    Four Die in Italian Helicopter Crash

    What: dell’eliambulanza medical emergency service helicopter
    Where: Mount Cristallo/ Mount Faloria near Cortina d’Ampezzo Ski Resort
    When: Saturday, 22nd August 2009
    Who: 4 people on board: two engineers (co-pilot Marco Zago and Stefano da Forno), a doctor (Spaziani Fabrizio) and the pilot (Dario De Felipe)
    Why: The helicopter appeared to have hit a high voltage cable while flying in bad weather. The helicopter was in the process of responding to landslide near Rio Gere, towards Passo Tre Croci. Continued bad weather hampered recovery efforts.

    This would have been Zago’s last mission before transferring to another job.

    The remains have been recovered, however, and were taken to the hospitap mortuary in Cortina Codivilla. This hazardous area frequently requires mountain rescue services.

    Small Plane Crash in California Leaves Two Dead

    A small plane crashed just outside Santa Paula, California, on March 31st.

    The RV-6A aircraft was carrying two people when it crashed into a storage building and caught fire.

    Both occupants of the plane were killed in the crash. No one on the ground was injured.

    The cause of crash is being investigated.

    Smoke in Cockpit Causes Saudi Arabian Airlines Diversion

    What: Saudi Arabian Airlines Boeing 747-400 en route from Kuala Lumpur to Jeddah
    Where: Chennai
    When: Jul 6th 2011
    Who: 200 passengers
    Why: While en route, the flight developed smoke in the cockpit. Pilots diverted to Chennai and made a safe landing. Emergency services examined the plane and found no fire or heat. Maintenance examined the flight.

    The flight continued about 4 and a half hours later, and made a safe landing eventually in Jeddah

    What: Corserve International Ltd. Robinson R22 Beta II
    Where: Harrop Green Farm in Adlington, near Macclesfield, UK
    When: Nov 15, 2009
    Who: 1
    Why: While en route, the helicopter crashed. The pilot, businessman Tony Sullivan, an experienced engineer and helicopter pilot, was pronounced dead at the scene.

    The crash is under investigation.

    Four Die in Fiery Crash in Lomas de Zamora, BA Argentina.

    On Feb 08 2013, a Centro Universitario de Aviación Cessna 172R Skyhawk #LV-GEU with four aboard took off from La Matanza – Club Universitario de Aviación for a student flight and crashed in Lomas de Zamora, BA Argentina.

    The crash site is a property located at 1500 meters from the intersection of Belt Road and Avenue Juan XXIII, former Black Road, in Lomas de Zamora, in a field next to the Lomas de Zamora federal shooting range.Two passengers were expelled from the plane, and two trapped inside.

    Ambulances struggling to get to the scene got lost because they had not established exact coordinates.

    Two doctors were flying the plane. The 38 year old was piloting, and the 44 year-old was the co-pilot. Two passengers were aboard, one 45 years old, and one 20 years old. The names have not been released.

    The cause of the accident is being investigated by ANAC and the police. Buenos Aires police personnel had custody of the outer perimeter of the crash site overnight.

    Eyewitnesses say the plane ignited in the air but a pilot who was flying over during the crash say it ignited on impact with the ground.

    Firefighters, police officers technicians, and the mayor of Lomas de Zamora, Martin Insaurralde were at the scene of the accident.

    Iran Air Emergency Descent

    What: Iran Air Boeing 727-200 en route from Chah-Bahar to Zahedan
    Where: Iran
    When: Oct 23rd 2009
    Who: Not available
    Why: While enroute, the cabin pressure dropped rapidly. Oxygen masks were deployed, and the plane began an emergency descent to height where a safe pressure could be maintained. The flight continued on to its destination and landed safely.

    The crisis is speculated to be due to a malfunctioning or blocked valve in the air conditioning system.
