Why: The plane suffered a tail strike on takeoff from Melbourne’s runway 16, climbed and dumped fuel, and when smoke filled the cabin, landed on Melbourne’s runway 34. Damage included some runway lights, damage to the tail skin and access panels.

Why: Before touching down, the crew reported technical problems. A hundred meters before the runway threshold, they landed onto grass, damaged a perimeter fence, and had a flat, but managed to taxi to the gate. There were no injuries. An alternate plane will be provided for the next leg of the journey.
Etihad Airways Plane Diverts to Germany after Loss in Cabin Pressure
Etihad Airways flight EY-38 had to divert and make an emergency landing at Munich Airport, Munich, Germany, on April 16th.
The Airbus A340-600, heading from Paris, France, to Abu Dhabi, UAE, was diverted after it lost cabin pressure mid-air.
The plane landed uneventfully. All people onboard remained safe.

Where: Irkutsk International Airport in Siberia

Who: 193 passengers and 10 crew
Why: The plane failed to decelerate on landing and crashed into the barrier.
124 died.
The investigation showed that as the pilot slowed with wheel braking, he deployed the right thrust reverser. The crew was unaware that the left engine thrust lever was (accidentally?)advanced. This prevented the plane from decelerating normally.
