What: AEROCLUBE DE PIRACICABA PIPER PA-34-200 Seneca I Where: Morro do Cristo village suburb of Sao Paulo, Brazil When: May 16, 2010 Who: 4 fatalities Why: The plane, which had six seats, was used to give flying lessons. It was returning from the city of Sao Jose do Rio Preto, when it crashed near a village outside the Brazilian city of Sao Paulo, killing its four occupants at 11 pm Monday, just after the pilot lost contact with Pirassununga control tower.
Searches were performed by a C-105 Amazonas plane, and a Military Police helicopter. The missing plane was found in dense forest between São Pedro and Itaqueri.
The bodies of Instructor Job de Oliveira, Jean Carlos Capelin (30, of São José do Rio Preto), Rodrigo Matos Gomes (27 of Recife ), and Diego Pereira da Costa (27 of Taguatinga) were identified today, and recovered.

What: Pawan Hans Helicopters Ltd Mil Mi-172 en route from Gauhati to Tawang India Where: Tawang India When: April 19, 2011, 13:57 Who: 23 aboard, 17 fatalities Why: The Pawan Hans helicopter Captain Varun Gupta and Captain A. K. Tiwari was flying was approaching the helipad to land in Arunachal Pradesh when it struck a wall, broke into pieces, burst into flame and fell down into a gorge by the helipad.
Six survived, three of whom are critical. All—two crew members and four passengers who survived —are badly burned.
The deceased were identified as Mrs Anita, A Baruah, Dr Tendon, Dr Asif, Mrs R Asif, Mrs Jahara, Ms Asif, Master Asif, Mr A Sharma, A K Saraugi, Ms N Botha, Col Sharma, Mr W Bhatia, Mr T Mustafa, second pilot captain Tiwari, flight attendant A Dixit and assistant maintenance engineer S B Kulkarni.
Survivors include Captain Varun Gupta, Randiv Kumar Chaturvedi, Dorjee Wangdi, Rishi Bothra, Rajendra Pal and Mrs Karishma Saraugi.
The injured were taken to Tawang hospital and on the 20th will be air-lifted to Guwahati.
This was a regular flight to Tawang from the Lokopriya Gopinath Bordoloi International Airport in Guwahati, and the helicopter has a valid certificate of airworthiness.
The DGCA speculates the chopper crashed was “likely” due to wind shear and down draft.

A 2006 Robinson R22 Beta helicopter made a crash-landing near West Riviera Drive and West Alhambra Avenue in Lindenhurst, Long Island, New York, on January 27.
The incident happened after the helicopter, carrying two people, lost power while returning to the Republic Airport. According to Frank Delia, of the Academy of Aviation, “It was a standard training flight…They were coming from the south practice area, which is off the south shore of Long Island, on their way back to Republic Airport after a successful training flight.”
Both people aboard the aircraft, including the pilot Joseph Lombardo, 26, and his student Ming Chen, 26, remained uninjured.

A single engine Cessna 182G crashed in a field near Cattle Creek Road and the Rio Grande Trail in Carbondale, Colorado, on September 27.
Authorities said the plane went down shortly after taking off from Glenwood Springs Municipal Airport. According to witnesses, the plane seemed to be experiencing engine issues before it crashed.
There were 2 people aboard at the time; both of them were injured and were shifted to Valley View Hospital. They were identified as the pilot Benoit de la Vaissiere and his passenger Dmitre.
According to the Sheriff’s Office, the aircraft’s owner is “responsible for the salvage and reclamation of the airplane from the site.”

A single-engine Cessna 172 and a twin-engine Sabreliner collided midair near Brown Field Municipal Airport in San Diego, California, at around 11 A.M. on August 16.
The accident happened near Otay Mesa Road and Harvest Street when both aircrafts were approaching Brown Field Municipal Airport.
Five people were killed in the crash. Their identities are not known at the moment.
The FAA and the NTSB are investigating.
