    Wrongheaded Industry Reassessing Vulnerable Pitot Tubes vs Ice Procedures instead of Brainstorming Next Generation

    Pitot tubes use external air temperature and pressure to calculate changes in speed. Ice crystals that get caught in pitot tubes are considered a contributing factor in a dozen significant events, and the crash of Air France flight 447. As a result of ice crystal accumulation, air temp and pressure readings are false, autopilots shut off, pilots lose altitude readings and receive false warnings. In a fly by wire plane, this is disastrous. In Air France Flight 447, it killed everyone aboard the plane.

    Today’s news is full of aviation-safety experts looking for changes in procedures and more-precise checklists. There’s a lot of buzz out there about a new study. I hope that the study, as it has been explained, is not just about adapting to broken technology. I hope this is not the only study out there.

    Air France and Boeing are now examining how these ice crystals function (or more correctly, malfunction.) That seems wise.

    But one questions why the next step of the anti-icing drive will be to get consensus on how pilots should respond to pitot tube failure.

    This is like figuring out the best way to drive on a flat tire, instead of making a tire that doesn’t go flat. Sure, pilots should have procedures, but shouldn’t a study be designed with the ultimate goal being to give the pilots a dependable speed sensor, not methods how to mitigate an oncoming disaster? Are designers afraid to trade a known quantity, even though it is flawed, for something unaffected by ice crystals? Even if it is cheaper for airlines to figure out how to muddle on with existing systems than to devise an alternative technology to pitot tubes, shouldn’t the focus of studies be on creating technology that is not disabled by ice crystals?

    Not being an engineer myself, I have no ideas on the subject—but somewhere there’s an aviation engineer with a brilliant idea of either a safe design of reconfigured pitot tubes or a completely different system. Let us hope that engineer is able to step up, and be recognized. But it may not be so simple. Apparently Airbus and Air France had been aware of chronic pitot tube problems for years and were content to continue using the same design anyway.

    Come on engineers. It’s time for a better mousetrap.

    Automated Cockpit Props up Undertrained Pilots

    The Asiana investigation continues.

    Back in July, the pilot who was insecure about making a visual approach in a 777 crashed at San Francisco International Airport on a visual approach in Asiana Flight 214’s Boeing 777. Specifically, he told NTSB investigators “it was very difficult to perform a visual approach with a heavy airplane.” The glideslope was not working at the San Francisco airport, and that was an instrument the (*undertrained) pilot was relying on. The plane came in so low the tail struck the seawall and broke off. The video below shows the plane rotate 360 degrees and catch fire by the runway.

    Before impact, the relief pilot in the jump seat repeated several times “sink rate” trying to warn the pilots at the controls that the jet was too quickly losing altitude. One of the pilots said “It’s low.” Then there was a stick shaker alert (which occurs when the plane is about to stall from flying too slowly. I once had a pilot do a presentation that included the disturbing grinding of the stick shaker alarm as it violently vibrated the control yoke. It’s an alarming direction to the pilots to increase thrust.)

    When the stick shaker went off, the instructor called for a go around. It went off four seconds before impact. It was too late.

    Both the instructor and the captain were relying on the auto throttle, and both were unaware it was off.

    I do not know how anyone can watch the surveillance video of the Asiana crash and not marvel that of the three hundred and seven people aboard the plane, there were only three deaths.

    I’m not discounting the wounds of the injured, nor those three deaths, nor the tragedy of one of the teen victims being run over by an airport crash tender. (That’s a whole tragedy by itself—who knows if she might have survived but for being so obscured by foam that she was not visible to crash responders—through the firemen who carried her out surely must have known she was there.)

    A dozen critically injured, a hundred-sixty-nine injured, but only three deaths.

    It’s nothing short of a miracle. Especially on inspecting the condition of the burned out shell of the hull. Especially on reviewing the just-released surveillance video that shows the plane splintering after impacting the firewall, cartwheeling like a crippled gymnast down the runway and dissolving into a cloud of dust and flame. No jet fuel fire here——leaking oil ignited as it poured on to a hot engine.

    The Kazan crash (Tatarstan Airlines Flight 363) from November 17th is fresh in my mind. Everyone aboard–fifty people (forty-four passengers and six crew) all died. The plane just fell from the sky while landing at an impossible 75-degree-nose-down attitude, piloted by a pilot whose license is apparently fake. Everyone in that crash died.     Of course one can see the physics—that everyone on the Tatarstan flight received the full direct impact, versus how the rolling of the Asiana plane dispersed some of the impact energy. Still, there is tremendous force in a crash.

    I know I should be talking about pilot training, because this is yet another crash that appears to be due to pilots becoming too dependent on technology. But I will focus on that another day. Right now, I am overwhelmed after looking at the crash tape.

    Asiana—Cartwheeling Catastrophe
    I am surprised that I have neither heard or seen choruses of amazement that all but three people survived the rolling catastrophe in San Francisco. Some credit should perhaps go to the rescue crew, quick actions of the cabin crew, performance of the emergency slides, and maybe even the aerodynamics of the 777 whose seats are required to withstand 16g of dynamic force.

