    Low-flying Ultralight Snaps Strut Turning Near Cliff, Plunges into Hawaii Waters

    What: Kauai Aero Sport P&M Pegasus Quick 912S en route from Port Allen Airport, Hawaii
    Where: North Shore of Kauai, 100 yards offshore near Honopu beach
    When: May 17, 2011 10:53 a.m
    Who: 2 aboard
    Why: The ultralight belonging to Kauai Aero Sport landed in the water 100 yards off shore.

    Witnesses saw the ultralight in trouble and tried to come to the aid of the two men in the crash. (see video below)

    Captain Bob Butler describes hearing a (possible) aluminum strut break, and the aircraft hit the water at a 70 degree angle. They pulled the craft to the surface but not soon enough to save those aboard.

    The bodies of both men were recovered as well as the wreckage. The pilot/owner of Kauai Aero Sport is Steve Sprague.

    The NTSB is investigating.

    American Airlines Backs Miami Dolphins Charter Into a Parked MD80

    What: American Airlines Boeing B767 en route from Dallas to Miami.
    Where: Dallas TX
    When: August 30, 2012
    Who: no injuries
    Why: According to the FAA report, during Push-back for departure, the plane hit a parked MD80.

    Oops.

    No injuries were reported. THe plane involved happened to be the Miami Dolphins charter plane scheduled to return the Dophins to Miami. The team was provided a replacement jet and they returned to Miami at 7:30 a.m. the next morning.

    Delta Flight Lands in Emergency on Wake Island

    DeltaDelta Airlines flight 611 had to land in emergency on Wake Island, after a mechanical problem was discovered in one of the plane’s engines.

    The flight, with 145 passengers on-board, was on its way from Honolulu, Hawaii to Nogoya, Japan when the engine fault was noticed. It was then diverted to the Wake Island, located northwards of Marshall Islands, where it made safe emergency landing.

    All the passengers and crew members remained injured.

    The airline sent a team of mechanics to examine and repair the aircraft.

    Spirit Airlines Emergency Landing after Ingesting Bird

    What: Spirit Airlines Flight 436 Airbus 319 was en route to Chicago’s O’Hare International Airport after leaving Fort Lauderdale-Hollywood International Airport
    Where: Fort Lauderdale-Hollywood International Airport
    When: The bird struck at 11 a.m Monday, and shortly after, the pilot returned to the airport.
    Who: 115 passengers and four crew members
    Why:struck a bird shortly after takeoff and reportedly the bird got ingested by the left engine.

    Kit plane crash kills pilot

    A RV-4 kit plane crashed en route to crashed Laughlin/Bullhead International Airport.

    The plane with the deceased pilot was discovered at 7:20 on April 20, 2013.

    The FAA collected the plane at 2:15 p.m.

    No flight plan was filed.

    Overloaded (?) Cessna Crashes in Colombia

    What: Condor Cessna TU206G Turbo Stationair
    Where: El Astillero, Cerro San Antonio de Prado, Colombia
    When: October 20, 2010, 9:40 am
    Who: 4 aboard, 4 fatalities
    Why: The plane carrying provisions to Urabá and 3 passengers flew into San Antonio Mountain, killing all aboard. A would-be passenger’s life was saved because he could not board the flight—because it was full.

    The pilot did not report an emergency to the tower.

    The pilot was Juan Guillermo Mondragon. Passengers included John Amado Pérez, Osías Sierra and Xenobia Palacios.
