    Pilot Dies Fighting Forest Fire


    On Oct 24, 2013, a Rebel Ag PZL M-18A Dromader was engaged in firefighting over Ulladulla, NSW when the pilot crashed. The PZL-Mielec M-18 Dromader is a single engine vehicle manufactured by PZL-Mielec in Poland, often used as a firefighting machine.

    David Black, 43, from Trangie in central NSW, was water bombing the fire when his plane crashed in rugged bushland. The fire and forested terrain is making response difficult. After giving his life trying to control the fire raging near Ulladulla, New South Wales, David Black is being hailed as a hero.

    Whether pilot error, rustic conditions, or mechanical error, the cause of the crash has not been determined.

    Fires are still burning in the area.

    The accident is being investigated by the ATSB and the investigation #AO-2013-187 has been assigned.

    Who: 6 aboard, 3 fatalities
    Why: On May 24, 2008, at 0920 Pacific daylight time, an Aerospatiale AS-350-D helicopter, N67GE, operated by Island Express Helicopters, Inc., impacted terrain while landing at Two Harbors, Avalon, located on Santa Catalina Island, California, during a 14 Code of Federal Regulations Part 135 on-demand, commercial flight. The flight departed from the Queensway Bay Heliport, Long Beach, California, at 0907. The commercial pilot, a company employee, and one passenger were killed, and three passengers were seriously injured. The helicopter was destroyed by impact damage and fire

    Piper Crashes in Chile

    Where: Quellón Chile
    When: Mar 1 2012
    Who: 8 aboard, 7 fatalities
    Why: On a 20 minute flight, ATC lost contact with the plane six minutes into the flight. Air Force and Navy planes searched where the emergency signal beacon issued a single alarm before the aircraft crashed into the sea.

    The plane’s wreckage was located by passengers in a boat in the Gulf of Corcovado off Chiloe island on Piedra Blanca at the mountain’s base. Occupants of the stricken plane included a businessman, his son and company divers and pilot Ricardo Hein. The list of dead includes Richard Hein, Genaro Barrientos Barria, Christopher Barria, Carlos Muñoz, Juan Gonzalez de La Fuente, Onofre Uribe Soto, and Pablo Briceño.

    Five of those aboard have been found.

    United Airlines Plane Rejects Takeoff From Tokyo’s Haneda Airport

    United Airlines flight UA-876 had to reject takeoff from Haneda Airport in Tokyo, Japan, on May 14th.

    The Boeing 787-900 plane was accelerating to takeoff for San Francisco, California, when the crew noticed a blown tire and rejected takeoff at high speed.

    The plane was towed to the apron. Everyone aboard remained unharmed.

    5 Die in Hawaiian Chopper Crash into Mountain

    What: Blue Hawaiian Helicopters Eurocopter EC130 B4 helicopter departing from Kahului
    Where: Kaunakakai, Molokai island, Hawaii
    When: November 9, 2011 at 12:30 pm
    Who: pilot Nathan Cline, 30, of Kihei and 4 tourists
    Why: While on a 45 minute tour of West Hawaii, the chopper crashed into a mountain. The accident occurred on Wednesday, less than a mile from Kilohana Elementary School, right after a hard rain. At the school, students heard an explosion and saw black smoke when the chopper crashed.

    The pilot and four tourists died at the scene. Although the investigation is ongoing, according to at least one published report, the NTSB said that the pilot failed to maintain enough altitude over the terrain amid low-lying clouds.

    Two of the passengers were newlyweds, and they came from Pennsylvania and Ontario Canada.

    Blue Hawaiian, which conducts 160,000 tours yearly, leased the year old chopper from Nevada Helicopter Leasing LLC.

    Four bodies have been recovered. The fifth has been located, but is in a difficult location.

    One Fatality in Mexican Lake while Tourism Video Filming Underway, No Academy Actors Involved.

    Who: 5 aboard, 1 fatality
    Why: The Helicopter was filming a promotional tourism video when it crashed in Lake Zirahuen. Four aboard were injured. There was one fatality.

    The aircraft became entangled with a tension wire, used as ziplining.

    Despite the rumor, no members of the cast of “The Academy’ television show were aboard. The rumor appeared apparently after tweets immediately following the accident.

    The injured were reported as Enrique Barragán, Carlos Vela, Pablo Gabriel Angulo and Luis Fernando Gutiérrez. Daniel Martinez was initially reported as missing, then later as a fatality.

    THe helicopter is owned by the State of Michoacan.
