What: Cessna 210 Where: Poconé Municipal Airport, Brazil When: Nov 12 2011 Who: 4 aboard, 4 fatalities Why: Twelve miles from the airport, the pilot apparently lost control and crashed. The plane took off at 5:12 pm, and crashed 12 km later in the Pantanal region of Mato Grosso.
Martin Wagner, his son James, two mechanics named Dede and Alcindo died in the crash.
Wagner, who is a local cattle farmer, was reported missing by his family, after which a search plane was sent out and did not locate them. The charred wreckage of the plane was located the next day after sunrise, only minutes from the airport. The owner of the plane died in the crash.
What: Privately owned Piper PA-28-180 Cherokee B Where: Four miles south of Perryville, AR When: Nov 17 2011 Who: 4 aboard, 4 fatalities Why: Women’s basketball coach Kurt Budke and assistant women’s basketball coach Miranda Serna were on a routine recruiting trip to Arkansas, when the plane crashed in steep terrain in the eastern Ouachita Mountains in the Winona Wildlife Management Area. The other two aboard were Former State Senator Olin Branstetter and his wife, Paula. A local deer hunter’s wife told 911 when she saw the airplane nose-diving in a swirling pattern and crashing in the forest. By 6pm, the wreckage had been located. The passengers had been ejected from the plane, which drove a six foot deep furrow into a ridge but did not catch on fire. Rescuers reached the site on YO4 Forest Access Road via four wheelers.
Miranda Serna was in her seventh season as an assistant coach at Oklahoma State.
The cause of the crash is unknown but it was the catalyst of an outpour of support for Oklahoma State.
That the plane was co-owned by the Branstetters, who were from Ponca City. 82 year old Olin Branstetter was a longtime pilot.
Eleven years ago, a plane carrying 10 crashed, killing two OSU men’s basketball players, two members of its radio broadcast team, three members of the team’s support staff and two pilots in a snow storm in Byers, Colorado.

NTSB investigating Aviation Accident in Perryville, Arkansas

The National Transportation Safety Board (NTSB) has dispatched investigators to examine the circumstances of an aviation accident that occurred yesterday afternoon near Perryville, Arkansas, involving a Piper PA-28-180 (N7746W).
Jason Aguilera is the NTSB Investigator-in-Charge. He will receive headquarters support from air traffic control and meteorology. Parties to the investigation include the Federal Aviation Administration, Piper Aircraft and Lycoming.

What: Russian Emergency Chopper Mil Mi-8MTV-1 #RF-31129 Where: Altan, Trans-Baikal area Russia When: 09-OCT-2011 Who: 7 aboard Why: The missing chopper was located and all aboard are alive. 15 units of equipment and 115 rescuers were engaged in the search. When the flight failed to check in at 12:45, it was declared missing.
Three crew members and 4 Avialesookhrana representitives were all in good condition, found in the Trans-Baikal region of Kyrinskom. An emergency chopper picked up the 7 and flew them to the First Municipal Hospital in Chita.
What: PT Nusantara Buana Airlines CASA C-212 North Sumatra and Aceh, 19 year old plane Where: Bahorok, Indonesia When: Sept 29, 2011 Who: 15 passengers, 3 crew Why: The fate is known of the 18 passengers who went missing on a short Indonesian hop between North Sumatra and Aceh. The flight lost contact and crashed in the jungle canopy.
Rescuers reached the crash site via helicopter, and all the passengers were reported as casualties. Search and rescue teams found the passengers all strapped in their seats, lifeless.
Devastated family members accuse the airline of unprofessionalism. THe airline reports that rescue efforts were hampered by bad weather, terrible conditions, mountains and forest.
What: PT Nusantara Buana Airlines CASA C-212 North Sumatra and Aceh, 19 year old plane Where: Bahorok, Indonesia When: Sept 29, 2011 Who: 15 passengers, 3 crew Why: After 18 passengers went missing on an Indonesian flight after a flight is reported to have lost contact, and all were reported as casualties, the mother of 27 year old Samsidar Yusni said her daughter called her on the phone Thursday night, crying.
The PT Nusantara Buana Airlines plane is reported to have crashed in a mountainous area, at a midpoint in its route, in the mountainous Bohorok area about 70 kilometres north-west of Medan. Search and rescue teams have been deployed but did not reach the location of the wreckage, turning back due to difficult terrain and bad weather.
Thirty hours after the crash, part of the rescue team reported seeing a plane door being opened. After leaving Medan by truck, the trek on foot takes six hours, according to search teams. The plane is apparently mostly intact in the canopy. Chopper flyby photos show wings damaged and nose gone.
Before nightfall, a helicopter lowered two rescuers by rope about 300 meters yards from the wreckage but they are separated by a ravine. An open door seems to indicate there might be survivors.

