Angle of Attack (AoA) Probes Faulty:
Testing shows that data from two different Airbus planes were found inaccurate.
Investigation conducted by Airbus and Thales on the removed probes revealed oil residue between the stator and the rotor parts of the AoA vane position resolvers. This oil residue was the result of incorrect removal of machining oil during the manufacturing process of the AoA resolvers. At low temperatures, this oil residue becomes viscous (typically in cruise), causing delayed and/or reduced AoA vane movement. Multiple AOA probes could be simultaneously affected, providing incorrect indications of the AoA of the
aeroplane. If not corrected, this could lead to erroneous AOA information and consequent delayed or non-activation of the AoA protection systems which, if during flight at a high angle of attack, could result in reduced control of the aeroplane.Pilots Avoid Weather Towers

In case you didn’t know, planes should not fly into Meteorological Evaluation Towers.The NTSB has just issued a warning for pilots to avoid these towers, which at 200 feet are below the threshold for markings. The biggest danger, apparently is that they can be built quickly and whoever builds them is not required to inform aviators.
So pilots, if you didn’t know to avoid them, you know now. Read the alert here.
Small Plane Crashed at Granbury Municipal Airport; 2 Killed
A single-engine plane crashed at Granbury Municipal Airport in Hood County, Texas, on September 24.
Authorities said the plane went down and burst into flames just a few hundred yards from Granbury High School.
There were 2 people aboard at the time; both of them were killed in the crash.
The NTSB and the FAA are investigating.

EASA Proposes Fatigue Across the Board
New European rules ‘harmonizing’ the workload limits across 27 member states could allow pilots to fly aircraft for 22 hours without sleep, increasing a pilot’s work day from 16 hours 15 minutes to 20 hours, and the maximum shift time for a long haul flight with two pilots from 12 to 14 hours, as well as eliminating the need of a third pilot on long-haol flights.
The proposals have raised the attention of BALPA, The British Airline Pilots Association.
The seventeen percent increase in workload will result in a 5.5% higher chance of an accident.
It is inconceivable how EASA can call “flying farther with less rest-time, more frequently (7 starts in a row), no back up crew and more fatigue” bringing standards “up” when it is actually leveling down safety standards. Such a workload flies in the face of the constructs of human biology.
But the UK’s Civil Aviation Authority (CAA) “has said they have no fundamental problem with the rules.”
Aviation Safety Action Program
Because of management issues, labor vs management issues, and the question of how much protection should pilots/crew have when they make a mistake, a number of big carriers have dropped the Aviation Safety Action Program. THe official goal of the Aviation Safety Action Program (ASAP) is “to enhance aviation safety through the prevention of accidents and incidents. Its focus is to encourage voluntary reporting of safety issues and events that come to the attention of employees of certain certificate holders.”
But pilots are being punished for their honesty.
The program that is being dropped allows pilots and other airline workers to report mistakes they’ve made and safety issues.
This enables carriers to spot trends before they become safety issues.
So..why have Delta, American, U. S. Airways and Com Air dropped the program?
Should we even FLY these carriers if they are not following ASAP?

Senior investigators from the National Transportation Safety Board will hold a training session about the lessons that have been learned from several recent helicopter accident investigations at the 2014 Helicopter Association International’s Heli-Expo in Anaheim, Calif., next week.
Investigators will present case studies that emphasize NTSB safety recommendations related to helicopter maintenance, flight simulators, and flight recorders. The case studies will be followed by industry-led panel discussions on best practices in the industry. NTSB Vice Chairman Christopher Hart will provide opening remarks for the session.The preliminary schedule is posted below:
