    Dana Air History Comes Out at Inquest

    While testifying at the coroner’s inquest before Lagos State Coroner Mr. Oyetade Komolafe, Tony Usidamen of Dana Air revealed that the Dana plane (registration number 5N-RAM) that crashed in Lagos in June 2012 had its engine replaced after a bird strike while outbound from Lagos on April 19, 2010.

    Records indicate 5N-RAM also had hydraulic leaks on May 23 at Murtala Mohammed Airport.

    On the June 3rd, the plane was making it’s third round trip and crashed close to it’s destination airport.

    The Accident Investigation Bureau (AIB) investigator is Mr Emmanuel Dialla. His preliminary report said that the first sign of trouble came 17 minutes after the plane was airborne at 2.58pm, following a “non-normal” engine condition. According to the CVR at 1543:27 hours (3.43pm), the Captain informed the FO, “we just lost everything, we lost an engine. I lost both engines”…

    Pilot suspended

    A Kabo Airlines Boeing 747 which carried 512 passengers from Kano to make an emergency landing in Sokoto sustained tire damage, deflated tires, and damaged instrument landing systems on landing at Sokoto.

    The pilot who flew that plane and made the emergency landing has been suspended.

    The Director-General of Nigeria Civil Aviation Authority made the suspension because the pilot failed to obey the air traffic controller’s directive to land on runway 08, but landed on a different runway.

    United Airlines Flight Makes Emergency Landing in Chicago

    United Airlines flight UA-2192 made an emergency landing in Chicago, Illinois, on May 29th.

    The Boeing 737-900 plane flying from Tampa, Florida, was on final approach to Chicago when the crew declared an emergency due to issues with flaps and slats.

    The plane landed safely. All passengers and crew members remained unharmed.

    BA Flight Diverts to Belfast International Airport

    British airwaysBritish Airways flight BA1421 had to divert and make an emergency landing at Belfast International Airport in Belfast, Northern Ireland, on October 27.

    The plane took off from George Best Belfast City Airport and was heading to Heathrow Airport, London, when the pilot declared emergency. According to an airline spokesperson, “The flight landed normally after diverting shortly after take off as a precaution following a technical issue.”

    There were 108 people aboard at the time; all of them remained unhurt.

    China Southern Airlines Plane Diverts to China after Engine Shut Down

    China SouthernChina Southern Airlines flight CZ-3688 had to divert and make an emergency landing at Hohhot Baita International Airport, Inner Mongolia, China, on July 4th.

    The Boeing 737-800, en-route from Beijing, to Guiyang, in China, had to divert due to some issue with on of its engines. The crew had to shut the engine down mid-night.

    The plane landed safely.

    No one was injured.

    Northwest Airbus Loses Contact; Overshoots Destination

    What: Northwest Airlines Airbus A320-200 en route from San Diego, to Minneapolis
    Where: Minneapolis
    When: Oct 21st 200
    Who: 144 passengers 5 crew
    Why: The Airbus was out of communication for nearly an hour and a half, and overflew Minneapolis by 15 minutes. The plane had been in contact with Denver but lost contact southeast of Denver,CO after 5:56pm. Denver ATC conferred with Minneapolis ATC. Without reestablishing contact, just before 8 p.m., the plane flew over the airport.

    Military jets were put on standby.

    Meanwhile, on board, passengers had a very quiet flight, without input from the captain.

    Before communications were re-established at 8:14 p.m. the plane had overshot the airport in an easterly direction by nearly a quarter of an hour. Contact was reestablished over Eau Claire, Wis. It is not known if pilots fell asleep.

    Pilots didn’t discover their mistake until a flight attendant in the cabin contacted them over the intercom. Passengers were looking at their watches and wondering when the flight would end. Even after they landed, passengers were told to wait, eventually walking past a line of security officers.

    On landing in Minneapolis, police boarded the plane, as per standard policy.

    The two pilots are suspended until Delta/Northwest investigates the situation. Interviews of the pilots and rest of the crew are pending.
