  • Special Flight Rules

    July 1–The FAA announced today a number of changes to the restrictions governing the Special Flight Rules Area surrounding the nation’s capitol, effective September 1.

    The most significant change will affect pilots who lose radio contact with controllers while flying under visual flight rules after departing from an airport located within the Special Flight Rules Area (a circle of airspace extending 30 miles out from Ronald Reagan Washington National Airport). Under the current rules, those pilots must squawk 7600 on their transponders and immediately leave the Special Flight Rules Area by the most direct route. The new rule will allow pilots to return to their departure airport if it is closer than the Special Flight Rules Area boundary. This means pilots might not have to fly longer distances to airports outside the Special Flight Rules Area. Pilots who lose radio contact after departing from an airport located within the Flight Restricted Zone (an inner ring extending 15 miles out from National Airport) may return to that airport if it is within five nautical miles. If neither condition applies, pilots must leave the Special Flight Rules Area by the most direct route.

    To improve safety, the FAA added a speed limit of 180 knots within the Special Flight Rules Area for aircraft flying under visual flight rules. This will complement the existing 230 knot speed limit for aircraft flying under visual flight rules in airspace extending 30 miles beyond the outer ring of the Special Flight Rules Area.

    The agency made several clarifications to make it easier for pilots to file flight plans and understand radio requirements. The FAA added a phone number (866-225-7410) for pilots to call to file flight plans for the Flight Restricted Zone. Pilots will be asked to use their confidential pilot identification codes or their waiver numbers.

    The FAA also clarified that aircraft flying within the Special Flight Rules Area must be equipped with a functioning two-way radio capable of communicating with controllers on the appropriate frequencies or UNICOM. The agency strongly recommends that pilots continuously monitor VHF frequency 121.5 or UHF frequency 243.0 for emergency instructions while flying in the Special Flight Rules Area.

  • NTSB Safety Recommendations

    The National Transportation Safety Board makes the following
    recommendations to the Federal Aviation Administration:
    Work with the military, manufacturers, and National
    Aeronautics and Space Administration to complete the
    development of a technology capable of informing pilots
    about the continuing operational status of an engine. (A-10-
    62)

    Once the development of the engine technology has been
    completed, as asked for in Safety Recommendation A-10-62,
    require the implementation of the technology on transport-
    category airplane engines equipped with full-authority
    digital engine controls. (A-10-63)

    Modify the 14 Code of Federal Regulations 33.76(c) small and
    medium flocking bird certification test standard to require
    that the test be conducted using the lowest expected fan
    speed, instead of 100-percent fan speed, for the minimum
    climb rate. (A-10-64)

    During the bird-ingestion rulemaking database (BRDB) working
    group’s reevaluation of the current engine bird-ingestion
    certification regulations, specifically reevaluate the 14
    Code of Federal Regulations (CFR) 33.76(d) large flocking
    bird certification test standards to determine whether they
    should 1) apply to engines with an inlet area of less than
    3,875 square inches and 2) include a requirement for engine
    core ingestion. If the BRDB working group’s reevaluation
    determines that such requirements are needed, incorporate
    them into 14 CFR 33.76(d) and require that newly
    certificated engines be designed and tested to these
    requirements. (A-10-65)

    Require manufacturers of turbine-powered aircraft to develop
    a checklist and procedure for a dual-engine failure
    occurring at a low altitude. (A-10-66)

    Once the development of the checklist and procedure for a
    dual-engine failure occurring at a low altitude has been
    completed, as asked for in Safety Recommendation A-10-66,
    require 14 Code of Federal Regulations Part 121, Part 135,
    and Part 91 Subpart K operators of turbine-powered aircraft
    to implement the checklist and procedure. (A-10-67)

    Develop and validate comprehensive guidelines for emergency
    and abnormal checklist design and development. The
    guidelines should consider the order of critical items in
    the checklist (for example, starting the auxiliary power
    unit), the use of opt outs or gates to minimize the risk of
    flight crewmembers becoming stuck in an inappropriate
    checklist or portion of a checklist, the length of the
    checklist, the level of detail in the checklist, the time
    needed to complete the checklist, and the mental workload of
    the flight crew. (A-10-68)

    Require 14 Code of Federal Regulations Part 121, Part 135,
    and Part 91 Subpart K operators to include a dual-engine
    failure scenario occurring at a low altitude in initial and
    recurrent ground and simulator training designed to improve
    pilots? critical-thinking, task-shedding, decision-making,
    and workload-management skills. (A-10-69)

    Require 14 Code of Federal Regulations Part 121, Part 135,
    and Part 91 Subpart K operators to provide training and
    guidance to pilots that inform them about the visual
    illusions that can occur when landing on water and that
    include approach and touchdown techniques to use during a
    ditching, with and without engine power. (A-10-70)

    Work with the aviation industry to determine whether
    recommended practices and procedures need to be developed
    for pilots regarding forced landings without power both on
    water and land. (A-10-71)