    Sure, there was error involved in this crash, but when you look at the survival rate, some credit is due to the 5.5 billion Boeing put into research, development and safety of the 777.

    The IATA ( Air Transport Association) is an association representing the world’s airlines commitment to excellence and safety in aviation. All of its 224 Member airlines live up to best practice standards in airline safety. 8 airlines lost membership due to failing a 2008 IOSA ( IATA Operational Safety Audit) audit.

    Airlines which fail the safety audit, or are otherwise unable to reach the safety standards are banned by the European Union. One of those banned airlines is Garuda Indonesia, the national airline of Indonesia. That’s the same airline which has been in the news for the recent sentencing of one of it’s pilots. Not that Garunda Indonesia even flies to Europe.

    Marwoto Komar who was piloting Garuda Indonesia Flight 200 , ignored warnings not to land the plane when he brought it in at twice the safe speed, resulting in the plane slamming into the runway, skidding off and bursting into flame, killing 21. While he did not get life in prison, he did get jail time.

    The pilot’s sentencing is really a symptom of what is going on with Garunda Indonesia, which is a lack of government supervision. In fact, aircrafts which would otherwise be considered safe, are NOT safe simply because of being part of GI’s under-supervised, under-inspected, under-maintained fleet. In fact, there is a $24 million Indonesian governmental program now in place to train Indonesian air safety inspectors, an ambitious program which may still take years to bring the airline up to acceptable safety standards.

    In the meantime, apparently the burden of safety rests on the shoulders of pilots, which in view of recent history, does not particularly appear to be a good thing. Furthermore, no one yet has made it public exactly why Marwoto Komar ignored air traffic control AND his instruments AND his co-pilot to attempt a landing at twice the safe speed, but perhaps while he is in jail, the Indonesian skies may be marginally safer. Not that it does any good for the 21 victims of Flight 200.

  • Foreign Aviation Tragedies, Thoughts on Compensation

    Lives have value. It doesn’t matter if a man makes $10,000 a day, or 10 cents. If he dies in a plane crash, he’s irreplaceable. Maybe his boss can put a dollar figure on what it would cost him to lose that worker; maybe his banker can come up a specific value to subtract from the family’s credit rating.

    But to his wife
    to his son
    to his daughter
    to his parents
    to his friends
    to all those his life touched daily—
    he’s irreplaceable.

    His loss leaves a hole in a family that money can never fill.

    And then the attorneys for insurance companies will “try” to get by with compensation offers that are equal amounts for each passenger, that in some cases, equal to “peanuts” — I don’t see that too much, but ‘they’ try, especially if the laws in the country have established maximum low-values for the loss of life in aviation accidents. Even with Warsaw and Montreal the compensation I’ve seen is seldom governed by the caps provided for in these treaties.

    Instead, the compensation is based on the profile of the decedent. Age, Occupation, Family, etc. With this said, each seat on that plane is occupied by an individual and the family of the victim should be compensated accordingly, and in addition, the family should be paid non-pecuniary damages for pain and suffering, etc. In a perfect case, that’s how it works.

    Whether he makes $10,000 a day, or 10 cents will not affect a man’s pain and suffering. In fact, his pain may be worse, knowing his ten cents is all that lies between his family and ruin.

    PEANUTS is not acceptable.

    ATC Catches Runway Incursion, China Airbus Possible Collision

    What: China Airlines Airbus A330-300 en route from Taipei to Hong Kong
    Where: Taipei
    When: Aug 19th 201
    Who: 288 passengers
    Why: As the China Airlines Airbus was accelerating for takeoff, a private jet was also taxiing on to the same runway after landing. ATC caught the runway incursion and prevented an inevitable collision, and held the China Airlines Airbus flight, delaying the takeoff by an hour.

    The pilots of the South Africa registered aircraft reported they had been instructed to taxi taxiway NC to the end. At a crossing, they requested direction and were told to turn right onto taxiway NC. They made an error based on unclear signage turning back on to the runway instead of to the taxiway.

    Just another everyday incursion.

    Ok, someone needs to commend Taipei ATC on a pretty stupendous catch. From this side of the screen, it looks like this was a practically inevitable disaster.

    And do I have to say that someone needs to fix the signs? Something needs to be amended there so that even pilots unfamiliar with the airport can see which is the on ramp and which is the off ramp.

    I’m just saying.

    Aviation Safety Action Program

    Because of management issues, labor vs management issues, and the question of how much protection should pilots/crew have when they make a mistake, a number of big carriers have dropped the Aviation Safety Action Program. THe official goal of the Aviation Safety Action Program (ASAP) is “to enhance aviation safety through the prevention of accidents and incidents. Its focus is to encourage voluntary reporting of safety issues and events that come to the attention of employees of certain certificate holders.”

    But pilots are being punished for their honesty.

    The program that is being dropped allows pilots and other airline workers to report mistakes they’ve made and safety issues.

    This enables carriers to spot trends before they become safety issues.

    So..why have Delta, American, U. S. Airways and Com Air dropped the program?

    Should we even FLY these carriers if they are not following ASAP?
