On January 7, 2013, Chinook helicopter (registration N241CH) Pucallpa-Tarapoto was en route near the Laguna Yarinacocha in Pucallpa Peru when it came down in the Amazon.
The helicopter took off at took off at 2:57 pm, taking a load to Tarapoto.
According to reports, although the crew tossed the load while airborne, the plane broke up in mid air and caught fire. There were seven people aboard.
There were 2 Peruvians aboard: Joel Castillo and Luis Ramos.
Five Americans: Dan Immel , Ed Córdova, Jaime Pickett, Lee Bradford and Darrel Birkes.
Those aboard were employed by Petrominerales Ltd, a Canadian-based oil exploration firm.

What: Island Express Helicopter Service Where: Catalina Island When: May 24 2008 Who: 6 aboard, 3 fatalities Why: On May 24, 2008, at 0920 Pacific daylight time, an Aerospatiale AS-350-D helicopter, N67GE, operated by Island Express Helicopters, Inc., impacted terrain while landing at Two Harbors, Avalon, located on Santa Catalina Island, California, during a 14 Code of Federal Regulations Part 135 on-demand, commercial flight. The flight departed from the Queensway Bay Heliport, Long Beach, California, at 0907. The commercial pilot, a company employee, and one passenger were killed, and three passengers were seriously injured. The helicopter was destroyed by impact damage and fire

What: An Air India Airbus A330-200 flight en route from Tokyo Narita to Delhi Where: Delhi When: Sep 10th 2009 4:55 Who: 122 people on board 111 passengers and 11 crew Why: The pilot made a precautionary landing in Delhi when ten minutes prior to scheduled landing he had faulty readings of the plane’s hydraulics. The plane landed safely 25 minutes later, and was taxied safely. No injuries were reported.

No follow-up yet on this hydraulics issue. Was it the instrumentation? Was there a leak caused by lack of maintenance or too many flight hours squeezed in between maintenance checks? I’m sure Air India passengers would like to know.

What: Alitalia Boeing 767-300 en route from New York JFK to Rome Where: New York When: Jan 15th 2010 Who: 180 passengers Why: On take off, the Boeing apparently had a tail strike, but not one hard enough to damage the tail skid. The crew performed checks, but then decided to dump fuel and land. After circling, the flight landed safely approx. an hour and a half after initial takeoff. After two hours, the same aircraft took off again for Rome.

What: JetBlue Embraer 190 en route from Charlotte, N.C., to Boston Where: Boston When: Nov 17, 2010, 9 a.m. Who: 25 passengers Why: While en route, the plane developed a problem with the flaps. After declaring an emergency, the pilot made it safely to Logan, landing without flaps.
