What: NARZ Mi-8T helicopter RA-22376 en route from Kyshtovka Where: n the vicinity of the Krapivin oil field of OAO Tomskneft VNK in the Kargasoksky district When: November 25, 2010 12:12 Who: Eight killed, three injured Why: The helicopter carrying shift workers and 1.6 tons of cargo had been hovering over the landing site when an uncontrolled rotation of the aircraft began. The Mi-8 drifted for about 50 meters, tipped over to its side, collapsed and caught fire. A criminal case has been initiated, a violation of traffic safety and air transport operation regulations, entail the death of two or more persons by negligence.
The black box, previously reported as being missing, has been found.
What: single-engine 1968 Piper Cherokee en route after refueling in Quebec City en route to Saint John, N.B. Where: near Buckland*, 90 kilometres southeast of Quebec City. When: Tuesday Who: pilot Jesse Barrie, and two surviving passengers: Dan Weiher and Stephanie Shipley. There were two fatalities: Pilot Jesse Barrie and passenger Jeffrey McClymont Why: No cause has been determined.
After Joint Rescue Co-ordination Centre at CFB Trenton, Ont received a 911 call from the female survivor of the crash,rescue searchers parachuted in and the two survivors were found; they were still alive, though not outfitted for the winter weather conditions. They are described as being “frozen” when they were airlifted by helicopter at 10:30 a.m to Quebec city.

What: Polish Air Force Tupolev TU-154M Where: Smolensk When: Apr 10th 2010 Who: 88 passengers and 8 crew Why: On approach to Smolensk North Airport in dense fog, the plane impacted a forest, coming to rest short of the runway. There are conflicting reports leading up to the decision to land at Smolensk (versus Minsk or Moscow), holding pattern, the number of approaches.
Everyone aboard the flight was lost, including Polish President Lech Kaczynski, his wife Maria, the national bank president, deputy foreign minister, army chaplain, head of the National Security Office, deputy parliament speaker, Olympic Committee head, civil rights commissioner and at least two presidential aides and three lawmakers.

A bird strike on the HH-60G Pave Hawk at 110ft at 126mph caused the crash of a helicopter on a moonlight training mission on the Norfolk Wildlife Trust reserve, killing staff Sgt Afton Ponce, Capt Christopher Stover, Technical Sgt Dale Matthews and Capt Sean Ruane.
The crew was confirmed dead at the scene. The investigation has begun, and there is a fund opened for them.
A number of twelve pound geese struck the helicopter after the flock flew up alarmed by the noise.
The USAF chopper was flying out of Lakenheath in Suffolk.

When the Hamburg Airways flight’s landing gear failed to retract after taking off from St. Petersburg, the pilots had to circle for two hours before being able to return to the St. Petersburg airport.
The Swiss Europa league team was aboard, returning from the quarterfinals in St. Petersburg. Instead, they returned to St. Petersburg for their plane to be examined.
The flight to the competition also had problems. While en route from Basel to Russia, pilots diverted to Berlin. On the recommendation of team physician Marti, the pilot make an an emergency stop so that forward Mohamed Elneny could receive medical treatment for a dizzy spell. His medical diagnosis was caused by hypoglycemia and dehydration.

What: Mountain Lifeflight Aerospatiale AS.350 Ecureuil en route from Renown Medical Center, Reno to Susanville, CA Where: 29 miles northwest of Reno, Lassen County, CA When: NOV 14, 2009 Who: 3 aboard Why: After delivering a patient to a Reno Medical Center/hospital, the helicopter crashed and was destroyed by fire while on the way to its home base.

What: 1930s single-engine de Havilland Tiger Moth Where: East Windsor, Conn When: Saturday August 30 Who: pilot and passenger Why: The plane lost power 200 feet from the runway at the Skylark Airpark Saturday. The pilot and passenger crashed in treetops and were stuck in the tree until help arrived. A tree surgeon climbed the tree, and set up a pulley to rescue the pilot and passenger who were apparently unhurt. The pilot was an instructor who “maintained an airspeed where he was in control and buffeted [the plane] into the top of the trees. That’s what saved both their lives.”
