Update on El Segundo Helicopter Crash

What: Heliflite Inc/Aris Helicopter Sikorsky S-58ET
    Where: El Segundo
    When: March 13, 2011. 9:30 am
    Who: pilot
Why: Earlier in March we talked about the Sikorsky that crashed while lifting an ac unit from atop the Raytheon Space and Airborne Systems warehouse in El Segundo. The injured pilot survived though he had to be extracted from the helicopter. Anthony Savedra, an employee of Excel Mechanical, pulled the pilot to safety. The helicopter capacity is 5000 pounds, and the unit that was being removed from the building was under 4500 lbs, but the helicopter apparently suffered an engine failure (the pilot said the engine when “offline”) while the operation was in progress. A witness in the video below said the helicopter made a popping noise on impact, and there was white smoke; then a few moments later, there was an explosion, and black smoke. The pilot escaped burn injuries.

The crash started a fire on two floors of the warehouse but no one was injured in the fire, because the accident occurred on a Sunday. Seventy firefighters put out the blaze within forty minutes.

An unconfirmed sighting narrowed the search parameters to Mt. Hotham. Eight helicopters and two planes searched Thursday morning until the wreckage was found at 8.40am on Thursday by a search helicopter that was also employing “radar and mobile data.”

The ATSB is investigating the fatal plane crash. The only one aboard was the pilot, whose injuries were fatal.

An Australian attorney told the press that the pilot, 60-year-old Peter Brereton from Euroa was killed in a light plane crash on his way back from dropping off spare parts for a helicopter used in the fire fighting efforts on the far south coast of NSW.

A specialized team of 4 ATSB investigators with expertise in mechanical and aeronautical engineering, materials failure and aircraft operations began investigation #AO-2013-186, a four day examination of the wreckage on the 25th, (friday).

    Clutha Helicopter Crash

The preliminary investigation of the Clutha helicopter crash revealed …

    • 95 liters left in the police helicopter
    • no evidence of engine failure
    • no evidence of gearbox failure
    • all significant components were present
    • all rotor grades were attached at the time of the crash
    • neither main blades nor tail blades were rotating

Other than ruling out what is not the cause, investigators may be no closer to determining the reason the helicopter fell out of the air. The report says there was “no evidence of major mechanical disruption of either engine” of the Eurocopter EC135 as it returned from a police operation in Dalkeith, Midlothian, on the night of the crash.

There were no flight recorders, but there is radar data and radio communications. The pilot had 5,500 hours, 646 in type.

The wreckage is at AAIB’s headquarters, and the investigation continues.
