What: Vision Airlines Dornier Do-328-100 en route from Atlanta to Louisville Where: Louisville When: May 11th 2011, 8 pm Who: 21 aboard Why: After takeoff from Atlanta, the pilots experienced hydraulic problems, which they attempted to amend while en route. They announced the issue to passengers who were somewhat anxious on the way, but apparently did not panic. On approach to Louisville, a flyby revealed the landing gear was down. They continued to make a safe landing, leaving skidmarks on the runway, although without nose gear steering, they veered off the runway. After the landing, one passenger was hospitalized due to a medical condition. The runway is closed pending the investigation.

What: Kyrgyzstan/Altyn Air Tupolev 134A-3 en route from Bishkek to Osh Where: Osh Airport, Kyrgyzstan When: 28 DEC 2011 Who: 6 crew, 82 passengers, 25 injured Why: After a hard landing in dense fog, the plane skidded off the runway, collapsing the right main gear, and flipped. As it rolled over, the right wing separated, and the ensuing fire was quickly contained by rescue services standing by. Passengers inside were hanging upside down, suspended by their seatbelts, but had to find a way to evacuate quickly, before the plane had a chance to explode. A fire occurred from a fuel leak in the remaining wing, but it was controlled.

Six crew members and nineteen passengers were hospitalized, but there were no fatalities reported. Nine children were aboard. The plane was evacuated.
Because of the incident, the airport (runway) was temporarily closed, and Osh-Bishkek flights are delayed.
Of the 25 reported injured and hospitalized, these have been identified: Rustamov A. b. 1977, Ismatova A. b. 1993, Turgunaliev Charles b.1960; and two injured, Mamatov A., born 1992, and Suyunbaeva A. 2011g.r. who are in a children’s hospital in Osh. The most seriously injured is a 19 year old girl.
Kyrgyzstan Airlines is on the EU banned list.

On November 15, the National Weather Service warned the aircrafts to avoid airspace near erupting Pavlof Volcano in Alaska, which is spewing ash around 30,000 feet above sea level.
The volcano began erupting lava from a vent near its summit a few days ago. On November 14, the ash cloud was recorded at 16,000 feet. Dave Schneider from the Alaska Volcano Observatory said that the eruption became more intense at 6 a.m. on November 15, further rising the ash cloud.
According to Schneider, the eruption can last for several weeks or months with fluctuations in intensity.

A small plane crashed in Turtlecreek Township, Warren County, Ohio, on October 16th.
The plane, carrying two people, went down in a wooded area near the YMCA Camp Kern.
Both occupants of the plane were killed in the crash. They were identified as Eric Hackney, 43, and Jesse Loy, 36. Both of them were from Punta Gorda, Florida.
The cause of crash is being investigated.

What: unauthorized vehicle Where: Phoenix Sky Harbor When: Nov 16, 2012 Who: 2 aboard Why: Reports say that Koko Nicole Anderson was intoxicated on drugs when the car she was driving barged through a partially closed gate and onto a runway while driving with her two month old child in the car. The gate was being tested when Anderson drove through it. She struck a portable toilet and kept driving until an officer rammed her car.
Anderson’s mother says the girl was not on drugs but suffering from bi-polar disorder.
The intoxicated driver with a history of mental illness was taken into custody, and booked into jail on aggravated DUI and criminal damage charges.
An expert determined that the intoxication was related to drugs instead of alcohol. In order to safeguard planes from being either accidentally or intentionally struck by misguided drivers, the airport is considering beefing up security with airport gates that include additional pop-up barriers.

Three flights made emergency landings at LaGuardia Airport, New York, after they were separately hit by birds within a period of 4 hours on September 3.
ExpressJet Flight 5400 reported bird strike shortly after 7 a.m., while it was turning to land on Runway 31. In the second incident, ExpressJet Flight 5432 was hit by the birds at about 8:53 a.m. while taxiing to the ramp and after less than 3 hours, the Air Canada 706 reported bird strike at about 11:15 a.m. while it was some 12 miles northeast of the airport.
All three planes made safe emergency landings and no injuries to personnel or damage to property was reported.
The FAA is investigating the incidents.
