What: Virgin Australia/Pacific Blue Boeing 737-800 en route from Brisbane,QL to Denpasar Bali Where: Brisbane When: Oct 14th 2011 Who: 1st officer Why: The plane was in its parking bay as the first officer was inspecting the plane prior to departure, when the exhaust of a departing Qantas jet blew the mobile stairs, including the first officer, over. Both stairs and pilot ended up on the tarmac, the first officer ending up with a broken arm and leg. The plane was in its parking bay. The pilot departed for the hospital, presumably by ambulance, and the plane departed for Bali, with only a 90 minute delay.
I don’t think I’ve ever heard of this happening!

What: KLM Boeing 777-200 en route from Atlanta,GA to Amsterdam Where: Amsterdam When: Feb 3 Why: After a successful flight from Atlanta to Amsterdam, the flight crew reported a flat tire prior to landing.
* (The plane’s indicators include a tire pressure indicator. As a rule, when 5 of the 6 main landing gear tires are correct, the pressure is adequate for landing.)
However, they made a successful landing and taxied to the gate.

During an early inspection of the 787 in Takamatsu by Japanese officials, the battery’s electrolyte had leaked from the lithium-ion battery.
The charred insides of the battery show the battery received voltage in excess of its design limits.
In October 2011, the FAA issued an emergency order requiring lithium ion batteries in 42 Cessna Citation CJ4 to be replaced with nickel-cadmium or lead-acid batteries, after a connection with a ground power station led to overcharging and a fire.
Wait…was this human error and not battery error? Or a combination of both?
