What: Kyrgyzstan/Altyn Air Tupolev 134A-3 en route from Bishkek to Osh
    Where: Osh Airport, Kyrgyzstan
    When: 28 DEC 2011
    Who: 6 crew, 82 passengers, 25 injured
Why: After a hard landing in dense fog, the plane skidded off the runway, collapsing the right main gear, and flipped. As it rolled over, the right wing separated, and the ensuing fire was quickly contained by rescue services standing by. Passengers inside were hanging upside down, suspended by their seatbelts, but had to find a way to evacuate quickly, before the plane had a chance to explode. A fire occurred from a fuel leak in the remaining wing, but it was controlled.

The metar at the time of the landing was:
    UAFO 280700Z 02002MPS 0300 R12/0550 FG VV001 00/00 Q1023 R12/19//50 TEMPO 0300 FZFG VV002 RMK QFE691/0922 BASE050M
Wind from 020 degrees at 2 m/sec; visibility 300m; on runway 12, touchdown zone visual range is 550m in fog; vertical visibility 100 m; pressure: 1023 hPa; temperature: 0°C.

Six crew members and nineteen passengers were hospitalized, but there were no fatalities reported. Nine children were aboard. The plane was evacuated.

Because of the incident, the airport (runway) was temporarily closed, and Osh-Bishkek flights are delayed.

Of the 25 reported injured and hospitalized, these have been identified: Rustamov A. b. 1977, Ismatova A. b. 1993, Turgunaliev Charles b.1960; and two injured, Mamatov A., born 1992, and Suyunbaeva A. 2011g.r. who are in a children’s hospital in Osh. The most seriously injured is a 19 year old girl.

Kyrgyzstan Airlines is on the EU banned list.

What: Helios Airways Boeing 737-300 en route from Cyprus to Athens to Prague, Czech Republic
    Where: Athens
    When: Aug. 14, 2005
    Who: 121 passengers
Why: Before losing radio contract, pilots reported trouble with the air conditioning system. In the flight, cabin pressure failure knocked out the pilots. They made it to Athens, but the plane ran out of fuel and crashed. Crash video shows fire fighting planes controlling fires, bodies, baggage, airplane spread over a large debris field, the tail and engines relatively intact. At the time of the crash, except for one pilot (who was observed trying to land the plane), everyone aboard was comatose due to lack of oxygen.

In the case, prosecution failed to prove that German pilot Hans-Juergen Merten and his Cypriot co-pilot Charalambos Charalambous didn’t meet the minimum standards

A Cyprus court has acquitted four former airline officials of manslaughter charges.

This court case is just another example of how long aviation cases, including the criminal cases of potentially/allegedly responsible parties, can drag on in court.

What: Cool Stream Media LLC Socata TBM700 C2 en route from Teterboro airport to Atlanta-Dekalb Peachtree Airport
Where: Interstate 287 in Harding, NJ
    When: Dec 20, 2011, 10 am.
    Who: 5 aboard, 5 fatalities
Why: After taking off from Teterboro airport en route to Atlanta-Dekalb Peachtree Airport, the pilot inquired about icing conditions, and shortly afterwards, crashed on I-287 in New Jersey. The plane came down on the median and broke up just outside of Morris township. One witness heard nothing but saw an explosion. Another witness smelled burning rubber and said it sounded like a wood chopper.

The plane lost contact with ATC just before the crash.

Aboard the flight were Jerry Buckalew, 45 and Rakesh Chawla (36) managing directors of Greenhill & Co Investment Banking Firm. Buckalew’s wife Corinne and two children Jackson and Meriwether and a dog were also aboard. Witnesses say the plane was spinning and flipping, then a wing came off and the plane went straight down. Buckalew owned the plane.

No cars were stuck but the plane’s right wing landed in a tree 400 yards from the crash site..

The pilot had gotten clearance to climb to 17,500 feet. ATC said there was “moderate rime” at 17000 feet. Another controller warned of extreme icing.

The debris field covered an area half a mile wide. The northbound late of I-287 was blocked a heap of burning charred metal.

Air India’s Loss of Income and pilots

Air India’s delinquency in payroll which earlier this year led to a strike, has led to about fifty pilots quitting.

Employees were promised incentives and raises to 30,000 employees. Currently pilots and other employees are owed a month’s salary.

Civil Aviation Minister Vayalar Ravi said ” the management of the Air India was in a constant dialogue with the unions and associations of the pilots.”

In the October strike, 100 pilots threatened to quit.

    Where: Warsaw
    When: Nov 1st 2011
Who: 220 passengers and 11 crew
Why: On the first approach to Warsaw, the flight crew aborted the approach. They had gotten an unsafe gear indication for the three gear struts. They spent over an hour trying to amend the problem, but efforts to lower the gear failed, and the failure was confirmed by a visual fly by.

Captain Tadeusz Wrona and First Officer Jerzy Szwarc then nailed a gear up landing, landing on the Boeing’s belly and skidding to a stop. The runway was foamed beforehand to lower potential fire damage.

