What: helicopter en route to SeaBird Exploration’s seismic vessel Kondor Explorer
Where: Agbami field offshore Nigeria
When: Dec 11, 2009
Who: 18 on board (16 crew, 2 pilots)
Why: After the helicopter made a controlled “landing” and ditched into the sea, all 18 crew members aboard were rescued.

No word has been released about why the helicopter ditched.

The Agbami seismic project is run by Chevron.

    What: SpiceJet Boeing 737-900 en route from Mumbai to New Delhi

    Why: About an hour after take-off, when the plane stopped maintaining “differential” pressure, the crew decided to return to Mumbai for repairs.

    On landing, the plane was grounded for maintenance and passengers were provided alternate transportation.

    Blown tire and Engine Down Leads to Emergency Landing in Newark

    What: United Boeing 757-200 en route from Newark to Berlin
    Where: Newark
    When: Aug 18 2012
    Who: 173 aboard
    Why: On takeoff, the plane blew a tire. Simultaneously, the plane gave feedback of problems with both engines, with witnesses seeing fire coming from the left engine. That engine was shut down. The plane managed to circle for two hours to burn off fuel before returning to make a safe landing in Newark.

    I should probably throw in my usual nag Maintenance! Maintenance! Maintenance! while at the same time praising the CRM and skill of the pilots. This could have certainly come out a much worse report, except that the pilots knew exactly what to do to have a safe outcome.

    Air France Paris-New York Flight Turns Back Due to Ice

    What: Air France Airbus A330-200 en route from Paris to New York
    Where: Paris
    When: Dec 25th 2011
    Who: 187 passengers and 12 crew
    Why: On takeoff from Paris, the crew detected problems.

    The engine anti-icing system indicated a problem, so the pilots returned to Paris two hours after leaving.

    An anti-ice system valve was apparently repaired. After two more hours, the flight took off for New York.

    Brownsville Medical Helicopter crash


    What: Hospital Wing Eurocopter AS.350B3 Ecureuil helicopter en route from Jackson-Madison County General Hospital to Brownsville
    Where: 100 yards off Springfield School Road, about four miles east of downtown Brownsville Tennessee
    When: MARCH 25, 2010
    Who: 3 on board (no patients)
    Why: Dispatchers lost contact with the helicopter crew before 6 a.m. A lightning strike and a fireball was seen near the field where the wreckage was found, a location that was found by GPS.

    What: TAM Linhas Aereas Airbus A330-200 en route from Miami, FL to Sao Paulo Brazil
    Where: en route to Sao Paulo
    When: May 21st 2009
    Who: not listed
    Why: While in flight, the flight crew observed a drop of the outside temperature and recorded seeing St. Elmo’s Fire. They lost the air data reference system. Autopilot and autothrust system disconnected, and fly by wire switched to alternate law (and remained in alternate law for the remainder of the flight.)

    Rudder travel limiter was deactived and airspeed and altitude indication was lost.

    Standby instrumentation allowed data restoration in under ten minutes.

    Delta: Whacked Tail at JFK

    What: Delta Airlines Boeing 767-300 from New York JFK,NY to Sao Paulo
    Where: New York
    When: Mar 6th 2011
    Why: The Delta Boeing encountered heavy wind on takeoff and suffered a tail strike. The pilot returned to JFK and made a safe landing. According to Flight Aware, the duration of the flight was 49 minutes.