The flight took off at 7:18 am, and was to land at 8:00, normally less than one hour. All aboard are being reported as fatalities.
The pilot was American.
The PT Nusantara Buana Airlines plane is reported to have crashed in a mountainous area, at a midpoint in its route. Search and rescue teams have been deployed. Though some reports say rescuers have found the wreckage, other reports say the wreckage is in difficult terrain and has not been reached.
There were 4 children and 14 adults aboard the flight. A local villager reported seeing the plane smoking and circling and passengers are presumed dead, but the report was accompanied by a photo of another crash site, so the source is dubious.
Minor Vidal, the sole survivor was found in the Amazon jungle. Minor said “I did not eat anything, nothing, just drank my urine when I was thirsty. The first night I slept on the plane and two nights in the jungle. I did not understand why the plane was flying so low. Then there was a heavy blow. I do not remember more. When I awoke, I saw the airplane game, could not move. I stayed in my seat, I was in the back of the plane. The next day [Wednesday], I went and did my blood with an arrow on a shirt to get me. They were all dead. I walked day and night to be found by the patrol.”
Colombian Oscar Valencia, the William Lara Ecuador and Bolivian indigenous leader Eddy Martinez died in the crash. Rescuers reached the crash site in the afternoon of Sept 8 and confirmed that there were survivors.
The wreckage of the plane was located 30km north-east of the city of Trinidad, in the Amazon jungle province of Beni. Chacon reported on thursday that a helicopter crew had seen the plane “completely destroyed” in a difficult-to-reach jungle region.
Six bodies were recoverable immediately when the wreckage was found. Two bodies trapped in the wreckage required rescue teams and equipment.