What: PNG Airlines de Havilland Canada DHC-8-102
Where: 20 km (12.5 mls) S of Madang Airport (MAG) (Papua New Guinea)
    When: Oct 13, 2011
    Who: 4 crew, 28 passengers, 4 survivors
Why: About 5.15pm, while en route from Lae to Madang, the flight went down in dense forest in bad weather. The pilots managed to send a distress call. Four survivors were pulled from the wreckage and taken to Yamad clinic. The survivors include an 64 year old Australian pilot and a New Zealand pilot and an Asian passenger who was sitting in the seventh seat, who crawled through a crack in the fuselage, and who suffered burns on 27% of his body (including back and arms). Survivors are being transported by boat to Modilon hospital in Madang.

First responders were initially blocked when they could not cross the Guabe river, then a boat suffered engine problems, then the third attempt made it to the scene of the crash. A violent storm and dense forest hampered the rescue efforts, which were facilitated by chopper.

Australia engaged four army Blackhawk helicopters to assist in the rescue. Rescuers on the Gogol river were impeded due to heavy rains. It took four boats to reach the crash site.

PNG quarantined Laes Nadzab airport and grounded the Dash 8 fleet.

Witnesses saw the plane crash on the Guabe River bank and on impact, burst into flame. Most of the passengers were local Papuan parents of Madang’s Divine Word University students. According to a resort manager who went to pick them up from the airport, six of the plane’s occupants — one passenger and five crew members – had reservations to stay at Madang Resort.

PNG Accident Investigation Commission’s spokesman Sid O’Toole talked to the media.

    PNG Airlines released the following:

POMSOX/MEDIA RELEASE – PORT MORESBY 10:30 PM THURSDAY, 13th OCTOBER 2011 – AIRLINES PNG ANNOUNCEMENT

We are sad to confirm that there has been an accident involving an Airlines PNG Dash 8 aircraft near Madang late Thursday afternoon.

Emergency services have been activated and Airlines PNG is co-operating with authorities to mount rescue and recovery efforts.

There were 28 passengers and 4 crew members on board. There appear to be some survivors while a number of people remain unaccounted for.

Airlines PNG is working with the emergency service authorities to confirm this information in more detail.

A full investigation is underway by authorities and Airlines PNG as to the possible cause of the accident.

Airlines PNG fully supports the action of local authorities at Lae who have quarantined aviation fuel at Lae airport from where the aircraft originated.

Airlines PNG has also grounded its Dash 8 fleet of 12 aircraft until further notice.

Our prayers and thoughts are now with all those affected by this very sad day for Papua New Guinea.

    Where: Newman Australia
    When: Oct 4th 2011
Who: 75 passengers and 5 crew
Why: The plane had taken off normally from Newman, but while still ascending, the engine popped. That is, it made a loud noise which was reported later by local media as a compressor stall. Inside, the cabin, passengers heard the explosion, the lights dimmed and the plane shuddered. Outside, witnesses saw a puff of smoke. Inside, passengers say it went “dead silent” for five minutes, before anyone made announcements. (No one is complaining. They’re all happy they survived.) When the captain did speak, he was calm and reassuring. No doubt he was busy at the time.

The landing hit hard and was described as being all over the runway. Passengers did joke about needing a drink afterwards, and Newman not having a bar; it was mentioned that the replacement flight should have all given them free drinks before they ran out of food.

The plane made a safe (emergency) landing in Newman fifteen minutes after takeoff.

Gulf Air Airbus Skids into Mud On landing in Kochi.

What: Gulf Air Airbus A320-200 en route from Bahrain to Kochi, India
    Where: Kochi
    When: Aug 28th 2011, 3.55 am
Who: 137 passengers and 6 crew, 8 injuries
Why: On landing in gusty wind and severe rain at Cochin International Airport, the plane skidded to the right and slid ten meters from the pavement into mud. The nose was damaged, and according to some reports, interfered with the operation of the slides; some slides were brought to the plane. Those who were injured were the ones who jumped. Said Muhamad, 47, sustained a fractured right ankle, left rib and left elbow and is recovering at Little Flower Hospital.

One report says flight 270 hit an air pocket and overshot (or landed too far down) the runway. It may have been simple turbulence. The approach appears to have been too fast on the slick runway. No mention has been made of the runway surface, or if the runway had been resurfaced.

According to Said Muhamad, the flight crew apparently did not adequately direct passengers, or explain what was going on, which may be why he did not wait for slides, and just jumped off the plane.

    The Metars:
    VOCI 282300Z 00000KT 4000 HZ FEW005 SCT015 OVC080 26/24 Q1008 NOSIG
    VOCI 282130Z 29005KT 3000 HZ SCT010 SCT080 26/24 Q1008 NOSIG

Passengers disembarked quickly after it appeared (to them) that the pilot had lost control. Evacuation occurred promptly, after “Mayday!” “Evacuation!” was shouted in the plane’s cabin.

Reports conflict as to the success of the emergency slides which did not apparently deploy perfectly, and some people jumped from the plane. There were 8 injured, one with extreme fractures.

Evacuation was reported to be in more than twenty seconds. (though I interpret that to mean that the evacuation began within twenty seconds of landing—one can hardly picture 137 passengers and six crew members making it out of a disabled plane within twenty seconds.)

The interior of the plane smelled like some kind of fumes, and the pilot was described as being in shock.

Although the English video says otherwise, the flight originated In Bahrain, and landed in Kochi.
