What: Amazonaves EMB-810C Seneca PT-EFS en route from Manaus to Santarem Where: Manaus When: April 21, 2011, 14:52 LT Who: 7 fatalities and a dog called Pimpolho Why: A minute after takeoff, an Amazonaves Air taxi which crashed at the threshold of the runway in the neighborhood of Tarumãt West Zone at 3 pm on Thursday had the owner, and his wife and son among those aboard. Antonio Neto Picão, his wife, Luciana, and a son, Matthew, 12 died in the crash and were buried in Boa Vista. Two daughters chose not to fly.
The four other fatalities were Pilot Waldeir Alencar da Silva,, Kelly Cidrônio, Janilça Oliveira Nascimento and Magno Souza. Santos was buried in Manaus, Alves in Minas Gerais and the two other passengers in Santarém. Janilsa and Magno Nascimento Souza were employees.
The cause of the crash has not been determined but Infraero confirmed that the plane was carrying 7 people, but only has a capacity for 5.
Serviço de Investigação de Acidentes Aeronáuticos (Seripa) expects a report in 30 days.
In 2002, an Amazonaves plane crashed in the Amazon;
In 2003, an Amazonaves crashed during a flight inspection;
In 2007, an Amazonaves crashed into a house in Manaus.
All of the crashes were determined to be due to mechanical failure.
The two engines and propellers of the Amazonaves Seneca (PT-EFS) will be sent to Sao Paulo where they will undergo deconstruction to be examined for signs of defect.

What: Rockford Memorial Hospital MBB BK 117 A-3 en route from Rockford to Mendota Where: Rochelle, Illinois When: Dec 10, 2012 8:30 p.m. Who: 3 aboard, 3 fatalities Why: A Rockford Memorial Hospital’s REACT helicopter crashed in a field at about 8:30 p.m. in Rochelle midway between Rockford and Mendota. Pilot Andy Olson, flight nurse Jim Dillow, R.N. and flight nurse Karen Hollis, R.N. died on the scene.
The medical helicopter was en route to Mendota to pick up a patient. No patients were aboard at the time of the accident.

What: Avis Amur AN12 AN-12 en route from Magadan to Keperveyem Where: Magadan When: Aug 9th 2011 Who: 11 crew Why: While en route, the plane developed an engine fire and radioed that they would be returning to Magadan. The fire is being attributed to a fuel leak. The flight crashed on the way back, disappearing from radar. Fog and adverse weather prevented rescue teams from being able to fly in. The 9 crew and 2 attendants have been reported missing. The plane was carrying 16 tons of food.
The cargo plane was being leased from KnAAPO. Rescuers took a ground route to the area of the crash. The emergency beacon went off 10 kilometers from the Omsukchan village. No survivors were found.
Avis-Amur is being investigated for a maintenance of safety violations, and there may be criminal charges pending.
What: CHC (for Total) Helicopter Eurocopter EC225 Super Puma en route from Aberdeen to West Phoenix drilling rig Where: 32 miles south of Shetland, off Fair Isle When: October 22, 2012 Who: 19 aboard, 0 fatalities Why: The helicopter that was ditched in the North Sea has been retrieved and taken to Peterhead, Aberdeenshire.
The UK Air Accidents Investigation Branch (AAIB) is investigating the incident in which seventeen passengers and two crew were on the way to the drilling rig when the crew had to ditch the helicopter (controlled ditching) in the North sea.

Summary:
This Special Bulletin contains information about the helicopter’s main gearbox (MGB) lubrication system and the results of an investigation into the indication of a failure of the emergency lubrication system, after it was activated by the crew. It follows publication of two earlier Special Bulletins on this accident, AAIB Special Bulletins S2/2012 and S3/2012.
This was the first occasion that the EC225 LP MGB emergency lubrication system had been operated in-service. One Safety Recommendation is made.
The investigation into the failure of the bevel gear vertical shaft in the MGB continues.
What: Indian army Aerospatiale SA-315B Alouette III en route from Mumbai to Bangalore Where: Goa When: October 15, 2012 Who: 3 aboard Why: During the approach to landing at the air base, one of the rotor blades broke off, initiating a fire.
Three aboard the flight died at the scene. Two of those aboard were officers.

What: Mexican Geological Services helicopter Where: northwestern state of Sinaloa 19 miles from the city of Los Mochis in the mountain town of Sinoloa de Leyva When: Wednesday Who: Three Mexican Geological Service employees: David Barbosa Luna, engineer managing SGM’s Western Region, Alberto Montañes Castro, project coordinator , and helicopter’s pilot Capt. Rodrigo Navarro Jimenez Why: 3 SGM workers managing field activities for the preparation of two geological mining charts were killed on the job. The flight was returning from the mountain and landed on or in the Sinoloa de Leyva Baseball stadium. They were en route to Sinoloa to refuel and check a faulty engine.