Where: Mountains South Of Tzaneen, South Africa When: August 14, 2011 Who: 12 aboard Why: Two planes which had been flying in the Tzaneen Air Show took off afterwards to ferry passengers to Johannesberg.
The flights were en route to Rand Airport in Germiston but did not arrive. The weather over Tzaneen was described as “bad.” The identities of the (missing)12-14 aboard have not been released and the planes have apparently not yet been found.
The pilot of one of the missing aircraft is businessman Gruar Brian of Johannesburg. His wife and two young children are believed to be aboard and Mr Kevin Woolocott Ordinance program coordinator of the air show was one of the passengers. The copilot was Peter Oldenburg.
Also presumed aboard are Mmes. Warden Louise Spence and Tess, photographer Frans Dely.
The Wolkberg area near Tzaneen and George’s Valley in Limpopo are being searched. The search for the planes with six helicopters and military aircraft was delayed due to poor visibility due to fog, and has resumed. The mountainous terrain has made the search more difficult.
Piaggio P166 planes are twin engine mediumsized aircraft, and ex air force planes.
What: Asiana Boeing 747-400 Cargo plane en route from South Korea’s Incheon International Airport and to Pudong China Where: southern resort island of Jeju, South Korea When: July 27, 2011 3:05am local time Who: two people Why: At 7600 feet, the crew reported control problems, and the aircraft’s transponder transmitted the emergency code. Sixty-seven minutes after takeoff, the crew reported fire in the cargo hold and lost contact with ATC. They were planning to divert to Jeju, but instead the plane crashed off of Jeju island. There were strong winds at the time of the plane’s disappearance. The captain was 51 and the first officer was 44; and pilot, Choi Sang-ki, and co-pilot, Lee Jung-woong are still missing.
One wing of the plane has been recovered, then the rest of the wreckage was found by a coast guard patrol boat 67 miles west of Jeju city. The 58 tons of cargo included amino acid solution, synthetic resin, lithium batteries, paint and other potentially dangerous material. A search for the black boxes is ongoing.
What: Thai military Black Hawk chopper Where: Myanmar border When: July 18, 2011 Who: 9 aboard Why: The chopper was on a search and rescue mission looking for 5 bodies in Saturday’s chopper crash in Krachan National park.
Those aboard the chopper were On board were Major General Tawan Ruangsri, the 9th Infantry Division chief and commander of the Surasee Task Force, Major Prapan Jiamsoongnoen, the 1st pilot, Major Chuphan Polwan, the 2nd pilot, Sargent Major Somkid Wongtoksaeng, the senior flight mechanic, Sargent Aram Wongsing, the second mechanic, Captain Jate Sudchai, Captain Chakraphan Bamrungphuet, Lance Corporal Itthisak Hinasuthi, and Sornwichai Kongtannukul, a TV Channel 5 cameraman.
While the chopper was en route, weather turned bad, and two of the three choppers on the mission returned to base. The third chopper was reported missing. Radio contact was lost at 11:55 a m.
What: Ernesto Ortiz(private owner) Cessna A185F Skywagon en route from Culebra to Ponce Mercedita Where: 1 mile from Yabucoa Puerto Rico When: July 1, 2011 Who: 5 aboard Why: When the plane failed to reach its destination, a search began for the lost Cessna. It was apparently lost in a storm.
The family was visiting from Hapeville Georgia and had flown to Puerto Rico for a day trip.
The missing were identified as Antonio Torregrosa Sánchez, 65, pilot of the ship, Sonia Torregrosa Torres (68 years), Edd Diorio (58), Rosarito Villa Gomez (19 years) and Mario Villa Gomez, 13. Coast Guard Cutter Key Biscayne and response boats from the San Juan boat station have continued to search, along with local authorities and Puerto Rican Emergency Management. The Federal Aviation Administration tracked the plane’s radar history to its last point of contact, approximately a mile off the Yabacoa coast. Only the remains of Maria have been found.
What: Mircea Chira’s Flight Design CTSW Where: Baru Romania When: June 13 2011 Who: pilot Why: The ultralight took off from Deva airport, and crashed 60 kilometers away in rough topography between Pui and Baru Mare. The plane was loaned by the owner, Mircea Chira, to a businessman with more than 200 flight hours.
The wreckage was found in a a rocky and forested area in Hunedoara county.
The 47-year-old German pilot who lived in the Romanian town of Orastie apparently sent a farewell letter which some have interpreted as a suicide note.
What: Pawan Hans Helicopters Eurocopter AS 350B3 Ecureuil en route from Tawang to Itanagar Where: Luguthang, India When: April 30, 2011 Who: 5 fatalities, including Dorjee Khandu Why: The helicoptor taking Dorjee Khandu from a retreat disappeared en route. Two MI-17 and two Cheetah helicopters were engaged in the search.
The helicopter wreckage was found 5 days after it went missing. Dorjee Khandu, Mr Khandu’s security officer Yeshi Choddak, Yeshi Lamu, and crew members JS Babbar and KS Malick all died in the crash. The wreckage was at 4900 meters, near Jung Falls in the Tawang district. The search was hampered by bad weather.
The chopper that crashed was 4 months old, according to Chief Minister Khandu’s adviser Kiren Rijiju. However, the maintenance department of Pawan Hans has a bad reputation.
What: Fuerza Aérea Boliviana Cessna T210 Turbo Centurion en route from El Alto International Airport in La Paz Where: Santa Elena River, Iquisiva Province, La Paz When: May 5 2011 Who: 6 aboard Why: The Bolivian Air Force plane piloted by the air force and carrying four United Nations workers was doing a fly-by and did not return at 2:30 as scheduled. The plane, which had been missing since May 5 and a search going on in Yungas (Coroico, La Asunta and Caranavi). The flight was located near the Santa Elena River, where it appears that it hit a tree during an emergency landing.
The passengers lost in the crash were Ivan Alfaro Santiago, Estephan Campos Ruiz, Mariela Romero Torreblanco and Patricia Delgado Rua, and the Cessna was being flown by Lt. Néstor Álvaro Ottich Arenas and Lt. José Javier Alegría Estrada.
What: Flight Simulation Center Grob G115 # D-EOFS Where: near Meggara Airport, Greece When: 29-APR-2011 Who: 2 aboard, 2 fatalities Why: The two aboard were on a training flight on the blue and white Grob G115 when the single engine plane crashed off the island of Spetses.
The instructor was a sixty year old Greek from Aigion, and the other was a non Greek man taking a flying lesson.
A marine taxi driver, Mr Lefteris Fostinis, was the first to discover the wreckage. A private diver found the two victims still in the cabin of the aircraft.
The bodies were retrieved and taken to Kranidi Medical Center.
What: Wanaka Helicopters Robinson R22 on a training flight from Wanaka to Haast and back Where: Arawhata Saddle, Wanaka South Island, New Zealand When: April 27 2011 Who: Pilot instructor and student Why: The training flight did not return to Wanaka as scheduled at 1:00; it’s last known position was near Bonar Glacier.
The beacon, one of two types aboard, was detected and the helicopter found at 9:00 am Thursday *local time.
Both pilot and student were fatalities, and the wreckage was found near the Olivine Wilderness Area in a stream at the head of the Arawhata River by search teams investigating an area near Mt Aspiring. The search was focused on Bow Peak in the “Waipara Saddle.”

