What: Campbell Helicopters Ltd. Bell 212 Twin Two-Twelve Where: Slave Lake, Alberta, Canada When: May 20, 2011 Who: 1 fatality Why: The chopper responding to a (wildfire) fire call crashed for unknown reasons 30 meters offshore Slave Lake, Alberta. The forest fire leveled a third of the Slave Lake community. The pilot’s name has not yet been released.
The STARS Air Ambulance call was cancelled. The pilot died in the crash. Emergency responders were treated for hypothermia.

What: Southern Seaplane Where: BELLE CHASSE, La in Plaquemines Parish Oak river When: July 24 2010. Who: Instructor and student pilot Why: While the student pilot and teacher were practicing touch-and-go landings near Delacroix Island, the small plane encountered wind shear, causing it to flip over. The pilot and student on-board were on board. Two men out boating were flagged down by the instructor pilot Lyle Panepinto, 51, who survived the crash and was standing on the pontoons of the overturned plane. The student pilot, Jeffrey Wallace Dickson of Cumming, Ga, who for the U.S. Department of Interior, was trapped inside in the marsh and was taken directly to the hospital where he was declared dead.

Two Southwest Airlines jets were involved in an accident at Bob Hope Airport in Burbank, California, on June 6.
Southwest Flight 2183 was parked at the gate when Flight 4721 that was pushing back from the gate, clipped wings with it.
All the passengers were disembarked safely and both aircrafts were taken for inspection. An airline spokesperson said, “Our employees are working as quickly as possible to re-accommodate all customers… Safety is our number one focus and we will conduct an internal review to identify the details surrounding event.”
The FAA is investigating.

What: Nigerian Navy Agusta A109E Reconnaissance helicopter en route from Port Harcourt to Lago Where: Mmiri Ochiochi swams, in Obodo Isiokpo, Port Harcourt Nigeria When: Jan 26, 2010 Who: 4 Fatalities Why: While en route from Warri via Brass to Port Harcourt, the helicopter crashed into a swampy area about 2 minutes flying time from Port Harcourt International Airport. Apparently naval officers on the scene prevented locals and reporters from taking photographs.
The helicopter went into a spin, and emitted a bang before crashing. Locals on the scene used a machete to hack through a door to retrieve passengers in a rescue effort.

US-Bangla Airlines flight BS-211 hit the ground and burst into flames at the runway of Tribhuvan International Airport, Kathmandu, Nepal, on March 12th.
The de Havilland Dash 8-400 was flying from Dhaka, Bangladesh, to Kathmandu, Nepal, at the time of the crash.
The plane was carrying 67 passengers and 4 crew members at the time.
At least 39 people lost their lives in the crash, while 31 people were rescued alive with injuries.
The cause of the crash is under investigation.

On December 29, 2012, at 16:33, a Red Wings Tupelev 204-100V was on a ferry flight from Pardubice Airport Czech Republic to Moscow-Vnukova Airport, Russia when it crashed-landed in snow, overrunnning the runway. Weather conditions showed heavy gusts accompanying a light snowfall. The Federal Air Transport Agency said that the plane touched down in the proper landing area but for some reason was unable to stop on the runway.
Initially traffic controllers refused to authorise landing, and the plane had to circle the airport. Fire trucks were not on standby; they were undergoing maintenance and were nearly an hour late.
The captain, copilot, flight engineer and one flight attendant were killed. Two died on the scene, one died en route in the ambulance, and one died at the hospital. Four crew survived with serious injuries. Pieces of the plane caught fire.
The plane overran the runway and struck the M3 highway embankment, breaking into three parts. The fuselage landed on the highway 320 meters from the end of the runway. A BBC report said the aircraft was in several pieces, with the cockpit on the highway. Kiev Highway and Vnukovo Airport were both shut down temporarily.
