarais, Campinas to London but disappeared from radar while talking to ATC in Pirassununga Brazil at about ten on January 23rd. The experimental plane was registered but not certified, and barred from flying over populated areas.

The Brazilian Air Force, Botucatu and São Manuel police forces were searching with a C-105 Amazonas, a H1 and an Eagle Helicopter, and found the remains of the passengers in dense forest, in a rugged area near Monte Alegre.

The Squad of Campo Grande rescue team is on the scene.

Locals who heard the failing engine, a roar and saw a flash of light in the woods notified authorities.

The remains have been identified as:
-Alderman and Deputy Mayor of Suzano (SP) Jessé Almeida,
-Edson Geraldino, identified as owner of the plane.

The identity of the third party, Rubens Geraldino, has not been confirmed by the family. The passengers (uncle and nephew) were from Mogi das Cruzes, SP, Brazil.

Helicopter Lost in Dordogne river

Four people are missing after a helicopter crashed into the Dordogne river.

The helicopter fell into the waters of the Dordogne, by the commune of Lugon-and-l ‘ île-du-Carnay, bordering the River, where is located the castle of île-du-Carnay.

Divers were searching the waters of the Dordogne while police swarmed the banks at the port of Petit Chartron. At midnight Friday, an unidentified corpse was recovered in the wreckage of the helicopter. The search for the other three missing was suspended at 11:30 and will resume Saturday morning at 8:00 a.m..

Gold Coast Crash Victims Found

Volunteer Coast Guard, Volunteer Marine Rescue and Water Police searched the coast Monday and Tuesday for a missing plane.

Pilot Alexander Rae, AKA Jimmy, was flying a Gold Coast Tiger Moth Joy Rides de Havilland DH 82 biplane off South Stadbroke Island off the coast of Queensland. His last message was “Conducting aerobatics over 3500 feet.” Jimmy was twenty-six years old, and had a reputation of being an excellent pilot. He was newly-wed to Alice, who is an equine dentist.

Tuesday at 9:30 a.m., sonar located the plane on the seabed near the Couran Cove Resort where the passenger and pilot were subsequently found by police divers, still inside the plane. Debris from the plane had already washed up.

The plane crashed into the water about four hundred meters from the coast on December 16. Police are looking for witnesses.

The Tiger Moth was built in the thirties. Tiger Moth Joyrides had been in operation since 1978.

Mexican Plane Crash, 14 fatalities

On Nov 14, 2013, an Aereo Services Guerrero Cessna 208B Grand Caravan en route from Loreto to Ciudad Constitucion was lost en route. All attempts to contact the plane from ATC failed.

Locals reported seeing the plane flying low in Tembabichi. Search perimeters were extablished but Tropical Storm Octave with 50 mph winds interfered with the flight as well as the search.

The wreckage was found at Agua Verde and Tembabichi area, Baja California Sur.

The wreckage was found on Oct 17, 2013, with fourteen passengers were aboard, all fatalities.

Star Marianas Crash in North Marina Islands

Piper PA-32-300 Cherokee Six operated by Star Marinas crashed between Tinian and Saipan, North Marina Islands on Oct 5.

The four survivors of the crash were airlifted to the Commonwealth Health Center.

Three boats and a US Navy helicopter and 57 rescue personnel were part of the search and rescue of the Star Marianas flight that was missing.

There were three fatalities, a 29 year old Chinese man, a 26 year old Chinese woman and the pilot, a male Caucasian. The flight was reported lost at sea, but the wreckage indicated that: 2 of the bodies were still in the fuselage and one was outside the plane on the ground.

An earlier report announced the missing flight:

At approximately 3:15am local time on October 6, 2013 a Piper PA-32-300 Cherokee Six aircraft being operated by Star Marianas Air, Inc. which had departed from West Tinian Airport (Airport ID: PGWT) at 2:41am enroute to Saipan International Airport (Airport ID: PGSN) was reported as having been overdue for arrival.

June 30, Yemenia Flight 626

We remember on June 2009 when 152 people died aboard a Yemenia Airlines Airbus A310-300.

We remember.

The years pass and the headlines fade, but the families will never forget the loss of their loved ones, and neither should we.