The ultralight is based out of Port Allen Airport in Hanapepe. Rescue services with a boat, jet skis and the county’s new helicopter, Air-1 searched for five hours for survivors in overcast weather and murky water, and found only debris.
What: Central American Airways – CAA Let L-410UVP-E20 en route from San Pedro Sula-Ramon Villeda Morales Airport to Tegucigalpa-Toncontin Where: Santa Ana, Cerro de Hula Honduras When: Feb 14 2011 Who: 2 crew, 12 passengers, 14 fatalities Why: The Central American Airways flight from San Pedro Sula to Tegucigalpa departed from San Pedro Sula at 07:04 for the 40 minute flight to Tegucigalpa and crashed at 8:02 in a forested area.
Crash victims include labor leader, Israel Salinas Castellanos and Pedro Sula businessman Carlos Chahine, who served as minister Economy. The plane’s captain was Eduardo Ernesto Pach Pineda, and co-pilot Andersón Mejía. Salinas Elvir and Rodolfo Rovelo were also killed in the crash.

What: private Partenavia P.68C Victor plane en route from Caracas to Charallave, Venezuela Where: Lima Blanco, Western Venezuela When: Jan 7 2011 Who: 7 passengers: 6 fatalities, 10 year old girl survives Why: There were 7 passengers aboard when the Maiquetia control tower tracked the plane as heading to Cojedes.
The wreckage was found near Limo Blanco, in Cojedes. 4 adults and either one or two children died on the scene. A ten year old girl survived the crash and was taken to the San Carlos Hospital.
Pilot Said José Saldivia, Luis Oropeza, Edith Araujo, and Martha Araujo were the adults aboard the flight.
What: Beech 60 Duke en route from Brownwood Regional Airport, Texas to Colorado Where: 3 1/2 miles North of Edwards, Colorado When: Dec 15, 2010, 4:30 Who: 2 aboard, pilot Barton Harris and Jerry Hoggatt Why: While en route, the pilot lost radar contact with the Eagle County and Denver airports while making a final approach to the Eagle County Regional Airport in Eagle. Pilot, Barton Harris, 67, of Brownwood, Texas, and Jerry Hoggatt, 73, of Pelham, Alabama, were flying to ski in Colorado.
The wreckage was found by a National Guard OS58 helicopter crew 3.5 miles north of Edwards in steep terrain at an elevation of approximately 10,800 feet.
The plane is registered to Barton Harris of Brownwood, Texas.
Eagle County Airport Rescue and Fire Fighting, Eagle County Ambulance District, Eagle County Coroner’s Office, Eagle County Sheriff’s Office, Eagle River Fire Protection District, Garfield Search and Rescue, Greater Eagle Fire Protection District, the Salvation Army, Summit County Search and Rescue, Vail Mountain Rescue, Vail Police Department and Western Eagle County Search and Rescue assisted in the recovery.
Harris had spent 12 years as a Navy aviator. Harris was co-founder of W.T. Harris Company, a service firm providing warehousing, transportation, assembly work and wooden pallets, crates and other products.
What: Tara Air de Havilland DHC-6-300 Twin Otter en route from Lamidanda to Kathmandu Where: Okhaldhunga, Palunge Mountain, Nepal When: Dec 15th 2010 Who: 19 passengers and 3 crew Why: Five minutes after takeoff, Air Traffic Control lost contact with the plane.
The flight was scheduled to arrive in Kathmandu 35 minutes after departure and it never arrived.
The wreckage was found at 8850 feet, where it crashed into Palunge Mountain.
All the bodies have been recovered.
What: Monarch Aircraft Group (fort Lauderdale Florida) twin-engine Beechcraft 18 cargo plane en route from Fort Lauderdale to Nassau Where: Nassau When: December 14, 2010 Who: 2 passengers aboard Why: The cargo plane was en route to Nassau’s Lynden Pindling International Airport when it vanished from radar.
It crashed in the Atlantic, north of Nassau. A man’s body has been recovered. A U.S. Coast guard helicopter and 3 Royal Bahamas vessels are searching for the missing passenger. Neither has been identified.

He reported he was diverting to Chehalis but transmission stopped. Radar contact was lost at 7:44 am
A search is being conducted by a navy helicopter and a jet ranger. There is a weak beacon signal indicating the location.
What: Aviones Taxi AIFE Cessna 501 Citation ISP en route from Minatitlan to Veracruz Where: Sierra de Santa Martha Mexico When: Oct 6 2010 Who: 8 aboard Why: Flying in cloudy weather, the Citation 511-Alpha Extra TKY disappeared from radar into the Santa Martha mountains. Six senior executives of Coppel (Arturo Peraza, Alejandro Quintero Uzun, David Gerardo Soria, Luis Salvador Sánchez, Norma Leticia Torres and Brenda Camacho. ) and 2 pilots ( Javier Montoya, Bernardo Estrada) were on board, visiting a flood damaged property.
The plane disappeared near the Sierra de Santa Martha, in Soteapan. Three helicopters tracking the flight have not located the wreckage, which may be located in jungle of Los Tuxtlas
