    Melbourne Training Fight: Why did it Crash?

    What: FIT Aviation LLC Piper PA-44-180 Seminole en route back to Melbourne after flying to Grand Bahama.
    Where: Palm Beach International Airport
    When: Nov 11, 2010 after 6 PM.
    Who: 4 passengers, 4 fatalities
    Why: Taking off for a practice run, the plane suffered engine failure in its left engine, caught fire and the plane crashed. Three of those aboard were killed immediately; one was taken to St. Mary’s Medical Center in West Palm Beach where he died later. The crash location is between the gates of terminals B and C east of the airport’s fire station. where most of the wreckage is covered in flame retardant.

    The Piper had been operating since 2008

    FIT is part of Melbourne’s Florida institute of Technology. A candlelight vigil was held Friday night at the Florida Institute of Technology campus for the four victims of Thursday’s plane crash.

    Indonesian Crash Kills 2

    What: PT Sinar Mas Pacific Aerospace Fletcher FU24-950 Pontianak, West Kalimantan Indonesia
    Where: Agoes Djam Hospital
    When: Dec 31, 2009 8:50 AM
    Who: Pilot and technician (Pilot Let. Col. (retired) Sigit Yudiarto and technician Titin Winarno.)
    Why: While engaged in spreading fertilizer (or spraying germ, or some other agricultural activity), the plane apparently fell from the sky and landed on the roof of the hospital, then on the ground on to a parked car.

    The roof over the hospital’s emergency area was damaged, and also one of the hospital’s cars. The hospital is operating as usual.

    Prison Transport Helicopter Crashes in Mexico, several fatalities

    What: Mexico’s Atty General’s Office Bell 412 helicopter#HFJ XC
    Where: Veracruz, Mexico
    When: December 8, 2010, 16:30
    Who: 4 fatalities, 11 aboard
    Why: While en route to Villa Aldama prison, the helicopter, carrying Pilot, copilot, six federal police, a prosecutor and two detainees crashed in bad weather, strong winds, low visibility. There were three fatalities on the scene, three injured. Later it was confirmed 4 guards were killed, the two pilots are in serious condition.

    Airblue Crash in Pakistan

    On 28 July 2010 after taking off from Karachi at 0241 UTC (0741 PST)on a domestic flight to Islamabad, Airblue Flight 202 (AP-BJB), an Airbus A321, executed a circling approach for RWY-12 at Islamabad, crashed into the Margalla Hills nine and a half miles north-east of Islamabad apparently due to bad weather. One hundred and forty-six people were aboard, and six crew. One hundred forty-six passengers and all six crew members died. The plane was a total loss. It is the first fatal accident involving an Airbus A321 and Pakistan’s worst air disaster.

    Russian Helicopter Crash Kills 3


    On Sept 9, a DOSAAF-operated Mil Mi-2 helicopter en route from Starorusskoye to Poronaysk Russia crashed with three aboard on Sakhalin Island. There were no survivors. The helicopter, registration HS-TEF, is a total loss.

    The crew was monitoring rivers when it crashed about 30 kilometers from Dolinsk.

    Emergency services responded, including the Dolinskogo fire squad, the Russian Emergency Management Services group and an Antonov An-2 aircraft out of Yuzhno-Sakhalinsk airport. THe fire was extinguished at 1:45 pm.

    The cause of the accident is under investigation and no conclusions have been drawn. Visibility and weather conditions at the time were good according to the Center for Crisis Management of the Main Directorate of the Russian Emergencies Ministry.

    DOSAAF is an acronym for the Russian Volunteer Society for Cooperation with the Army, Aviation, and Fleet.

    Ryanair Boeing Emergency Landing

    What: Ryanair Boeing 737-800 en route from Lanzarote Spain to London
    Where: Porto
    When: Jun 2nd 2011
    Why: The flight was east of porto when the pilot saw an indication of engine problems (technical malfunction.) The pilot diverted to Porto and made a safe landing at Porto.

    Passengers disembarked normally on the apron, without emergency slides.

    Media reported smoke although Ryanair said there was no smoke.

    A replacement jet was provided.
