What: Pawan Hans Aérospatiale AS365 N3 Dauphin Where: Chandigarh Airport India When: Dec 16, 2010 Who: 2 pilots, S P S Brar and M S Brar Why: While en route to pick up a government official Punjab Industries Minister Manorajan Kalia to Mukerian in Hoshiarpur distric, after takeoff from Chandigarh Airport, the helicopter tilted and fell to the ground, and caught afire. The helicopter had been leased to the Indian government for the past 14 years.
The pilots managed to get out of the burning helicopter alive. They have been hospitalized at Western Command Hospital at Panchkula.
An inquiry is underway.

What: Air India A-330 en route from Thiruvananthapuram to Dammam Saudi Arabia Where: Thiruvananthapuram When: Nov 10, 2010, 6 pm Who: 82 passengers including two infants Why: The flight had a normal takeoff, but forty minutes into the route, the pilot received a sensor warning about the plane’s doors. The jet returned to the airport, circled to burn off fuel, and made a hard landing during which a tire burst and the brakes jammed. The plane was stuck on the runway for four hours.
Passengers were rebooked on a Thursday morning flight out and provided overnight hotel accommodations.
Officials of the Director General of Civil Aviation have arrived to begin the official inquiry.

What: Tatarstan Air/ Yak Service Yakovlev Yak-42 en route from Yaroslavl Russia to Minsk Belarus Where: Yaroslavl When: Sep 7th 2011, 16:05 Who: 37 passengers (including the Lokomotiv Yaroslav Hockey Team) 8 crew Why: The flight took off from Yaroslavl but failed to climb, and struck an antenna outside of the airport, then struck the ground. Tunoshna ATC and other witnesses saw the flight roll (list) left and impact the ground.
Five fire brigades and a group of psychologists are on the scene.
Visibility was above 50km and it was partly cloudy according to local radar.
The plane is broken in two, the fuselage in the Volga river and the tail of the plane on the bank, a thousand feet from the airport. Reports are that the plane hit an antenna and Russian media quoted a flight traffic controller saying that the pilot failed to gain adequate height on takeoff.
Two passengers, flight engineer and the Russian player Alexander Galimov have been hospitalized with severe burns to 90% of their bodies. A third (unnamed) individual also has been hospitalized. Currently all bodies have been recovered. 103 rescue personnel were on hand, and 44 machines, including TSEPP aircraft.
The team was heading to Minsk, the capital of Belarus, where it was to play Thursday against Dinamo Minsk.
The charter flight was operated by Yak-Service Airlines, a Tatarstan Air plane. According to Russian FAA press officer, the plane “went through cursory maintenance service before departure to Yaroslavl and fully complied with all safety standards. The plane was built in 1993 and was certified airworthy until October 1.”
There has been discussion whether RA-42433 or RA-42434 was involved.

A Philippine Air Force jet crashed in Batangas province of the Philippines, at around 9:40 a.m. on January 31.
According to the Air force spokesman Lt. Col. Enrico Canaya, the single-prop SF-260FH went down off Nasugbo township during a formation training flight.
Both pilots aboard the Italian-made plane were killed in the crash. They were later identified as 1st Lt. Nazer Jana and Capt. John Bayao.
The cause of crash is being investigated.

Lion Air flight JT-610 crashed into the Java Sea in Indonesia, on October 29th.
The incident happened shortly after the Boeing 737-800 MAX plane took off from Jakarta, Indonesia.
There were one hundred and eighty-nine people aboard at the time; all of them are feared dead.
The plane was bound for Pangkal Pinang, Indonesia.

Boeing Statement on Lion Air Flight JT 610
The Boeing Company is deeply saddened by the loss of Lion Air Flight JT 610. We extend our heartfelt sympathies to the families and loved ones of those on board.
Boeing is providing technical assistance at the request and under the direction of government authorities investigating the accident. In accordance with international protocol, all inquiries about this accident investigation must be directed to the investigating authority in charge, the National Transportation Safety Committee of Indonesia.

What: Helios Airways Boeing 737-300 en route from Cyprus to Athens to Prague, Czech Republic Where: Athens When: Aug. 14, 2005 Who: 121 passengers Why: Before losing radio contract, pilots reported trouble with the air conditioning system. In the flight, cabin pressure failure knocked out the pilots. They made it to Athens, but the plane ran out of fuel and crashed. Crash video shows fire fighting planes controlling fires, bodies, baggage, airplane spread over a large debris field, the tail and engines relatively intact. At the time of the crash, except for one pilot (who was observed trying to land the plane), everyone aboard was comatose due to lack of oxygen.
In the case, prosecution failed to prove that German pilot Hans-Juergen Merten and his Cypriot co-pilot Charalambos Charalambous didn’t meet the minimum standards

What: single-engine Globe Swift (GC-1A) manufactured in 1946 in Texas en route from Rancho Murieta airport to Rancho Murieta airport Where: on the exposed dry lake bottom of Camanche Reservoir, Calaveras County When: Thursday. Who: Walter Guy Boeck, 68, of Rancho Murietta Why: The flight plan was to fly with another plane for a couple of hours and return to the airport. The other pilot reported seeing Boeck’s plane flying erratically and Boeck slumped unconscious over the controls and tracked him until he was low on fuel. The search for the missing craft began about 5 p.m. Thursday after eyewitnesses reported the aircraft going down over Lake Comanche . 9:30 a.m. Airborne searchers discovered the wreckage. Boeck’s body was discovered about 10:00 a m.