    Require applicants for aircraft certification to demonstrate
    that their ditching parameters can be attained without
    engine power by pilots without the use of exceptional skill
    or strength. (A-10-72)

    Require Airbus operators to amend the ditching portion of
    the Engine Dual Failure checklist and any other applicable
    checklists to include a step to select the ground proximity
    warning system and terrain alerts to OFF during the final
    descent. (A-10-73)

    Require Airbus operators to expand the angle-of-attack-
    protection envelope limitations ground-school training to
    inform pilots about alpha-protection mode features while in
    normal law that can affect the pitch response of the
    airplane. (A-10-74)

    Require all 14 Code of Federal Regulations Part 139-
    certificated airports to conduct wildlife hazard assessments
    (WHA) to proactively assess the likelihood of wildlife
    strikes, and, if the WHA indicates the need for a wildlife
    hazard management plan (WHMP), require the airport to
    implement a WHMP into its airport certification manual. (A-
    10-75)

    Work with the U.S. Department of Agriculture to develop and
    implement innovative technologies that can be installed on
    aircraft that would reduce the likelihood of a bird strike.

    Require Airbus to redesign the frame 65 vertical beam on
    A318, A319, A320, and A321 series airplanes to lessen the
    likelihood that it will intrude into the cabin during a
    ditching or gear-up landing and Airbus operators to
    incorporate these changes on their airplanes. (A-10-77)

    Conduct research to determine the most beneficial passenger
    brace position in airplanes with nonbreakover seats
    installed. If the research deems it necessary, issue new
    guidance material on passenger brace positions. (A-10-78)

    Require, on all new and in-service transport-category
    airplanes, that cabin safety equipment be stowed in
    locations that ensure that life rafts and/or slide/rafts
    remain accessible and that sufficient capacity is available
    for all occupants after a ditching. (A-10-79)

    Require quick-release girts and handholds on all evacuation
    slides and ramp/slide combinations. (A-10-80)

    Require 14 Code of Federal Regulations Part 121, Part 135,
    and Part 91 Subpart K operators to provide information about
    life lines, if the airplane is equipped with them, to
    passengers to ensure that the life lines can be quickly and
    effectively retrieved and used. (A-10-81)

    Require that aircraft operated by 14 Code of Federal
    Regulations Part 121, Part 135, and Part 91 Subpart K
    operators be equipped with flotation seat cushions and life
    vests for each occupant on all flights, regardless of the
    route. (A-10-82)

    Require 14 Code of Federal Regulations Part 121, Part 135,
    and Part 91 Subpart K operators to brief passengers on all
    flotation equipment installed on an airplane, including a
    full demonstration of correct life vest retrieval and
    donning procedures, before all flights, regardless of route.

    Require modifications to life vest stowage compartments or
    stowage compartment locations to improve the ability of
    passengers to retrieve life vests for all occupants. (A-10-
    84)

    Revise the life vest performance standards contained in
    Technical Standard Order-C13f to ensure that they result in
    a life vest that passengers can quickly and correctly don.

    Conduct research on, and require 14 Code of Federal
    Regulations Part 121, Part 135, and Part 91 Subpart K
    operators to implement, creative and effective methods of
    overcoming passengers? inattention and providing them with

    OAG Reports Air Travel Growth, Over 285 Million Seats Offered Worldwide

    WASHINGTON, Feb. 10, 2011 /PRNewswire/ — OAG (www.oag.com), the global leader in aviation intelligence reports that worldwide scheduled airline capacity increased 5% in February, year-on-year, to a total of 285.7 million seats. The number of flights increased 4%, to 2.3 million departures worldwide during the month.

    In its monthly Frequency and Capacity Trend Statistics (FACTS) report, OAG finds all regional markets recorded year-on-year growth in February, with the exception of capacity to and from Central and South America. This region lost 3% of its seat capacity, when compared to the same month last year, feeling the impact again this month of the loss of Mexicana services.

    Although capacity fell in Central and South America, overall; within Lower South America, scheduled capacity increased 12% year-on-year. The Brazilian market is showing the strongest growth in the region, with a 14% increase in domestic capacity in February compared to the same time last year.
    “The current expansion in some South American markets may be at the height of a growth period. The impending rationalization of carriers in the region, and the development of larger alliances such as that proposed by the LAN Group and TAM, could lead to capacity consolidation as network rationalization occurs,” said Peter von Moltke, Chief Executive Officer, UBM Aviation.

    Year-on-year, the two fastest growing markets in the world are those to and from the Middle East, and to and from Asia Pacific, in terms of frequency of service. The total number of flights offered to and from the Middle East grew 13% to a total of 49,014; flights to and from Asia Pacific increased 13% to a total of 55,965.

