come on Boeing/Rolls Royce, FIX-IT, waiting can be life-costly!
As you can see by the NTSB Press Release, It is time for Rolls-Royce and Boeing to move forward on a corrective redesign of Rolls-Royce RB211 Trent 800 Series engines.
The solution is to amend the safety vulnerability. Redesign the FOHE and eliminate the potential of ice build-up.
NTSB agents believe there’s a good chance this will happen again; we would hazard a guess that it has already happened more than the twice we’ve mentioned here, and will continue to happen until the solution suggestion is implemented, built, and installed asap before we have to write here about 777 incidents or fatalities.

What: Aires Boeing 737-700 en route from Bogota to San Andrés Island Colombia Where: Gustavo Rojas Pinilla International Airport on the Colombian island of San Andres When: August 15 2010, 2:15 a.m. Who: 131 aboard, 6 crew, 4 minors, 121 adult passengers Why: Prior to touchdown, the plane was struck by lightning (11 strikes recorded within 6 miles of the runway within a five-minute span of the crash); the plane impacted 80 meters shy of the runway. The impact scattered pieces of the plane, and it broke at the midsection into three main parts. When help arrived, passengers were already strewn about the area. Praise Boeing for no actual fatalities.
The injured are being treated at “Amor de Patria Hospital” and “Villarreal Clinic” on San Andrés Island, with the most injured being ported to Bogata via air ambulance.
After the crash-landing, Mrs. Amar Fernández died of a heart attack en route to the hospital. She was 73. 66 were reported injured, five of them seriously, 35 hospitalized. Alejandro Colmenares Rua (18 months), Yiseth Rua Rojas (23), Catherine Rua Rojas (27), Jacqueline Rua Rojas(23), María Alejandra Ballesteros (21) and Andrés Cagua Calderón (33) were unhurt.
A room has also been opened for relatives at the Sheraton Hotel in Bogota. Aires published a release regretting what has happened. They are working with Aerocivil to serve the passengers and their families. They have provided a care line number: 018000-949490
Relatives of the occupants may contact the Hotel Habitel where they will receive all Aires information.
Flights at Gustavo Rojas Pinilla are suspended. Civil Aeronautics, the Navy, the Colombian Air Force, National Police, Red Cross, the company Aires and other institutions of the Regional Committee for Disaster Prevention and Response have responded.

What: Omega Air Refueling Boeing 707 tanker en route from Point Mugu naval air base Where: near the beach, Point Mugu naval air base, California When: May 18, 20115:25 p.m. Who: 3 crew Why: The plane skidded off the runway while taking off, and burned for three hours before being extinguished by a full response team, a chopper and three fire engines.
The plane was loaded with 150,000 pounds of fuel. After a chopper doused water with no discernible effect, flight crews concentrated on letting the plane burn out within a small perimeter. Plumes of black smoke reached Malibu.
The three-member (civilian) crew walked away with minor injury. Alexandria, Va.-based-Omega Aerial Refueling Services, Inc. contracts with the military.
The NTSB is investigating.

LGW Airlines flight AB-8683 made an emergency landing in Munich, Germany, on May 23rd.
The plane, heading from Bologna, Italy, to Dusseldorf, Germany, was diverted after a propeller over-speed indication prompted the crew to shut down one of the engines.
The plane landed safely. No injuries were reported.

Southwest Airlines flight WN-3095 suffered a bird strike near Baltimore, Maryland, on March 17th.
The Boeing 737-700 plane flying from Norfolk, Virginia, was on final approach to Baltimore–Washington International Airport when it hit a flock of geese.
The plane continued for a safe landing. Everyone aboard remained unharmed.

A Hyderabad-bound Air India flight made an emergency landing immediately after take-off at Delhi airport on April 12.
The decision to land was made after some technical problems in the plane were discovered. The 131 passengers of the flight 560 faced some tense moments till landing safely. They later resumed their journey on another aircraft.
Also on board was a senior BJP leader, Mr. Venkaiah Naidu, who has been aboard nine flights which made emergency landing in the past.
