    Rescue Helicopter Crashes in Spain; 3 Civil Guards Killed

    A helicopter crashed after its blades brushed a mountain near Picos de Europa range in Leon province of Spain on August 24.

    The accident happened when the helicopter was on a rescue mission for a mountain climber, with 4 members of Spain’s rural Civil Guard police aboard.

    According to local government, 2 pilots and a winch operator was killed in the accident while the fourth member escaped the accident as he had already descended to the ground for helping the climber.

    Cessna Crashes by New Hampshire Runway

    What: Cessna T182T Turbo Skylane
    Where: WEST LEBANON, N.H.
    When: Feb 9, 2012, 3:45 p.m
    Who: pilot, 1 fatality
    Why: The Cessna T182T Turbo Skylane took off from Lebanon New Hampshire Municipal airport returning the plane from repairs  but developed problems. The pilot, Paul Schlieben, of Peterborough, N.H requested a turnback to the airport.

    He was returning to the airport when he crashed in the airport safety area impacting beside a runway. West Lebanon firefighters responded to the scene and put out the flames. The pilot did not survive the impact.

    Morning Takeoff after Layover Ends in Fatality

    What: Rose Resources Oil & Gas Twin engine Beechcraft Baron 55 plane from Louisiana to Bixby Oklahoma
    Where: DeQueen airport Southwest Arkansas
    When: April 15, 2011 8:30 am
    Who: pilot Timothy Stanley
    Why: Encountering bad weather while en route, Stanley diverted to Dequeen Airport. He made a safe landing, and spent the night.

    In the morning after taking off, while circling, the pilot lost control and crashed off of the east end of the 5,000-foot De Queen runway in a ditch. He died on the scene.

    Where: crashed into a storage yard
    When: January 2006
    Who: Michael Waugh ( general manager and chief operating partner of Joe’s Seafood, Prime Steak & Stone Crab in Chicago), Kenneth Knudson (founder of Sybaris), and Scott Garland (financial adviser at Morgan Stanley)
    Why: Circuit Judge John Ward awarded $6 million to Waugh’s widow, with the rest being divided among the couple’s three young sons.

    Wrongful death suiit: Morgan Stanley improperly allowed employees to fly personal planes to conduct business–a situation which other financial institutions prohibit.
    Pilot Mark Turek, a Morgan Stanley senior vice president was charged with negligence.

    Two in one: Pia Emergency

    What: PIA Pakistan International Airlines Airbus A310-300 en route from Quetta to Islamabad
    Where: Islamabad
    When: Oct 24 2010
    Who: 187 passengers
    Why: On approach to Islamabad, nose gear failed. The pilot shut down the plane on the runway, and the plane was towed.

    Who: 190 passengers
    Why: After takeoff, suffering a technical problem, the pilot returned to Manchester where he made a safe landing. The flight (or a replacement) was rescheduled for the next day.

    GOL Inspects Flight for Airworthiness

    What: GOL Transportes Aereos Boeing 737-700 en route from Bauru to Sao Paulo Brazil
    Where: Bauru
    When: Feb 2 2010
    Who: 82 on board
    Why: Prior to take-off, the engine presented an automated warning sufficient to deter the pilots from taking off. Details about the warning have not been made public although they were adequate for Gol to begin an investigation. While the original plane went into maintenance, a replacement plane was provided within four hours.

    While we of course have no idea what the warning was, nor how important an issue it represented, we commend GOL for taking proactive action and responding to warning systems as they were intended to be. Hopefully maintenance will become as diligent as the crew that actually has to be in the plane aboveground.
