Why: Witnesses saw the plane fly into the storm. The plane crashed during a storm and burned.

What: Kenya Police Air Wing Eurocopter AS 350B3e Ecureuil Where: Ngong Hills, near Nairobi Kenya When: June 10, 2012 Who: 6 aboard Update: The Kenyan Commission of Inquiry established to look into the June 10 helicopter crash in the Kibiku Forest in Kenya is set to hold its first formal session on July 16. The container holding the wreckage will be stored at the Kenya Police Air-Wing facility at the Wilson Airport.
The Kenya Police Airwing Unit Commandant Rodgers Mbithi will be appearing as the first witness.
The pilots involved, Captain Nancy Gituanja and Luke Oyugi, underwent pilot training in the Ukraine in April 2009.
According to commission members, the intent of the commission is “…not to apportion blame, but to prevent occurrence.”
Fatalities included Minister of State for Provincial Administration and Internal Security George Saitoti and his permanent secretary Orwa Ojode, pilots Luke Oyugi and Nancy Gituanja and bodyguards Inspector Joshua Tonkei and Sergeant Thomas Murimi.

What: Rusline/Rusair Tupolev TU-134A en route from Moscow to Petrozavodsk Where: outskirts of the Besovets village, Petrozavodsk, Russia When: Jun 20 2011 at 11:40 pm Who: 43 passengers and 9 crew N Why: On approach to the airport, the plane struck the ground about 2600 feet shy of runway 2. On impact, the plane burst into flame, and skidded into the gardens of a residential area, although missing residences.
Initially eight people survived, but on the 21st, a nine year old boy died, a woman died on the 25th, and a man died on the 26th.
What: Twin-engine Cessna 421B on approach to Chicago Executive Airport in Wheeling Where: crashed into a storage yard When: January 2006 Who: Michael Waugh ( general manager and chief operating partner of Joe’s Seafood, Prime Steak & Stone Crab in Chicago), Kenneth Knudson (founder of Sybaris), and Scott Garland (financial adviser at Morgan Stanley) Why: Circuit Judge John Ward awarded $6 million to Waugh’s widow, with the rest being divided among the couple’s three young sons.
Wrongful death suiit: Morgan Stanley improperly allowed employees to fly personal planes to conduct business–a situation which other financial institutions prohibit.
Pilot Mark Turek, a Morgan Stanley senior vice president was charged with negligence.
What: American Airlines Boeing 757 en route from Aruba to Miami Where: Miami When: July 10, 2012 Who: 185 passengers, 6 crew, 12 injured Why: While en route thirty minutes from landing, the flight encountered some turbulence. Fifteen seconds worth of turbulence was enough to knock people around. People were knocked out of their seats, and at least one person hit the ceiling. Some of the injured were treated at the airport and some at a Miami hospital.

On April 4, 2013 about 12:30 am, a Leair Charter Service Limited Cessna 402C was landing in the process of a medical evacuation and collided with a parked truck being used to illuminate the runway.
The pilot and passenger in the plane survived.
The vehicle caught on fire, killing three occupants. The fatalities are named in the video.
Additionally, the video calls for the need for lights on the runway.
Official statement: Tragic aviation accident in Mayaguana

In the early hours of this morning I was informed of an aviation accident in Mayaguana resulting in the tragic death of 3 people. I am advised that all 3 persons are residents of the island of Mayaguana. A preliminary report from the Department of Civil Aviation says that on 4th April, 2013 at approximately 12:30 a.m. a Cessna C402 owned by Leair Charter Service Limited with a crew of 2 on board was making a landing at night for an emergency evacuation flight with two vehicles strategically placed to outline the useable portion of the runway. It is reported that the aircraft made contact with one of the vehicles resulting in the deaths of the 3 passengers in that vehicle.
An investigative team comprising the Royal Bahamas Police Force and the Air Accident Investigation and Prevention Unit of the Department of Civil Aviation will fly into Mayaguana at 5 a.m. this morning.
I wish to assure that a full investigation will be carried out by the Department of Civil Aviation into the causes and circumstances surrounding this terrible event. I will report further as information permits. Out of respect for the families, the names of the persons killed will not be disclosed until an official release is made by the Royal Bahamas Police Force.
I extend my heartfelt condolences to the families of these beloved persons and to the wider community of Mayaguana who share in this loss.