The pilot was about to land on an airstrip in the Comores Islands when ATC lost contact.

The plane and all its passengers crashed into the ocean.

12-year old Bahia Bakari survived the crash, rescued after thirteen hours in the ocean, clinging to debris. She was and is a miracle.

The final report on Flight 626 concluded the crash was due to the crew’s inappropriate flight control inputs that led to an aerodynamic stall.

Crews inappropriate flight control inputs means pilot error. In this case, the pilot error was exacerbated by a nineteen year old plane in dubious condition, bad weather conditions (winds gusting to 64 km/h 40 mph; 35 kn) and a primitive airport.

Crashes like Yemenia should function like red lights. The only way they make sense is if when they happen, we stop and consider those who were lost, and investigate what went wrong. On behalf of safety, the aviation industry should mandate to use every atom of what is discovered to prevent future accidents. The industry in general, and the airlines in specific should make changes that will help prevent future accidents.

Pilot error means airlines like Yemenia are still having problems training pilots. Still problems in the cockpit. Sadly, conditions still exist for more accidents like this in Yemenia’s future.

Crash in Nuevo Porvenir, 6 fatalities

A Cessna 206 Stationair #XB-MSN or XB-MSI en route from Aeropuerto Internacional de Tapachula MMTP to Hermanos Serdán International Airport in Puebla crashed thirty minutes after takeoff near Nuevo Porvenir, Guatemala.

The plane was not authorized to fly in Guatemala, and crashed near the border of Mexico. No homes, buildings or outbuildings were damaged. The plane crashed in a field.

Neither the pilot nor any of the five passengers survived. Everyone aboard was apparently Mexican. Identification of the bodies will be by DNA. One description says all the young men aboard were corpulent.

Locals heard the plane crash, but access to the scene is difficult, surrounded by trees ad crops in rural San Marcos, NW Guatemala. A radio broadcast by the mayor of Ixchiguán, Augusto Merida, said the small plane fell in Nuevo Porvenir. The plane is a total loss. One wallet was found with the name Eduardo Castillo Perez. Some luggage was also found. According to flight records:

Volunteer Firefighters told local radio that the identification was based on documentation from Nuevo Porvenir.

Authorities seized $2 million aboard the plane. The luggage contained a gun, jewelry and money in pesos and dollars.

The investigation is underway.

Helicopter Crash in Hickory County

A helicopter failed to return from a flight on May 24 2013 in Missouri. The flight took off at 5:45 p.m.

The Lucas Oil Products-owned helicopter was located crashed, and two people died in the crash.

The NTSB will arrive on site Sunday to begin the investigation. The pilot was flying passenger 21-yearold Missouri State University student Catalina Richard around the perimeter of a Lucas Oil Speedway event. The pilot’s name has hot been released as of this posting.

The helicopter crashed in rural Cross Timbers Missouri.

Russian Chopper Crew Lost in Peacekeeping Mission

UN Russian Mi-8 helicopter crashed at an altitude of 2,700 meters in the Democratic Republic of Congo killing four.

The UTair helicopter was returning to Bukavu when it crashed 35 miles from its destination.

The four crew members were Russian. The crew was en route between Shabunda and Bukavu when it hit bad weather.

The crash occurred in mountainous terrain that is inaccessible, but it has been located. Forty rescuers are en route on foot. The helicopter was part of peacemaking mission MONUSCO.

Cessna Crashes off Florida Coast

On Feb 14, a privately owned Cessna 310 en route from Okeechobee to Bartow, Florida went missing 15 miles west of Ft. Pierce, and was located west of Vero Beach Florida. Three passengers were aboard.

Paramedics flew in a Coast Guard Chopper to the crash site to recover the three victims; they were fatalities.

The cause of the crash is under investigation.

Woman Dies in Venezuela Crash, Survivors Rescued

On Feb 2, 2013, a privately owned Beechcraft 58 Baron with six aboard crashed in the Gulf of Cariaco, Venezuela.

The flight had taken off from Puerto Ordaz Airport and was en route to DelCaribe Gen S Marino Airport but suffered a complete loss of engine power. The pilot declared an emergency due to failures in both engines.

