From the NTSB to the FAA to an MD-11 near you:
- Work with Boeing to (1) assess the effectiveness of flare cueing systems to assist MD-11 pilots in making timely and appropriate inputs during the landing flare
- (2) provide a formal report on the findings of the assessment, and
- (3) if the assessment shows that flare cueing systems could be useful to MD-11 pilots, provide copies of the report to all US operators of MD-11 airplanes and encourage them to install such a system on these airplanes.
- Work with Boeing to (4) assess methods for providing weight-on-wheels cueing to MD-11 pilots to enhance pilot awareness of bounced landings and facilitate proper pilot reaction and effective control inputs when bounced landings occur,
- (5) provide a formal report on the findings of the assessment, and
- (6) if the assessment shows that the weight-on-wheels cueing methods could be useful to MD-11 pilots, provide copies of the report to all US operators of MD-11 airplanes and encourage them to provide a means for weight-on-wheels cueing for these airplanes. (A-14-005)
- Work with Boeing to (7) evaluate the effect of brief power increases on simulated MD-11 landing distances, adjust the values in published MD-11 landing distance tables accordingly, andprovide the adjusted values to MD-11 operators. (A-14-006)

Why: While over the Atlantic, the right engine developed problems. Pilots shut it down, and diverted to Shannon where they made a safe landing. Passengers were provided an alternative flight.George’s Point of View
I usually try just to get the facts. But when people read these alerts, it is not just the plane. Like on this flight. Can you imagine being aboard this flight, over the Atlantic—as on the infamous Air France Flight 447—and losing an engine? Can you picture your feelings at the moment you realize the failure, and every moment thereafter, until you were safely on the ground? Even with a capable functioning plane, and competent crew. Even with the knowledge that the plane could afford to shut down an engine. Having to sit politely in your row, and believing on some level, that you were placidly waiting to die while strapped in like a trapped like a Chilean Miner in a snow globe hurtling through space toward who knows what fate? Even if I do not write it, behind these lines, there is always a human element.

Who: 318 passengers, 16 crew
Why: After a louder than normal take-off, fifteen minutes into the flight, pilots of the Air Canada Boeing en route from Toronto to Japan shut off an engine. Witnesses reported smoke and fire coming from the right engine.The flight had to return to Toronto following engine problems. Falling bits of engine debris “hot bits of blackened metal” fell from the sky and caused some damage to parked vehicles near Derry and Kennedy Rds in Mississauga. The entire circuit of the flight took about an hour and a half.
There were no injuries on the flight, but emergency services were on standby on landing, in expectation of trauma.

El Al Tel Aviv Flight Returns to Heathrow on Three Engines

Why: While en route, pilot Ilan Margalit felt that one of the right side engines was losing power as it reached 26,000 feet. He shut down the affected engine and returned to London where he made a safe landing at Heathrow.The Israel Times reported that the engine caught fire; however other sources say there was no actual fire. The plane landed safely on three engines. A replacement flight was provided on July 5.
The affected engine was a Pratt & Whitney.
In April of 2007 this plane was damaged when it “taxied over its pushback tractor.” No one was hurt in that incident either.

Gol Airlines Flight Makes Emergency Landing in Brazil
Gol Airlines flight G3-2020 had to return and make an emergency landing in Brasilia, Brazil, on January 22nd.The Boeing 737-700 plane took off for Boa Vista, Brazil, but had to return shortly afterwards after the crew failed to retract landing gear.
The plane landed safely. There were 130 people aboard at the time; all of them remained unharmed.
