Why: While en route to Villa Aldama prison, the helicopter, carrying Pilot, copilot, six federal police, a prosecutor and two detainees crashed in bad weather, strong winds, low visibility. There were three fatalities on the scene, three injured. Later it was confirmed 4 guards were killed, the two pilots are in serious condition.

Laser Douglas crashes in Colombia
A Laser Aereo Colombia Douglas DC-3 en route from San Jose del Guaviare to Villavicencio declared emergency on approach to Villavicencio. The plane crashed on a rural dirt road near Villavicencio and burst into flames. All of the fourteen aboard perished. The mayor of Taraira, Meta, was traveling on the crashed plane. Pilot Jaime Carrillo, co-pilot Jaime Herrera and aviation technician Álex Moreno also died in the crash.

Small Planes Crash into Each Other in Georgia; 3 Killed
Two single-engine planes collided with each other and crashed at West Georgia Regional Airport, Carrollton, Carroll County, Georgia, on September 7th.
According to sources, the two planes apparently tried to land simultaneously that resulted in the fatal crash.
All 3 people aboard both the planes were killed in the crash.
The crash remains under investigation.
Twin Engine Plane Crashes in NC Waters
On Jan 13, 2013, a private plane with two aboard taking off from Roanoke Island crashed into the water on its second approach back to the airport.
ATC made contact with the plane before it came down with the pilot making no indication of problems.
The female passenger was rescued from the water and taken to Outer Banks Hospital. The coast guard is searching for the pilot.
The plane went down between 11 and 11:15 a.m and the weather was hazy with rolling periods of fog. Visibility on the ground was less than 30 feet.
The woman was located in the water, found by rescuers who followed her screams. They were unaware that a plane had crashed.
Hatteras Island and other dive teams were searching for the Pilot in limited shifts due to the water temperature, looking from Umstead to Virginia Dare bridges. Agencies cooperating in the search included Dare County Emergency Management, Dare County EMS, Roanoke Island Volunteer Fire Department, Chicamacomico Volunteer Fire Department, Nags Head Fire and Rescue, NC Highway Patrol, NC Marine Patrol and NC Wildlife.
The search resumes monday morning.
Air Ambulance Emergency Landing
What: ORNGE AugustaWestland air ambulance en route from Billy Bishop island airport to a patient west of Toronto
Where: Colonel Samuel Smith Park south of Lake Shore Boulevard West.
When: March 23, 2012, 6:00 pm
Who: 2 pilots, 2 medics
Why: While en route to pick up a Toronto patient, the air ambulance door opened and a window blew out. The pilots had to make an immediate emergency landing near Humber College.
No one was injured, but they had been in jeopardy. Sn ORNGE insider said if the door or window hit the tail rotor “it could have been catastrophic.”
Two aircraft engineers removed the faulty door. The flight crew flew the chopper back to the airport for examination.
A known problem of the AW139 is the helicopters tail rotors. An EASA directive of Aug. 19 2011 ordered helicopter owners to replace the tail rotors after 600 hours of flight following a fatal accident “possibly caused by cracks in a TR (tail rotor) blade.”

What: Beech A36 en route from Kerrville Municipal Airport to Pearland Regional Airport, Houston
Where: Bulverde, Texas
When: Nov 8 2009
Who: Pilot Carlos McKinney Jr., 60, Dylan Lord, 13, and his 12-year-old brother, Brandon
Why: The plane encountered thunderstorms en route, lost contact at 9,000 feet and disappeared from radar. Three fatalities were confirmed; and the pilot was reportedly the children’s grandfather.
Small Plane Crash in Scotland Kills 2
A Piper PA-28 plane crashed near Loch Etive in Argyll region of Scotland, on April 4.
Authorities said the plane crashed after it lost contact in Glen Kinglass’ Beinn nan Lus area, at around 1:50 p.m. It is believed that the accident happened when the aircraft was heading from east Scotland to Hebrides Island, Tiree.
Two people were killed in the crash. Their bodies were recovered along with the wreckage.
The cause of crash is under investigation.
