    Potentially unsafe situation in Crimea area lives up to potential. Statement from Maylaysia Airline, Partial Manifest, Cargo

    ICAO’s take is that the air traffic control region where the passenger plane came down was believed to be under the control of the Ukraine, and they will be pointing blame at the Ukraine for its internal strife.

    The Ukraine is saying…but the ICAO said it was safe.

    Other countries were already avoiding the hot zone. We should ask the ICAO why they were professing it was safe when it was so obviously not.

    Let’s all remember that greasing the wings of airline commerce does not mean greasing up the safety glasses too.

    Malaysia Airlines Flight 17 was in the wrong place at the wrong time. Safety measures failed because of corporate optimism, cutting corners and who knows what other reasons. Prepare for the mudslinging and blame throwing to begin. Someone pulled the trigger, and someone let it happen. Someone will be held accountable.

    1. Flight plan

    MH17’s flight plan was approved by Eurocontrol, who are solely responsible for determining civil aircraft flight paths over European airspace. Eurocontrol is the air navigation service provider for Europe and is governed under ICAO rules.

    The route over Ukrainian airspace where the incident occurred is commonly used for Europe to Asia flights. A flight from a different carrier was on the same route at the time of the MH17 incident, as were a number of other flights from other carriers in the days and weeks before. Eurocontrol maintains records of all flights across European airspace, including those across Ukraine.

    In April, the International Civil Aviation Organization identified an area over the Crimean peninsula as risky. At no point did MH17 fly into, or request to fly into, this area. At all times, MH17 was in airspace approved by the ICAO.

    2. Altitude

    MH17 filed a flight plan requesting to fly at 35,000ft throughout Ukrainian airspace. This is close to the ‘optimum’ altitude.

    However, an aircraft’s altitude in flight is determined by air traffic control on the ground. Upon entering Ukrainian airspace, MH17 was instructed by Ukrainian air traffic control to fly at 33,000ft.

    3. Nationalities

    Following this afternoon’s press conference, Malaysia Airlines can confirm that a further 16 passengers’ nationalities have been verified. The latest breakdown of nationalities of those on board the flight is as follows:

    · 189 Netherlands

    · 44 Malaysia

    · 27 Australia

    · 12 Indonensia

    · 9 UK

    · 4 Belgium

    · 4 Germany

    · 3 Philippines

    · 1 Canada

    · 1 New Zealand

    Four passengers’ nationalities remain to be verified.

    4. New flight route

    Following this incident, Malaysia Airlines now avoids Ukrainian airspace entirely, flying further south over Turkey.

    Third NTSB Investigative Update on Boeing 787 Battery Fire in Boston

    WASHINGTON – The National Transportation Safety Board today released a third update on its investigation into the Jan. 7 fire aboard a Japan Airlines Boeing 787 at Logan International Airport in Boston.

    The lithium-ion battery that powered the auxiliary power unit has been examined in the NTSB Materials Laboratory in Washington. The battery was x-rayed and CT scans were generated of the assembled battery. The investigative team has disassembled the APU battery into its eight individual cells for detailed examination and documentation. Three of the cells were selected for more detailed radiographic examination to view the interior of the cells prior to their disassembly. These cells are in the process now of being disassembled and the cell’s internal components are being examined and documented.

    Investigators have also examined several other components removed from the airplane, including wire bundles and battery management circuit boards. The team has developed test plans for the various components removed from the aircraft, including the battery management unit (for the APU battery), the APU controller, the battery charger and the start power unit. On Tuesday, the group will convene in Arizona to test and examine the battery charger and download nonvolatile memory from the APU controller. Several other components have been sent for download or examination to Boeing’s facility in Seattle and manufacturer’s facilities in Japan.

    Finally, examination of the flight recorder data from the JAL B-787 airplane indicate that the APU battery did not exceed its designed voltage of 32 volts.

    In accordance with international investigative treaties, the Japan Transport Safety Board and French Bureau d’Enquêtes et d’Analyses pour la sécurité de l’aviation civile have appointed accredited representatives to this investigation. Similarly, the NTSB has assigned an accredited representative to assist with the JTSB’s investigation of the Jan. 15 battery incident involving an All Nippon Airways B-787. Both investigations remain ongoing.

    Singapore Airlines Plane Diverts to Indonesia due to Smoke Indication

    Singapore airlinesSingapore Airlines flight SQ-7108 had to divert and make an emergency landing at Denpasar International Airport in Bali, Indonesia, on October 26.

    The Boeing 747-400 freighter aircraft was transporting 2,186 sheep from Sydney, Australia to Kuala Lumpur, Malaysia, when the crew received smoke indication in a cargo bay and declared emergency.

    The plane landed safely. All four crew members aboard remained unhurt.

    It is believed that the exhaust gasses and sheep manure was the cause of smoke indication.

    The plane resumed its journey after 2.5 hours.

    Condor Flight Rejects Takeoff From Mombasa

    Condor flight DE-2282 had to reject takeoff from Mombasa, Kenya, on May 29th.

    The Boeing 767-300 plane was accelerating to takeoff for Zanzibar, Tanzania, when one of the engines ingested birds, prompting the crew to reject takeoff

    The plane safely returned to the apron.

    All two hundred and thirty passengers and ten crew members remained unharmed.

    What: Continental Airlines Boeing 777-200 en route from Newark,NJ (USA) to Hong Kong

    When: Mar 10 2010
    Who: 301 aboard
    Why: After takeoff, the Boeing encountered a flock of geese, with bird strikes on the right wing and nose. The jet circled the airport to burn off fuel, then landed safely.

    TuiFly Plane Rejects Takeoff in Spain due to Blown Tyres

    TuiFly flight X3-2149 had to reject take-off at Fuerteventura Airport, Spain, on September 18th.

    The Boeing 737-800 en-route to Hanover, Germany, had to abort taking off because its both left tyres had separated from the wheels.

    The plane returned safely to the apron.

    All 170 people aboard remained safe.
