    Jet Blue Emergency Landing in Baltimore

    Why: THe Jet Blue plane made an emergency landing at BWI Thurgood Marshall Airport after getting a mechanical alert.

    There was an odor in the cockpit.

    Emergency vehicles were standing by.

    Passengers were evacuated via emergency slide on to the tarmac. No injuries were reported.

    Kelona Flightcraft Makes Hot Landing with Wrong Engine On Fire

    What: Kelowna Flightcraft Boeing 727-200 en route from Edmonton,AB to Hamilton,ON
    Where: Edmonton Canada
    When: Aug 19th 2011
    Who: flight crew on freight flight
    Why: After takeoff, the crew got an indication that one engine was hot.

    The crew shut down the engine before fire suppression engaged.

    They returned to Edmonton and made a safe landing, if a landing can be called safe with flames shooting out of an engine.

    The emergency crews dealt with the engine that had been reported on fire, but actually it was another engine entirely which had been the source of the flames.

    We aren’t entirely sure whose wires were crossed, the flight system or a human reading the flight system. What matters for the time being, is that there were no injuries. It will be interesting hearing how the investigation goes on this.

    MCAS At the forefront of Boeing 737 MAX Controversy


    Everything that is essential to a plane’s operation should be included in pilot training. The pilot must know his tools backward and forward. What happens when that training is curtailed?

    On March 19th, the US Department of Transportation announced that its inspector general will audit the 737 Max 8’s certification process. Some pilots say they know how to address problems with the 737 Max 8’s maneuvering characteristics augmentation system (MCAS) system. Pilots say the MCAS software system can (apparently) be disabled by hitting the trim switch on the control column. Difference training is required for pilots who fly the Max, but apparently (MCAS) explanations, operations, procedures related to the differences were left out of the manual. The flight manual of Boeing’s 737 Max 8 planes mentions MCAS computer system only once. If MCAS information was cut from the manual to prevent the MCAS from having to be included in 737 Max transition training, whoever cut it considered only that it would save 737 Max operators training costs, but not what it would cost in terms of human lives.

    Jet Airways Plane Makes Emergency Landing in Mumbai

    Jet AirwaysJet Airways flight 9W 354 had to make an emergency landing at Mumbai airport in India, on March 3rd.

    According to the airline, “Jet Airways flight 9W 354 on landing in Mumbai reported a technical fault with the main landing gear.” It is believed that the plane’s main landing gear collapsed upon landing, also causing damage to the right wing.

    All 127 people aboard the plane remained uninjured.

    Where: off Golden Gardens Park in Shilshole Bay

    Why: When Collins experienced problems with his landing gear, he landed on Shilshole Bay, and his plane flipped. This post’s title is a quote originating from Mike Pugel with the Seattle Police Harbor Patrol who was on the scene.

    Smoke and Other Issues in Flight

    Just like it happens with cars, obscure plane parts wear out. Take for example an incident that happened in 2008, when a 747 cable burned out after the protective covering was worn off due to friction against a bolt.

    Take a look at this final report on this 747 smoke event on Eva Airways Flight BR67 at Bangkok Suvarnabhumi International Airport.

    Prior to this event, Boeing had sent out a service Letter to inform operators of the potential fire hazard from the arcing of a wire bundle which might result in a fire on Corrosion Inhibiting Compound (CIC) contaminated insulation blankets. They predicted it. It happened.

    So as we think about Malaysia Airlines Flight 370. There is always the possibility that some small component like the one below failed, and led to something catastrophic. What if the pilots were overcome by smoke in the cabin? I am not saying that this DID happen, I am only saying there are myriad small parts that can lead to catastrophic events. It makes sense to look at any and all Boeing 777 safety advisories, in case they might predict some small event that cascaded into disaster.