Fishermen found the plane located 15 nautical miles (28 km) east of Cumana in the Gulf of Cariaco.

Two of the passengers were reported to be minors.

There was one fatality, a sixty-two year old woman, Vetide Ramos, who died in the crash. Five survivors were brought ashore in a boat, including Ruben Herrera, Poztty Ramos, Jose Ramos, Ivan Herrera (a minor) and pilot Jhonatan Mata.

Canadian Antarctica Support Flight Missing

On January 23, 2013, a Kenn Borek Air de Havilland DHC-6 Twin Otter was en route from the South Pole to Terra Nova Bay when it went missing en route, somewhere around Northern end, Queen Alexandra Range, Antarctica.

Rescue Coordination Centre New Zealand is coordinating the search.

Three Canadians were aboard. The pilot is Captain Bob Heath, a Canadian with 25 years of Arctic and Antarctic experience. His plane was carrying survival tent and equipment and 5 day food supply.

The flight was a support mission from Amundson-Scott South Pole Base (USA) to an Italian base at Terra Nova Bay, Antarctica. A distress beacon sounded but a C-130 sweep sent by United States authorities at McMurdo failed to detect the plane. The beacon activated at around 10pm Wednesday night from the northern end of the Queen Alexandra Range, during hurricane force winds blowing slow below at temperatures below zero.

A DC-3 circled for five hours in heavy snow and 190 km winds.

A Twin Otter was scheduled to take off at around 8am NZ Time from McMurdo Station to fly over and establish a forward base approximately 50km from the mountainous area where the beacon is sounding, but weather may be prohibitive. A joint New Zealand and US field rescue team is planning a helicopter rescue as soon as weather permits.

Lawsuit Filed in Jenni Rivera Crash

The family of the victims (Arturo Rivera, makeup artist Jacob Yebale, attorney Mario Macias Pacheco and hair dresser Jorge “Gigi” Armando Sanchez Vasquez) of a 43-year-old Learjet that crashed in Mexico on December 9 have filed a negligence suit against Starwood Management LLC, Rodatz Financial Group Inc., McOco Inc. and Jenni Rivera Enterprises Inc.

Jenni Rivera was killed in the crash along with the 78 year old pilot and 20 year old co-pilot, neither of whom appeared to be licensed to operate the flight under the conditions (time and altitude) they were flying.

Moroccan Family Dies in Grenoble Plane Crash

On January 5, 2013, a twin engine plane hit a remote hill near Grenoble and crashed, killing five. The pilot was flying his wife and three children from Grenoble France to Morocco. The plane (probably a Piper Seneca) disappeared from radar not long after takeoff.

The family was returning home from a vacation in France. No one on the ground was hurt.

Witnesses in Saint Pierre de Bressieux saw the plane crash. Emergency services dispatched to the scene found 5 deceased victims and a burning plane.

Fashion Mogul Plane Missing in Venezuela

On January 4, 2012, a plane taking off at 11:00AM went missing at 11:30AM. The party was en route from the resort Los Roques Venezuela traveling 100 miles to Caracas and disappeared from radar. A search has been initiated for the private plane, a twin-engine Britten Norman BN2 Islander.

Listed on the flight were fashion mogul Vittorio Missoni who owned the plane, and his wife Maurizia Castiglioni, entrepreneur Guido Foresti and Elda Scalvenzi from Brescia and two crew members.

The search stopped at nightfall and began again on January 5.

NTSB Assists Government of Mexico in Aviation Accident

Washington – The NTSB is dispatching investigators to assist the government of Mexico in its investigation of the crash of a Learjet LJ25.

On December 9, 2012 at about 3:30 a.m. local time, the airplane crashed about 70 miles south of Monterrey, Mexico, due to unknown circumstances while en route to Mexico City, Mexico. All 7 persons on board were fatally injured including Latin singer Jenni Rivera.

NTSB Chairman Deborah A. P. Hersman has designated David Helson as the traveling U.S. Accredited Representative. Mr. Helson will be assisted from NTSB headquarters by investigative staff specializing in airplane airworthiness and flight crew operations as well as advisors from the Federal Aviation Administration (FAA) and Learjet.
