Update: CRJ Gear Foulup at JFK, Atlanta, Philadelphia, O’Hare, et al.

What: Atlantic Southeast/Delta Airlines Canadair CRJ-900 en route from Atlanta to White Plains, New York
    Where: New York
    When: Sept 25 2010 8:20 pm
    Who: 60 passengers 4 crew

The fifth + landing gear failure in the US in five years has launched an engineering investigation of Bombardier’s CRJ landing gear. The objective of the investigation is to determine if there is a connection between all of the gear failures.

    • SkyWest CRJ200, operating under a code-share with AirTran as Flight 3074 from Omaha, Neb., made an emergency landing with one gear retracted at Milwaukee General Mitchell International Airport (Sept 28 2010)
    • Atlantic Southeast Airlines Bombardier CRJ900 at JFK landing gear up (September 25 2010)
    • SkyWest Airlines Flight 6467 CRJ200 couldn’t extend its nose landing gear on approach to Ontario (Calif.) International Airport. (May 23 2010)
    • Atlantic Southeast jet–a 50-seat CRJ200–couldn’t fully extend its left main landing gear, but again, managed to land safely at Atlanta Hartsfield International Airport. (June 11 2009)
    • Air Wisconsin CRJ200 on a flight from Norfolk, Virginia landed at Philadelphia International Airport with its left main gear retracted. (Dec 14 2008) The prelim. NTSB indicates that mechanics attached the upper attachment bolt for the left main landing gear uplock assembly to the airplane structure only, rather than both the structure and the uplock mechanism as called for in the design
    • CRJ700 operated by Mesa Airlines received an unsafe gear indication for the left main gear as it approached for landing in South Bend, Ind. The crew declared an emergency and returned to land with the left main gear still retracted on Runway 28 at O’Hare. (Dec 15 2008) The investigation found that improper positioning of the inboard main landing gear door during rigging caused premature wear of parts that eventually restricted the movement of the door during its extension.
    • South Africa Express CRJ200 gear up landing on flight from Cape Town to Windhoek, Namibia( April 17 2010).

Saturday the landing gear of a CRJ failed, resulting in a dramatic landing at JFK. The dragging wing created a “shower of sparks” during the landing.

Fred Flintstone Airlines? No, it’s Air Zimbabwe

Like the nineteen year old Yemenia Airlines* Airbus A310-324 that was written up as having passengers with standing room only, unattached seats and an assortment of safety problems that were so bad that the plane was disallowed from flying over European airspace, Air Zimbabwe’s fleet demonstrates how not to run an airline.

There are apparently 5 planes and 50 pilots; far too many engineers (300? For a fleet of 5?), and is an accounting nightmare, being $108 million in debt and accruing more as it is operating in the red, and it has reportedly fallen behind on quarterly insurance payments.

A B767 may be auctioned if Air Zimbabwe fails to pay a monthly installment of US$500 000 to Lufthansa Technics. Repair of the B767-200 engine which requires US$2,5 million for repairs, hinged on the condition of making US$500 000 monthly payments from May 2011 to redeem the debt.

Skytrax, which rates airlines on a scale of 5-1 (5 being good) rates Air Zimbabe as a 2. Passenger reviews include random cancellations, ancient planes, unexpected delays, missed connections, owed reimbursements, misdirected charter flights, and strange excuses from gate personnel.

Yemenia Airlines* is also a 2 star airline.

Air France Airbus Flight 447: Information Withheld

    George’s Point of View

The next big question is whether or not information is withheld.

That’s what some families are wondering, and that’s part of the reason why they’ve hired lawyers. Plaintiffs have access to more information; and they want to know what reason Air France has for not replacing a known Airbus problem, the pitot probes (airbus speed sensors.)

What has been published is error messages based on inconsistent readings taken by the pitot probes.

Air India Responsible for May 2010 Crash

    In George’s Point of View

Crew Resource management (AKA Cockpit Resource Management) came about as a resource for competent pilots with deficient people skills. This is crucial because a pilot does not fly in a vacuum but as part of a team. CRM teaches pilots how to improve communication, a crucial supplement to normal training. CRM helps a crew avoid potentially antagonistic work environments, such as the one that occurred in Mangalore, on May 2010, the Air India Crash which killed 158.

It has come to light that Air India did not provide mandatory CRM training. This deficit is clearly reflected in pilot behavior, and violates international norms. The court of inquiry is considering inadequate CRM as a key factor in the Mangalore crash.

Anyone reading or hearing the transcript knows that leadership and decision making on this flight was not up to standard. The Captain failed to discontinue the ‘unstabilised approach’ after touching down halfway down the runway, and persisted in continuing with the landing, despite three calls from the First Officer to ‘go around’ and a number of warnings from EGPWS. If there had been adequate CRM training, the captain would have known to listen to the first officer and warnings; and barring that, the F.O would have known to follow his own (correct) judgement.

Air India Express did not even have a CRM safety training division.

    NTSB Meets

NTSB TO MEET ON BUSINESS JET RUNWAY EXCURSION ACCIDENT THAT KILLED FOUR AND INJURED TWO IN SOUTH CAROLINA

The National Transportation Safety Board will hold a public Board meeting on its investigation into an accident in which a chartered business jet crashed during a rejected takeoff in Columbia, South Carolina, 18 months ago.

The purpose of the meeting will be to determine the probable cause of the accident and to consider proposed safety recommendations to reduce the likelihood of future such mishaps.

On September 19, 2008, at 11:53 p.m. EDT, a Bombardier Learjet Model 60 (N999LJ) operated by Global Exec Aviation and destined for Van Nuys, California, overran runway 11 during a rejected takeoff at Columbia Metropolitan Airport. The captain, the first officer, and two passengers were killed; two other passengers were seriously injured.
