A civilian helicopter crash, near Beccles, Norfolk,UK claimed four lives. The deceased include Lord Ballyedmond, who was a Northern Ireland peer and industrialist.
The accident happened at around 19:30 GMT, when the area was covered in thick fog. All four passengers died at the spot.
Some witnesses have reported that the helicopter crashed soon after it took off.

What: Frandley Aviation Partnership Cessna 501 Citation I/SP en route from Belfast to Birmingham

Why: On a mercy mission carrying a liver for transplant, the Cessna 501 Citation corporate jet was damaged while landing at Birmingham International Airport in heavy fog, striking the landing system glide path antennae and ending up 426 feet to the right hand side of the runway, while on fire. The plane was damaged beyond repair.
The crash closed the airport. 80 cancelled flights have affected up to 5000 passengers.
However, the liver was undamaged, and transplanted into the patient on Friday in a 4 hour operation at Queen Elizabeth Hospital. The transplant patient is reported to be in stable condition.
The pilot and passenger were injured; the 58 year old pilot suffered injuries to the chest, abdomen and pelvis. The passenger in his thirties suffered a back injury and flash burns. The Warwickshire and Northamptonshire Air Ambulance pilot entered the burning plane to cut the fuel supply so the fire service could extract the pilot. The airport is closed.
Police provided an escort to the ambulance carrying the transplant to QE.

Why: Encountering bad weather while en route, Stanley diverted to Dequeen Airport. He made a safe landing, and spent the night.
In the morning after taking off, while circling, the pilot lost control and crashed off of the east end of the 5,000-foot De Queen runway in a ditch. He died on the scene.
The National Transportation Safety Board today released the agenda for the investigative hearing on the ongoing investigation into the Aug. 14, 2013 crash of UPS Airlines flight 1354, an Airbus A300-600, on approach to Birmingham-Shuttlesworth International Airport in Birmingham, Ala.
The hearing will be held February 20 at the NTSB’s Board Room and Conference Center at 429 L’Enfant Plaza SW in Washington and begins at 8:30 a.m.
Hearing witnesses, including representatives of the Federal Aviation Administration, Airbus, UPS Airlines, Independent Pilots Association and the Transport Workers Union will testify and answer questions from NTSB Board members, technical staff, and parties about non-precision approaches, human factors and flight dispatch issues. Investigative exhibits for the hearing will be placed in the electronic docket at the start of the hearing and will also be available on the hearing event page once the hearing begins.

Television coverage of the proceedings will be by network pool provided by CNN. Escorted cutaway for video media will be permitted for brief periods throughout the hearing. The National Transportation Safety Board has scheduled an investigative hearing on February 20 into the crash of a UPS Airbus A300-600 on approach to Birmingham-Shuttlesworth International Airport in Birmingham, Ala., on Aug. 14, 2013.
The two flight crew members were killed and the airplane was destroyed when it impacted the ground less than a mile short of Runway 18. The cargo flight had originated from Louisville, Ky. Runway 18 was being used because the main runway at the airport was closed for repairs at the time of the airplane’s arrival.
The one-day hearing will examine:
Execution of non-precision approaches, including initial and recurrent training, adherence to standard operating procedures, and proficiency
Human factors issues associated with effective crew coordination and resource management applicable to this accident, including decision-making, communication, fatigue and fitness for duty, as well as monitoring and cross-checking, policies, standard operating procedures, guidance, and training provided to UPS crewmembers.
Dispatch procedures, including the training, evaluation, roles and responsibilities of UPS dispatchers and the limitations of dispatch-related software.
The investigation is ongoing and this hearing will develop additional facts to support the investigation. The hearing will be held in the NTSB Board Room and Conference Center in Washington, D.C. A detailed agenda and a list of attendees will be forthcoming.
Parties to the hearing will include the Federal Aviation Administration, UPS, Airbus, the Independent Pilots Association and the Transport Workers Union. The accredited representative from the French Bureau d’Enquêtes et d’Analyses pour la sécurité de l’aviation civile (BEA) will participate on the technical panels.
The determination of the probable cause of the crash will be released when the investigation is complete. Just prior to the start of the hearing, the public docket will be opened. Included in the docket are photographs, interview transcripts and other documents.

Why: The businessman and son of Fidel Herrera left the town of Atlacomulco after a meal with the former governor of the State of Mexico, Enrique Peña Nieto and died in a chopper crash.
Toluca Airport lost contact with the helicopter an hour prior to the accident.
The pilot Miguel Angel Rodriguez Lopez, co-pilot Rodolfo Caballero and the passenger, Armando Hinojosa-Garcia, were killed in the town of Juquipilco when the Augusta 109 chopper they were in crashed close to the Jiquipilco-Temoaya road, in the State of Mexico, North of Mexico City.
Search patrols located the wreckage early Sunday morning in a place known as “Hill of Alpine”
A Civil Aeronautics investigation is underway.

A Hawker Beechcraft BE35 plane crashed onto Long Island Rail Road (LIRR) tracks at the crossing between Hicksville and Bethpage stations on Long Island, New York, on August 16.
The plane was heading from Francis S. Gabreski Ariport, Westhampton Beach to Morristown when the pilot reported difficulty in maintaining altitude. Authorities said he was attempting to make an emergency landing at Republic Airport, Farmingdale but could not make it to there.
The pilot was killed in the crash while his only passenger, identified as Carl Giordano, 55, of New Vernon, New Jersey, sustained injuries. He was taken to Nassau County University Medical Center.
The FAA and the NTSB are investigating.

What: Swiss Airbus A340-300 en route from Miami to Zurich

Who: 227 passengers
Why: Three/fourths of an hour out of Shannon, the flight developed various electrical problems with the circuit breakers affecting cabin lighting and the inflight entertainment system. With cabin lighting off, they diverted to Shannon, where the flight made a safe landing.
Passengers were provided alternative flights, albeit not on several different planes.
