    Where: 3 miles south of Dana Point, California
    When: November 19, 2005
    Who: Pilot and 3 passengers
    Why: The airplane impacted the ocean following a departure from controlled cruise flight, decelerating from 120 to 60 knots spiraling down corkscrew manner before impact. No pre-accident anomalies were noted with any systems examined, and no distress calls or communications preceded or followed the upset. The pilot had moderate to severe coronary artery disease with evidence of at least one prior small heart attack. The combination of coronary artery disease and LVH would have substantially increased the risk for sudden cardiac death in this pilot. Four died in the crash.

    NTSB sending Team to Peru Crash Investigation

    WASHINGTON – The National Transportation Safety Board is sending a team of investigators to Pucallpa, Peru, to assist the Government of Peru with its investigation of yesterday’s crash involving a Boeing helicopter. According to the U.S. Department of State, the accident claimed the lives of five American citizens.

    On Monday afternoon, in Pucallpa, Peru, a Boeing-Vertol 234 helicopter, operated by the U.S. operator Columbia Helicopters, crashed shortly after takeoff. The helicopter had departed from FAP Captain David Abenzur Rengifo International Airport, Pullcapa, Peru enroute to Tarapoto, Peru. It has been reported that all seven persons aboard the aircraft sustained fatal injuries.

    The NTSB has designated senior air safety investigator, Paul Cox, as the U.S. Accredited Representative. He will be accompanied by two NTSB investigators with expertise in helicopter systems and operations, a representative from the Federal Aviation Administration, and a representative from Columbia Helicopters. The team is expected to arrive in Peru tonight.

    SkyWest Airlines Flight Makes Emergency Landing in Syracuse

    SkyWest Airlines flight OO-7390 had to divert and make an emergency landing in Syracuse, New York, on December 17th.

    The plane heading from Detroit, Michigan, to Ithaca, New York, was diverted due to an issue with the flaps.

    The plane landed safely. All passengers and crew members remained unharmed.

    Sao Paulo Crash Sunday Nov 4

    Reali Taxi Aero’s small executive Learjet 35 took off in the rain at 14:10 local from Campo de Marte Airport in Sao Paulo and crashed into a house in the Casa Verde neighborhood half a kilometre from the runway after taking off. Eight people died; the bodies of four men, three women and a baby were found in the plane wreckage.

    Three others were rescued, including Claudia Lima Fernandes, 30,who was badly burned, and an 11 year old girl.

    Other neighborhood houses had to be evacuated and are being evaluated for safety.

    Witnesses said that the jet, which took off from Campo de Marte for Rio de Janeiro, ploughed nose first into the houses when it seemed to attempt a return to the airport.

    This was Sao Paulo’s second large air tragedy this year. On July 17, a TAM Airbus crashed while landing at the Congonhas airport, killing 199 people.

    São Paulo is known for its large helicopter fleet. Businessmen often fly the vast city by helicopter to avoid traffic and security threats .However last Monday three helicopters crashed in Sao Paulo in two hours.

    Three people died. Five days earlier, after having mechanical problems, an air force plane crashed north of Sao Paulo, There were no casualties.

    Chinese Court Jailed the Pilot Over Deadly Henan Airlines Plane Crash

    A Chinese court has sentenced the pilot of the Henan Airlines flight which crashed in Yichun, Heilongjiang province of China in 2010, killing 44 people.

    It is believed that the ill-fated Embraer E-190 plane crashed in heavy fog after the main pilot, Qi Quanjun, decided to land without being able to see the runway properly. There were 91 passengers aboard the plane, around half of which were government officials. Quanjun survived the accident with serious facial injuries.

    After more than 4 years, a local Chinese court, on December 19, has ordered to jail the pilot for 3 years on the grounds of ‘causing a major airline accident’ and not properly evacuating the passengers.
