    Metrojet Flight 9268 Crash: Egypt’s Investigation Committee Refers Case to Attorney General

    imagesEgypt’s independent investigation committee into the Metrojet flight 9268 crash has referred the case to the attorney general.

    The committee said on March 17th that the decision was made after they received an official report from Russia suggesting that “criminal activity” was behind the deadly crash that killed all 224 people aboard.

    The plane, heading to St. Petersburg, Russia, went down in Egypt’s Sinai Peninsula on 31st October last year.

    Colgan Air Reports Lack of Speed Warning

    Colgan’s Dec. 7 report to the U.S. National Transportation Safety Board said last February’s crash was probably caused by pilots’ “loss of situational awareness,” failure to follow the training and procedures, but pointed out contributing factors:

    • the cockpit warning system failed to adequately advise pilots when the speed is set below the calculated stall warning speed.
    • the lack of an adequate warning in the turboprop’s flight and operating manual regarding the effect of setting a non-ice reference speed during approach and landing
    • the crew’s failure to follow procedures regarding the proper response to a “stick shaker”
    • the crew’s failure to follow procedures regarding “non-pertinent conversation” by the flight crew during the descent and approach.

    Although Bombardier is withholding comment until the NTSB completes its investigation, a spokesman pointed out that the existing avionics are reliable and certified; and there are currently no requirements for systems to report abnormally low air speed.

    What: Continental Airlines Flight 3407, Bombardier Q400 turboprop operated by regional carrier Colgan Air en route from Newark, New Jersey to Buffalo Niagara International Airport
    Where: Clarence Center, New York.
    When: Thursday Feb 12 struck a house at 10: 10 pm. Two homes were affected.
    Who: 44 passengers and four crew members, 1 off-duty pilot, 1 person on the ground, all fatalities. The passenger manifest has not officially been released.
    Why: The New Jersey-to-Buffalo flight was cleared to land on a runway pointing to the southwest. But the plane crashed with its nose pointed to the northeast. Seconds after two automatic warnings to the pilots that the plane was not moving fast enough to stay aloft, the twin turboprop aircraft went through a “severe pitch and roll” after positioning its flaps for a landing. It did not dive into the house, as initially thought, but landed flat on the house. Icing is emerging as the possible cause for why flight 3047 fell from the sky. The flight data recorder has been collected and is currently being examined.

    3 in Cirrus Crash in Green Cove Springs, Florida


    What: Cirrus SR-20
    Where: Crashed in a wooded, swampy area just west of the runway at Reynolds Park in Green Cove Springs.
    When: Wednesday morning
    Who: Passenger Yi Jun Zhao, of Palm Coast, was transported to Orange Park Medical Center. Pilot Kevin Black and passenger Jian Feng Guo were not hurt.
    Why: The pilot radioed authorities about 7:45 a.m. to report trouble with his throttle. He was attempting a landing at the landing strip along the St. Johns River.

    Small Plane Crashes Near Belfast International Airport; 2 Dead

    A small plane crashed near Belfast International Airport, Northern Ireland, on April 19th.

    The plane was carrying two people when it crashed between Nutts Corner and Loanends in County Antrim.

    Both occupants of the plane were killed in the crash.

    The cause of incident is being investigated.

    4 Die in a Single Engine Piper Crash

    piperlausanneFour people were killed after a single engine Piper 32 plane, en route from Hereford, Texas, to Gettysburg, South Dakota crashed in foggy weather, in a wind farm. According to FAA records, the plane was registered to Donald J. Fischer, from Gettysburg, South Dakota.

    The incident occured, when the passengers of ill-fated airplane were returning after attending a sale of live cattle and embryos in Hereford. The wreckage of crashed plane was found on Monday, April 28, at the South Dakota Wind Energy Center, located towards south of Highmore and containing 27 turbines, having a height of 213 feet, excluding the blade length.

    Three passengers and the pilot were pronounced dead in the incident. Luce Funeral Home confirmed the pilot’s identity as Fischer, 32, while Lien Funeral Home revealed identities of 2 passengers as Logan Rau, hailing from Java and Brent Beitelspacher, belonging to Bowdle. The identity of third passenger is not known at the moment.

    A spokesperson of Florida-based NextEra Energy Inc., Steve Stengel confirmed that the incident caused damage to turbine. He also said, “It’s been so foggy up there and we haven’t had a chance to investigate.”

    NTSB is investigating the cause of accident.

    Mil Crash in Borneo

    An Indonesian Army Mil Mi-17B-5 en route to a construction site so remote it can only be reached by helicopter was flying from Tarakan to Malinau with a crew of construction workers and a 1,100 pound construction cargo when the helicopter developed engine trouble.

    Eight crew and eleven civilian construction workers went down in a jungle in Borneo, thirteen of whom died on impact.

    Four passengers and two crew survived. Eight of the fatalities were civilian construction workers, and five were military crew.

    The helicopter burned out on the ground near Bulan. The survivors suffered burns.
