Where: Halavelhi Resort lagoon while coming in for a landing, one of the Otter’s wings struck the water causing the crew to lose control of the airplane.
When: Jun 2nd 2009
Who: 4 passengers, 3 crew
Why: After taking photographers on a photo-flight over the resort, on return, the wing struck water and the pilot lost control. There were no major injuries.

Southwest Airlines Boeing Nosegear Collapses in NY

On July 22, 2013, a Southwest Airlines Boeing 737-700 en route from Nashville,TN to La Guardia was making a landing when the nose gear collapsed.
Five passengers and 3 crew were injured on the slides and hospitalized.
There were 144 passengers and 6 crew aboard.
A passenger described landing as “a bang and a bounce and then just a slam on the brakes and then it was a skidding”
The airline confirmed a nose gear collapse, 5 passengers and 3 cabin crew reported injuries and were taken care of by local responders.

What: Cool Stream Media LLC Socata TBM700 C2 en route from Teterboro airport to Atlanta-Dekalb Peachtree Airport
Where: Interstate 287 in Harding, NJ
When: Dec 20, 2011, 10 am.
Who: 5 aboard, 5 fatalities
Why: After taking off from Teterboro airport en route to Atlanta-Dekalb Peachtree Airport, the pilot inquired about icing conditions, and shortly afterwards, crashed on I-287 in New Jersey. The plane came down on the median and broke up just outside of Morris township. One witness heard nothing but saw an explosion. Another witness smelled burning rubber and said it sounded like a wood chopper.
The plane lost contact with ATC just before the crash.
Aboard the flight were Jerry Buckalew, 45 and Rakesh Chawla (36) managing directors of Greenhill & Co Investment Banking Firm. Buckalew’s wife Corinne and two children Jackson and Meriwether and a dog were also aboard. Witnesses say the plane was spinning and flipping, then a wing came off and the plane went straight down. Buckalew owned the plane.
No cars were stuck but the plane’s right wing landed in a tree 400 yards from the crash site..
The pilot had gotten clearance to climb to 17,500 feet. ATC said there was “moderate rime” at 17000 feet. Another controller warned of extreme icing.
The debris field covered an area half a mile wide. The northbound late of I-287 was blocked a heap of burning charred metal.
Aripuanã Plane Crash kills Pilot and 3 passengers, including 2 Civil Servants
On April 6, 2013, an Embraer EMB-720D Minuano #PT-ROS en route from Aripuanã to Colniza, Mato Grosso went missing. A pilot saw the wreckage and called it in to Civil police before the official search began, which was set to begin on Monday. The wreckage was reported on Monday April 7, along with the remains of the pilot and three passengers. Two of the victims, Elias Borges Nogueira and Alexsandro Pereira da Silva were going to be in service in the region. The pilot and a local farmer were the other two victims. One of those aboard was found feet away from the plane, and had no burns.
The wreckage was discovered in an area of difficult access.
Aripuanã has no fire department, and the recovery work is being done by Aripuanã police. The remains were held at the local mortuary until being shipped to the Forensic Institute by Integrated Air Operations Center (Ciopaer).
The government statement said that they will be giving full support to the families.
The statement of the Secretary of Environment of Mato Grosso, José Lacerda, confirmed that the plane#PT-ROS crashed shortly after takeoff on Sunday. “They are Cuiaba and were on a survey of regional programming in the areas of Aripuanã, Colniza and New World.”
The police are speculating that the plane may have lost parts in the air, although we do not know if this is supported by the photographic evidence we have seen. (i.e. if the plane is intact now in pictures, it was likely intact in the air.) Also, the weather was rainy and windy at the time the plane went missing.
Woman Dies in Venezuela Crash, Survivors Rescued
On Feb 2, 2013, a privately owned Beechcraft 58 Baron with six aboard crashed in the Gulf of Cariaco, Venezuela.
The flight had taken off from Puerto Ordaz Airport and was en route to DelCaribe Gen S Marino Airport but suffered a complete loss of engine power. The pilot declared an emergency due to failures in both engines.
Fishermen found the plane located 15 nautical miles (28 km) east of Cumana in the Gulf of Cariaco.
Two of the passengers were reported to be minors.
There was one fatality, a sixty-two year old woman, Vetide Ramos, who died in the crash. Five survivors were brought ashore in a boat, including Ruben Herrera, Poztty Ramos, Jose Ramos, Ivan Herrera (a minor) and pilot Jhonatan Mata.
Emergency Landing in Hollywood

What: KTLA EUROCOPTER AS350 helicopter
Where: Hollywood Boulevard and Franklin Avenue.
When: August 28, 2012 1:00 pm
Who: pilot and passenger
Why: When the helicopter started giving off blue and white smoke, the first to see it was the pilot of another helicopter.
KTLA pilot Tim Lynn declared an emergency, then made a forced landing in a parking lot of the senior center at Hollywood Boulevard and Franklin Avenue.
The incident was witnessed by SKY2 pilot Dan Catalano (who taught Lynn to fly heicopters.)
The smoke came from an oil leak. Some newspapers report two aboard the helicopter but the FAA entry only reports the presence of the pilot.
South Bend Crash Raw Video

Steven Davis and Wesley Caves died in the South Bend Plane crash on Sunday.
The Hawker Beechcraft 390 plane struck three homes when it fell; two other people were aboard and were injured and one person on the ground was injured.
The plane was split in two by the impact. The raw video below was captured by a surveillance camera.
