    Are Pilot Instructors Qualified?

The path to skill is through studying mistakes. For pilots, that means finding the cause and correction of their own and others’ mistakes. What does a recent rash of student/instructor plane crashes indicate? Is the instructor adequately preparing the student, or is the instructor adequately qualified? See Video

May 2012: 6 Near collisions over Moscow Alleged

Although the Control tower denies this number, in the past three months, Moscow ATC (union) reported six near collisions over Moscow.:

    • March 7: at Domodedovo Airport, Swiss Air jet and a Boeing 737 averted by an alarm 15 seconds before a collision would have happened. Both were landing
    • March 22: at Ostafyevo International Airport: During takeoff
    • April 25: Vnukovo International Airport: Trans Aero Boeing 737 and Lufthansa Boeing 737 nearly collided while landing.

Other jets involved are said to be Falcon-900, and ATR-72.

The incidents are presumably due to old equipment, firing of established experienced personnel, new staff and recent reorganization of the airspace routing over Moscow.

    Disaster Drill in Springfield

Congratulations to Springfield-Branson National Airport for a successful disaster.

A successful disaster drill, that is, for airport, Willard, and Springfield fire departments and Cox and Mercy hospitals.

This year staged a mock airplane on fire that had to be put out, and then actors in the wreckage portraying victims.

At the old terminal, 20 actors were passengers who were dead or injured, and first responders were able to practice significant emergency response. Springfield-Branson National Airport is a likely location for a landing disaster situation because they have a long runway designed to accommodate a big airliner being able to land.

Video shows actors in preparation for the disaster.

    Hypoxia on F-22

F-22 pilots disoriented from lack of oxygen

Pilots are concerned about developing cases of hypoxia, symptoms such as nausea and blackouts resulting from a lack of oxygen while flying the F-22. Last year, this very concern grounded the F-22 last year for five months.
They returned to service when the Air force did not find the root of the problem. Below see the clip on sixty minutes where pilots declared that they were refusing to fly the aircraft because of oxygen deprivation in the cockpit. A Virginia Air National Guard pilot in the video below admitted that he developed hypoxia on a Febuary training flight and consequently recovered in a hyperbaric chamber.

Medicated Sleepwalker Mistakes Exit Door for Bathroom

What: US Airways/ Air Wisconsin Canadair CRJ-200 en route from Portland Maine to Philadelphia
    Where: Boston
    When: May 10th 2012
Who: 50 passengers and 3 crew
Why: A passenger with a history of seizures and on medication attempted to open the door while en route.

The disruptive passenger was caught by a flight attendant, who stopped him. He told her he was going to the bathroom. The attendant led him to a seat in the rear of the plane where state police found him. Other passengers may have been upset or frightened by the incident, but under normal conditions, it’s not physically possible to open the door while in flight anyway.

Pilots diverted the flight to Boston where the disruptive passenger was offloaded, presumably by gentlemen in white coats.

Michael Ensalaco, 40, of Mooresville, N.C was charged with interfering with a flight crew. His lawyer explained that his seizure disorder is similar to sleep walking.

    In George’s Point of View

While a medical event should not be handled in the same way as a malicious act, perhaps someone with this tendency should not fly, or else they should not sleep during the flight, and also be obligated to inform the flight crew ahead of time. It could happen to anyone taking Ambien!

    Update:

    • A team of around 200 police, military and rescue workers are headed in vehicles and on foot towards Salak mountain where the plane went missing.
    • A passenger list of 36 names posted at Halim airport showed that most of the people on board were Indonesian airline and aviation representatives, as well as five journalists and a representative of French aircraft engine maker SNECMA.

What: Sukhoi Superjet 100-95 en route from Jakarta to Jakarta Halim Perdanakusuma Airport
    Where: Indonesia
    When: May 9th 2012
Who: 36 passengers, 6 crew and 2 Sukhoi official
Why: The Sukhoi Sukhoi Superjet was thirty minutes into a demo flight over Jakarta when it disappeared from radio contact. It had already flown to Myanmar, Pakistan and Kazakhstan. After Jakarta, it was supposed to visit Laos and Vietnam.

The flight has not been reported landing at any airport. According to an individual who was supposed to be on the flight, it was only to last thirty minutes, and would have run out of fuel by now, if it had been flying all this time.

At 15:30L the crew asked ATC to descend to 6000 feet in an area where there is a 6,200 foot-high mountain. The plane lost contact on a descending right turn and has been reported as missing.

No crash site has been located; it may have crashed into Mount Salak.

The plane was flown by plane was operated by senior test-pilot Aleksandr Yablontsev and co-pilot Aleksandr Kochetkov, and there were six other russians aboard, Aero navigator Oleg Shvetsov, flight engineer Aleksey Kirkin, leading test flight engineer Dennis Rakhmanov, test flight deputy head Nikolay Martyshenko, “Sukhoi” company Sales Director Evgeny Grebenshchikov, and contract manager Kristina Kurzhukova.

Hijacking is unlikely, and appears to be a media rumor.

    In George’s Point of View

Along with the rest of the world, we are hoping and praying the plane will be found intact.

One helicopter was sent out on a search mission, according to a local twitter. (See video below.)

Susi Air Crash in Indonesia Kills 2, 3rd Fatal Crash in Eight Months

What: Susi Air Pilatus PC-6/B2-H4 Turbo Porter
Where: Ritan, East Kalimantan, Kutai Kartanegara, Indonesia
    When: April 25, 2012, 5:30 pm
    Who: 2 aboard, 2 fatalities
Why: On an aerial photography flight, the Susi Air plane developed problems. South African Pilot Jonathan Willis reported engine problems and was planning to make an emergency landing. Australian Ian McDougall was aboard, performing an aerial survey. The plane crashed 60 metres shy of a probable successful emergency landing site. He had sent his company PT Surtech Utama Indonesia a text message regarding a fuel program. They had been nearing the end of a six-hour survey flight when the pilot had attempted to fly or glide to a road 300 meters long, 20 meters wide.

The owner of the airline, Susi Pudjiastuti grounded its seven Pilatus Porter planes and the owner is considering shutting down. This is Susi Air’s the third fatal crash in eight months: In September 2011, a Susi Air crash killed two in Papua; and in November of 2011, a Spanish pilot crashed at Sugapa Airport.

Both Willis and McDougall’s remains were recovered Thursday.
