    Sukhoi SuperJet-100 Demo Disaster Final Report In October

    On May 9, the Sukhoi SuperJet-100 piloted by Alexander Yablontsev was making a demo flight in Indonesia when the pilot took it dangerously close to terrain, and flew south into the mountains instead of turning north to the airport, crashing the plane and killing all 45 aboard the plane. The Russian Trade and Industry Ministry accepted the findings of local experts which blamed the crash on human error. Pilots ignored the collision avoidance system’s warning.

    The investigation is not officially over. The French and US reports are part of the investigation. The NTSB Preliminary report is brief:

    On May 9, 2012, at 1450 local time, a Sukhoi SJ100-95, Russian registration RP97004, collided with terrain about 35 miles south of Jakarta, Indonesia. The airplane was on a demonstration flight. The 40 passengers and 4 crew were fatally injured, and the airplane was destroyed.

    This investigation is being conducted by the National Transportation Safety Committee of Indonesia (NTSC/KNKT). As the state of design and manufacture of the the ACSS/L3 Terrain Warning and Traffic Collision Avoidance System (T2CAS), the NTSB appointed a U.S. accredited representative upon being invited to do so by the NTSC.

    Russian officials expect to make the final report public in October.

    Air New Zealand Pilot Collapsed, Died after Landing Dreamliner in Perth

    An Air New Zealand pilot collapsed shortly after she landed the Flight NZ175 full of passengers in Perth, Australia, on October 26.

    Air New ZealandIt is suspected that Captain Ann Barbarich, 59, one of the first female pilots to work for Air New Zealand, suffered a brain aneurysm, just minutes after landing the Boeing 787-9 Dreamliner. One of the 3 other pilots took controls and taxied the aircraft to the gate.

    Captain Barbarich was examined by 2 doctors aboard and was then rushed to the Royal Perth Hospital where she died 2 days later on October 28.

    None of the 233 passengers aboard was injured.

    According to Air New Zealand chief pilot David Morgan, “Ann was at the controls when the aircraft landed in Perth and the aircraft landed normally…Unfortunately after landing she became seriously ill. Another pilot took over and taxied the aircraft to the gate while the other pilots began to assist Ann. The aircraft was safe and under control at all times…Ann will be sadly missed by everyone at Air New Zealand. We have been actively supporting her family over recent days and will continue to do so. Our thoughts continue to be with them at this difficult time.”

    Aeroflot Flight Makes Emergency Landing due to Cargo Smoke Indication

    Aeroflot flight SU-1936 had to divert for an emergency landing in Kazan, Russia, on July 15th.

    The Airbus A320-200 plane heading from Sheremetyevo International Airport, Russia, to Karaganda, Kazakhstan, was diverted after the crew received a cargo smoke indication.

    The plane landed safely. There were one hundred and seventeen people aboard at the time; all of them remained unharmed.

    British Airways Flight Makes Emergency Landing at London Heathrow

    britishairwaysBritish Airways flight BA 676, on its way to Istanbul, Turkey, had to return and make an emergency landing at Heathrow Airport, London, shortly after it took off at about 11 a.m. on July 3.

    The decision to make an emergency landing was made after the crew reported a ‘burning smell’ in the cockpit of the Boeing 767-300. A hundred eighty-five passengers were aboard.

    Metropolitan Police Service and fire fighters were on alert when the plane landed back at the airport, just 35 minutes after it departed. No one was injured.

    The airline confirmed that the plane experienced a minor technical problem which is being resolved and it will soon return to service.

    TAROM Flight Returns to Romania due to Cracked Window

    TAROM flight RO-361 had to return and make an emergency landing at Henri Coanda International Airport, Romania, on June 12th.

    The Boeing 737-700 plane took off for Amsterdam, Netherlands, but had to turn back due to a cracked window.

    The plane landed back safely. All passengers and crew members remained unharmed.
