Why: While en route, the helicopter sent out a distress signal and had to make an (unexplained) emergency landing/crash which destroyed the helicopter. There was one fatality and two crew members were severely injured. The helicopter was carrying 2 tons of food.

Saratov Airlines Flight Rejects Takeoff due to Engine Issue
Saratov Airlines flight 6W-9532 had to reject takeoff from Saratov, Russia, on March 22nd.
The plane was accelerating to takeoff for Surgut, Russia, when the crew received an engine indication and rejected takeoff.
The plane safely returned to the apron. All one hundred and seven passengers and seven crew members remained unharmed.

Why: After takeoff, the plane developed a crack in the windshield, and by the time it had been aloft for a few moments, the crack began spidering. The plane returned to Orlando and made a safe landing. Passengers were provided an alternative flight within a couple of hours—by 7 pm
Fighter Jet Crashes in Texas

Where: Trinity River, SW of Scurry, TX
When: Dec 13, 2012 10:30 a.m.
Who: 2 fatalities
Why: When the Aero Vodochody L-29 Delphin fighter jet impacted terrain, the two aboard, Fisher Floyd and Noell Rather were killed. The impact occurred near the juncture of the Trinity River and Highway 35, leaving a debris field for hundreds of feet.
Floyd won the flight in an auction. He leaves behind a wife and two kids.

What: Air Dolomiti Embraer ERJ-195 en route from Turin to Frankfurt
Where: Frankfurt
When: Dec 11th 2009

Why: On approach to Frankfort, the plane developed an electronic problems with the landing gear. The crew managed a safe landing, and a replacement plane was provided for passengers, while the original plane moved to maintenance.
The plane sustained tire damage.

What: Kuwait Airways Airbus 340-300 en route from Kuwait City (Kuwait) to London Heathrow
Where: Kuwait
When: Dec 6th 2010
Why: On takeoff from Kuwait City, the plane could not retract the landing gear. According to Al Abdulkarim, the procedure was routine, not an emergency. The crew returned to the airport, made a safe landing, and the problem was promptly repaired. The problem was apparently in the indicator, not the plane. After the warning light was repaired, the flight took off again within hours.
This is the third time in two months that Kuwait Airways made an emergency landing after take-off.