    Scheduled frequency and capacity to and from Europe was the second fastest growing region among the largest markets with scheduled capacity increasing by 11%, to a total of 21.3 million seats; frequencies increased 10%, to a total of 93,558. Growth within Europe, however, increased a modest 2% in both seat capacity and the total number of flights to a total of 59.5 million seats and 493,150 flights.

    “Medium to long haul carriers continue to build their presence in European markets, with the objective of securing greater shares of the longer haul markets, which traditionally deliver higher yields,” continued von Moltke. “Carriers such as Emirates, Etihad and Qatar Airways will continue to increase flights and open new markets as their development progresses, and increased frequencies to a number of European destinations are expected by these airlines throughout the year.”

    The number of scheduled services both within and to and from North America remained constant year-on-year, although a slight increase in average capacity per flight resulted in more seats being offered. Total capacity to and from this region increased 3%, to a total of 15.5 million.

    Air Canada Plane Makes Emergency Landing in Toronto

    Air CanadaAir Canada flight 549 made an emergency landing in Toronto, Canada, on January 29.

    The plane, en-route from Newark, New Jersey, to Vancouver, Canada, was diverted after it lost cabin pressure due to a fault in an air conditioning unit.

    The Airbus A319 plane landed safely. All 107 passengers aboard remained unhurt.

    Mystery Boeing Crash in Nunavut Canada

    What: First Air Boeing 737-210C en route from Yellow Knife to Resolute
    Where: near Resolute Airport, Nunavut, Canada
    When: August 20, 2011
    Who: 4 crew, 11 passengers, 12 fatalities
    Why: On approach to Resolute Bay Airport, the plane impacted the hillside. The wreckage was found five miles from the airport.

    Three survivors were recovered. There were twelve fatalities, including the entire crew. Two survivors (a seven year old and a 48 year old) were taken to Ottawa hospital, and the third (a 23 year old woman) is reported to be stable at Qikiqtani General Hospital in Iqaluit where all of the survivors were originally taken. Two forensic teams are on the scene (one to identify the bodies, and one for accident investigation), and the black boxes have been recovered.

    The weather at the time of the crash was he weather was rainy with a low cloud ceiling, and reported to be patchy fog. A mock rescue military exercise was going on with 700 military personnel when the crash was reported. They responded to the crash with helicopters and medical personnel. Eleven RCMP personnel were also part of the rescue unit. The senior administrative officer, Ralph Alexander, said it wasn’t foggy enough in Resolute at the time of the crash to have caused the accident, and that planes landed there in worse visibility. However there have been reports of locals hearing the impact but not seeing anything because of dense fog. The charred wreckage is in three parts with smaller plane parts and remains scattered.

    The crash site is less than two kilometres west of Resolute, a small tight-knit community with many individuals directly affected by the twelve fatalities.

    The passenger manifest has not yet been made public but the owner of South Camp Inn, Aziz (Ozzie) Kheraj, reportedly lost a granddaughter on the crash, and one survived.

    First Air held a news conference at in Kanata, Ontario on Sunday at 11 a.m. ET.

    Transport Canada issued the following release:


    At approximately 12:30 EST, First Air Flight 6560 from Yellowknife to Resolute Bay crashed on approach to Resolute Bay. There were 15 passengers on board. At this time, 12 people are thought to be dead and three injured. Our thoughts go out to those affected by this tragic accident.

    Transport Canada has appointed a Minister’s Observer, who will keep TC informed of the investigation’s progress. We will cooperate fully with the Transportation Safety Board who is currently on site investigating this accident.

    The Canadian Armed Forces are in Resolute Bay for Exercise Operation Nanook 2011 and are responding in support of emergency operations to the situation.

    On Sunday August 21, 2011, First Air Vice President of Marketing and Sales, Chris Ferris, made the following statement to the media from the First Air Head Office in Kanata:

    As you are aware, yesterday at 12:40pm First Air experienced a tragic loss of
    charter flight 6560. We are deeply saddened by this tragedy and offer our sincere condolences to the families.

    The details we have at this time are as follows: a First Air Boeing 737-200 combi travelling from Yellowknife to Resolute Bay, went down 5 miles from the airport. The flight was carrying 11 passengers and four crew. There were 12 fatalities including all four crew members.

    Our first priority has been contacting and looking after the families of passengers and crew. Out of respect for the passengers, our crew, and their families, we are not releasing any names or passenger details.

    The three injured passengers were Medivac’d to Iqaluit for further treatment last evening. Two of the three passengers have since been moved to Ottawa, while the third remains in Iqaluit in stable condition.

    At this time the cause of the accident is unknown. The Transportation Safety Board is conducting their investigation and First Air is cooperating fully.

    Our thoughts and focus are with the families and friends of the passengers and crew; and the community of Resolute Bay. Field teams including counsellors, have been deployed to provide support in Resolute Bay, Yellowknife and other main stations in our network.

    We would like to thank the Canadian Military whose on-site presence and immediate response was instrumental in the rescue efforts.
