On November 27, 2008 at 16:46 local time (15:46 UTC), an Airbus A320, equipped with International Aero Engines (IAE) V2500 engines, German registration D-AXLA, serial number 2500, impacted the Mediterranean sea during approach to runway 33 at Perpignan Rivesaltes airport (LFMP) in Perpignan, France. The aircraft impacted the sea approximately 7 kilometers from the coastline and is reportedly submerged 30-40 meters below the surface of the water. The French Navy is currently conducting recovery efforts, which include the use of remotely controlled vehicles (ROV).

The Bureau d’Enquêtes et d’Analyses pour la Sécurité de l’Aviation civile (BEA) of France is investigating the accident. As the state of design for the IAE V2500 engines, the NTSB will send a U.S. Accredited Representative to assist the BEA with their investigation. An NTSB powerplants specialist and representatives from the Federal Aviation Administration (FAA) and IAE will accompany the U.S. Accredited representative as Technical Advisors.

    Two Fatalities in Portuguese Cessna Crash on Training Flight

    Who: 2 fatalities
    Why: A pilot of the Leávia flight school and a student pilot had made one pass at the runway at Tires aerodrome in Cascais, and had circled to make another pass when they crashed on a training flight. The plane impacted between residences in Matarraque, Sao Domingos de Rana, Portugal.

    Judging by proximity, investigators conjecture that the pilot may have been intending to make an emergency landing on the A5. The plane ended up in a yard between two houses on Saudade street about two miles from the airfield. The plane crashed but did not burn. There were no casualties on the ground. Emergency responders included corporate volunteer firefighters and and an ambulance from the National Institute of Emergency Medicine.

    The instructor was 45 and his student was in her twenties.

    Witnesses say they heard the engine stop before the plane crashed. The “Gabinete de Prevenção e Investigação de Acidentes com Aeronaves” is investigating.

    What: Japan Airlines Airbus A300-600 en route from Aomori to Tokyo Haneda
    Where: Haneda Airport
    When: Apr 17th 2009
    Who: 120 people
    Why: After touchdown at Haneda Airport, the engine started smoking; flames were shooting out of the exhaust. The crew engaged the ” engine fire suppression system.” Passengers did NOT debark via emergency procedures.

    Ukranian Cargo Plane Crashes in Egypt

    What: Ukrainian Russian-made Antonov An-12 cargo plane [registration# S9-SVN] crashed during takeoff en route from Entebbe, Uganda to the Ukraine, on it’s way to Nikolaev, an avia-repair plant
    Where: Luxor, Egypt
    When: Friday Feb. 20, 2009
    Who: – Five crew members died
    Why: On liftoff, the plane slammed to the tarmac, then slid down and off the runway and hundreds of feet from the runway burst into flames. The plane had a full tank.

    The crew included one person from Russia, two from Belarus and two from the Ukraine. The plane’s black box data recorder were recovered.

    Germanwings A320-200 Airbus Crashes in France

    A Germanwings flight 4U-9525 Barcelona-Dusseldorf lost contact with ATC at 10:45. 144 passengers and 6 crew were lost—most likely German and Spanish passengers.The wreckage has been located between Prads-Haute-Bleone and Barcelonnette in France in a mountainous region.

    Germanwings and Lufthansa operated the flight.

    The area where the wreckage was spotted is difficult to access, but it is an area known for hiking and skiing.

    We must confirm to our deepest regret that Germanwings flight 4U 9525 from Barcelona to Dusseldorf has suffered an accident over the French Alps. The flight was being operated with an Airbus A320 aircraft, and was carrying 144 passengers and six crew members. Lufthansa and Germanwings have established a telephone hotline. The toll-free 0800 11 33 55 77 number is available to all the families of the passengers involved for care and assistance. Everyone at Germanwings and Lufthansa is deeply shocked and saddened by these events. Our thoughts and prayers are with the families and friends of the passengers and crew members.

    INFORMATION ON 24 MARCH 2015

    In accordance with the provisions of European Regulation 996/2010 the BEA has initiated a Safety Investigation after having been informed, at the end of the morning, that an Airbus A320-211 crashed near the commune of Prads-Haute-Bléone (Alpes de Haute-Provence, France).

    The aeroplane, registered D-AIPX, operated by Germanwings, flight GWI18G, was flying the route from Barcelona (Spain) to Düsseldorf (Germany). According to information from the airline, there were 144 passengers and 6 crew members on board.

    Seven investigators from the BEA, accompanied by technical advisers from Airbus and CFM International, are travelling to the accident site. They will be joined by a team of three investigators from the BFU (Bundesstelle für Flugunfalluntersuchung) the BEA’s German counterpart.

    A press conference will be organised at the BEA tomorrow, Wednesday 25 March, from 16h to 16h45. Journalists who wish to attend are asked to confirm their presence with Sonia Festou, sonia.festou@bea-fr.org
