What: American Airlines McDonnell Douglas MD-83 en route from Dallas Ft. Worth,TX to Pittsburgh,PA Where: near Dallas When: Nov 19th 2011 Why: Just after take-off, the pilot switched the fuel supply from center fuel tank to main wing tanks and the right engine flamed out.
Pilots returned to the airport and made a safe landing.
Maintenance is investigating why operating pumps were working, yet failing to provide fuel to the right engine.

What: Servicio Aéreo de Ibagué Hughes 369D Where: Pitalito, Colombia When: Dec 29, 2009 Who: 1 fatality (2 or 3 aboard.) Why: One of the people aboard died in the crash which occurred in an area between two Pitalito neighborhoods. The Air Society Ibague helicopter apparently originated in Ibague, capital of Tolima. Its destination is unknown. Witnesses say the helicopter made strange turns in the air, but the pilot managed not to strike any houses.
There have been reports of one and two individuals injured.

American Airlines flight AA-2472 had to divert and make an emergency landing at Los Angeles International Airport, Los Angeles, California, on November 16.
The Boeing, en-route from San Diego, California, to Dallas, Texas, had to divert due to a problem with its flaps.
The plane landed safely.
All 182 passengers and 6 crew members remained unhurt.

What: Primera Air Boeing 737-700 en route from Zakinthos Greece to Dublin Where: Rome Italy When: Jul 10th 2009 Who: 153 passengers and 6 crew Why: While en route, the Primera flight requested an emergency landing. It lowered altitude without declaring an emergency, which upset air traffic control. Fighter jets (Eurofighter) were deployed as an escort. The flight landed safely in Rome (after refusing Naples and burning off fuel.)

What: UT Air -ATR 72 en route from Tyumen to Surgut Where: Siberia When: April 2, 2012 Who: 39 passengers aboard, 4 crew, casualty numbers pending.
In the UTAIR crash, it has come out in the news no one de-iced the plane. This according to the airport’s deputy director, Vladimir Nyesmachny, who also told the media “Usually the captain of the aircraft makes the decision, as is his right.”
A similar public statement regarding the lack of icing was made by Investigative Commitee spokesman Vladimir Markin, a spokesman for the Investigative Committee “At this moment, the most likely cause for the crash is the fact that the aircraft was not treated with deicer before flight.” However, on April 2nd, Markin had said that investigators considered technical failure as the most likely cause.
Andrei Martirosov, UTair’s CEO promises that from now on, UTair crafts would undergo “mandatory” deicing before flight.
What: Travel Service Boeing 737-800 en route from Sharm el Sheikh Egypt to Prague Where: Cairo When: May 17th 2010 Who: 160 passengers Why: While en route, the flight crew found indication of a wheel well fire and diverted to Prague. The flight landed safely, and the indication proved to be groundless, causing only a minor delay at the airport.
