    Plane Crash in South Africa Claims Three Lives

    A kit plane that crashed when its wings fell off, impacted near a landing strip in KwaZulu Natal province near Campertown South Africa, and resulted in three fatalities. A resident near the impact zone saw the plane crash sans wings.

    According to media reports, the plane, which crashed on Saturday, March 15, 2014, burned completely. A private ambulance service spokesperson said that the plane could be a four or six seater and that it is burnt beyond recognition.

    The police have confirmed the death of three persons in the incident. Three Durban residents flew it from Wonderboom International Airport where Kim Gibbings of Kloof had taken possession of the plane. He was identified as one of the victims. Gibbings is survived by an ex wife in Canada, two daughters, and a second wife.

    The Ravin 500 broke up in the air and the wreckage rained down on Emoyeni sugar plantation, leaving a debris field of eight hundred meters.

    Linhas Aéreas Flight 4896


    Noar Linhas Aéreas Flight 4896 took off from Boa Viagem, Recife, Brazil, on 13 July 2011, issued a mayday shortly after, and after experiencing trouble with one engine, the Let L-410UVP-E20 crashed 1,300 yds from the end of the runway. Two crew and fourteen passengers were killed on the scene.

    The CVR data indicate a change in the emotional state of the PIC on account of the aircraft adverse condition, to which he had to respond with an operational procedure that was different from the one for a normal flight situation.
    The copilot, in turn, in his communications, gives the impression of being highly tense on account of the emergency situation.
    This became clear with the delay of the copilot in retracting the landing gear, and in the PIC’s initiative to “feather” the propellers that had already been feathered by the copilot, after an order given by the very PIC.
    The evident anxiety identified in the cockpit is reflected in the barriers and filters that influenced the process of communication between the two pilots, sensitively affecting the PIC’s situational awareness.
    Moreover, it is a known fact that non-routine situations can cause a rise in one’s level of anxiety, but such alteration of the emotional state may be intensified when one does not have enough knowledge to manage the circumstance, and this interferes in the analysis of the scenario and adoption of appropriate measures.

    Small Plane Makes Emergency Landing in Florida; No Injuries Reported

    A small plane made an emergency landing in Everglades, Florida, on November 13th.

    The Cessna 152 plane, carrying two people, went down into a swamp on mile marker 47 near Tamiami.

    According to Broward Sheriff’s Office spokesperson Mike Jachles, “When that airplane came down into the swamp, the two men told our firefighters that the nose gear head caused the plane to flip over.”

    Both occupants of the plane remained uninjured.

    Mesa Police Department Helicopter Crash Lands in Gilbert

    helicopterA Mesa Police Department helicopter had to make a hard landing in a field near Stapley Drive & Baseline Road in Gilbert, Arizona.

    The incident happened at about 1 a.m. on June 8, when the chopper carrying two police pilots lost engine power and crashed.

    According to Mesa Police Department Sgt. Tony Landato, “They had 10 to 15 seconds from engine failure to impact…. The preliminary information I have is that the pilot did a remarkable job avoiding homes by putting the helicopter down in a field… I’ve been told, luckily, there are no serious injuries.”

    During the impact, the tail of helicopter broke off.

    Life Jacket with AirPocket Can Save Lives


    The Lifejacket Airpocket Plus (LAP) combined lifejacket and hybrid rebreather is a lifesaver. Imagine a life jacket that lets you breathe air. Genius, right? I’ve seen too many helicopter crashes to and from oil rigs. Some of those fatalities should be preventable. This hybrid life jacket would really come in handy to have on a Super Puma helicopter plunging into the North Sea, or the Gulf of Mexico. I hope I’m never in that situation, but if I am, you can bet I’ll be wearing one, and I’ll have read the directions.

    A couple of lives may not have been saved with it simply because of directions.

    A rebreather is an apparatus that absorbs the carbon dioxide of exhaled breath to permit the recycling of available oxygen. The hybrid unit is a little something extra.

    The Air Accidents Investigation Branch (AAIB) believes the instructions on how to use this device need to include the information that the rebreather has an air supply; so if someone is in the middle of drowning, they don’t have to be going down with a last breath of air. The rebreather already has some. It’s not like a scuba diving tank with loads of air, but hey, it’s a way to breathe if you’re submerged in a helicopter and don’t have gills. What if all you need is just enough air to keep you alive to get the seatbelt off, or to swim out of an opening? That’s some precious air, all right.

    So kudos to the revised pre-flight briefing material. Even if it is a knee-jerk reaction after another tragic helicopter crash, good idea AAIB.

    Missing Light Craft Discovered Crashed in NZ field

    What: Zenair CH 601 UL Zodiac microlight en route from Ohakea to Dargaville
    Where: South Head, Kaipara NZ
    When: April 1, 2012
    Who: 2 aboard, 2 fatalities
    Why: The microlight crashed in a field in South Head, Kaipara, NZ after sending a distress call at 1 pm. The flight went missing at 1, and was found before 7 when weather delayed the recovery of the bodies.

    The owner of the field (on Fuller Rd in South Head, about 31km north of Helensville) contacted authorities after finding the microlight wreckage around two. The medic who arrived by chopper found both passengers deceased. The cockpit was crushed, and the tail shattered. There was no fire. Officials are investigating but the names of the victims have not been released.
