    Three Children, Three Adults lost in Arizona Crash

    What: Ponderosa Aviation Inc. Rockwell Commander 690A en route from Mesa, Ariz to Safford, Arizona
    Where: Apache Junction, Pinal County, AZ
    When: Nov 23, 2011
    Who: 6 fatalities
    Why: On a thanksgiving flight that took off at 6:15, the pilot was flying to Safford and struck a rockface at the Superstition mountains. The children’s mother, Karen Perry, was not aboard. The fuselage is jammed between rocks.

    The victims of the crash included Joseph Hardwick, 22, mechanic; pilot and aircraft owner Russell Hardy, 31; pilot Shawn Perry, 39; and Perry’s three children, Morgan, 9, Logan, 8, and Luke, 6.

    The plane had just left Mesa’s Falcon Field when it crashed. It is a six hour hike from a campground to the wreckage. A ten member sheriff’s rescue team was flown in by chopper, and hiked down from a mountaintop.

    The flight path that the pilot took is a controversial one. The FAA redesignated Phoenix airspace, raising the floor altitude for commercial traffic at almost the same height as the tallest peak in the Superstitions (5,057 feet). Planes such as this one flying east from Mesa’s Falcon Field must either go around the peak or request permission to enter commercial airspace to fly above it.

    Hawaiian Airlines Plane Makes Emergency Landing in Seattle

    Hawaiian AirlinesHawaiian Airlines flight HA-21 made an emergency landing in Seattle, Washington, on July 24th.

    The Airbus A330-200 plane took off for Honolulu, Hawaii, but had to return shortly afterwards after the crew reported an issue with windshield heating.

    The plane landed uneventfully. No injuries were reported.

    Smoke Sends US Airways Passengers on a Bus

    What: US Airways Boeing 737-400 en route from Dallas to Charlotte.
    Where: Greenville-Spartanburg International Airport
    When: April 18, 2011, 1 pm
    Who: 94 passengers, 5 crew (also reported as 107 aboard)
    Why: After pilots smelled smoke in the cockpit, they diverted the flight to Greenville-Spartanburg International Airport where they made a safe landing with local emergency responders as well as fire crews from Pelham-Batesville and Boiling Springs, and Greenville and Spartanburg EMS were on standby when the plane landed.

    After the plane was grounded, passengers were taken by bus to Charlotte.

    British Airways Emergency Diversion to Shannon

    What: British Airways Boeing 777-200 en route from London Heathrow to Boston,MA
    Where: Shannon
    When: May 23rd 2011
    Who: 221 passengers
    Why: En route over the Atlantic, smoke developed in the cockpit and the pilots declared emergency and diverted to Shannon.
    (There is some speculation that the smoke was ash drawn into the cabin.)

    The smoke increased on descent, but the pilot made a safe landing. The smoke is being blamed on an undesignated mechanical problem.

    Above FL200, the atmosphere was (reported as) clear of Grimsvotn ash.

    Passengers were provided hotel accommodations until the replacement flight arrived (May 24.)

    Helicopter Pilot Dies during Fair

    Carl R Enlow was killed by a helicopter rotor at the Bloomsburg Pennsylvania town fair.

    Enlow was a former military helicopter pilot who was giving rides. He was struck after his shift, in an accident. He was med-evaced by LifeFlight helicopter to Geisinger Medical Center where he died.

    The Heritage Rotors LLC Enstrom F-28C helicopter was standing at the time of the accident, in the parking lot near gate five was in the process of gassing up between rides. Enlow was returning to the helicopter to speak to the next pilot when apparently his hat blew off and he reached for it.

    The Bloomsburg Fair Association is donating their profit from the rides to Enlow’s widow, and a memorial fund is being arranged.

    Eurocopter Medevac Crew Perishes in AZ Neighborhood

    What: AirMethods American Eurocopter AS 350B3 Ecureuil en route from Marana to Southeast Arizona Medical Center
    Where: Near Park and 1st Avenue, Tucson, Arizona
    When: July 28, 2010, 2 pm
    Who: pilot, flight nurse and paramedic
    Why: On a medical helicopter service LifeNet Arizona flight, the crew had recently spoken to ATC when the helicopter crashed in a residential neighborhood behind a house. A witness said the pilot directed the helicopter away from the house; and that the engine made “popping noises” before it went silent. It’s rotors stopped, and it fell from the sky. No patients were aboard, but the crew died on the scene.
