Where: among trees between the 10th, 12th and 14th fairways in Golf Hammock, a golf community west of Sebring in Highlands County
When: 11:30 a.m. Saturday
Who: Pilot James Weener, and passenger James Richer
Why: Witnesses saw black smoke, heard the plane’s engine over-revving, then the plane came apart and in fell in pieces from the sky. A witness said “Some small parts came off first, then the wing came off and the tail shot off. It just disintegrated in midair.”

Why: After taking off, the plane hit trees at the end of the runway.Witnesses described seeing a ball of flames, which ignited a field of dry grass. The grass required seven Country Fire Authority crews to extinguish it.
The two men aboard the vintage plane were John Fisher and David Oxley. In the past, John Fisher, who worked at Maryborough airport, had flown the Tiger Moth from England to Australia. He was the pilot and the owner of the plane.
The accident is under investigation by three investigators from the Australian Transport Safety Bureau.

Private Cessna Crashes en route to Council Bluffs, 3 Fatalities Reported
What: Privately owned Cessna 401 en route from Riverside Kansas to Council Bluffs Kansas
Where: Thomas Road between 1900 & 2000 or 9 miles west of Chanute, KS
When: May 11, 2012 16:36
Who: 5 aboard, 3 fatalities
Why: The privately owned Cessna was en route to Council Bluffs when it told ATC it requested permission to descend to a lower altitude, then crashed near Chanute and caught on fire. The plane skidding 200 feet across a field before it hit trees.23-year-old pilot, Luke F. Sheets, Garret Coble of Tulsa and Stephen J. Luth, 22 died in the crash according to Shawnee County Coroner’s Office-Topeka.
22-year-old Hannah N. Luce of Texas and 27-year-old Austin G. Anderson were injured. Hannah was hospitalized at KU Medical Center-Kansas City, KS and Austin at Christi St. Francis Hospital-Wichita.
Chanute is about 90 miles northwest of Joplin, Mo.
The plane crashed in a field east of Highway 75. Two of the five aboard survived the crash.

Chopper Pilot Dies Aswering Slave Lake Call

What: Campbell Helicopters Ltd. Bell 212 Twin Two-Twelve
Where: Slave Lake, Alberta, Canada
When: May 20, 2011
Who: 1 fatality
Why: The chopper responding to a (wildfire) fire call crashed for unknown reasons 30 meters offshore Slave Lake, Alberta. The forest fire leveled a third of the Slave Lake community. The pilot’s name has not yet been released.The STARS Air Ambulance call was cancelled. The pilot died in the crash. Emergency responders were treated for hypothermia.

2 Injured after Small Plane Crashed near Port Townsend
A small plane crashed at the Discovery Bay Golf Course near Port Townsend, Washington, on September 28.Authorities said the plane, carrying 2 people, went down just a mile west of Jefferson County International Airport.
Both occupants of the plane were injured. Emergency responders had to extricate them from the wreckage before shifting them to Harborview Medical Center, Seattle.
The cause of crash is not clear at the moment.

Belle Isle Chopper Crash Kills Two

Why: After taking off from Houma-Terrebonne airport, the chopper crashed a few miles from East Bay and the Gulf of Mexico in a marshy area. Flames from the crash were seen from a distance. Hunters and fishermen in the area called 911 to report the helicopter afire. The crash claimed the lives of those aboard. Louisiana Department of Wildlife and Fisheries assisted along with several agencies with search, rescue, and recovery.Pilot Jason McKean, and Lanny Ledet had been on the way to an alligator-industry conference in Plaquemines Parish.
Jason McKean had been flying for twenty years. The bodies of the victims were recovered 13 miles southwest of Morgan City.
The cause of the crash is unknown. The FAA is investigating. Tje MTSB saod there was no indication that anything broke off the helicopter before the crash. The final report will not be due for another eighteen months.

Rusty the Border Collie Rescued After his Master’s Fatal Crash

What: Cessna 180 en route from Vulcan, Alta., to Springfield, B.C.,
Where: Williams Lake, Rocky Mountains
When April 26 2011, between 8 and 9 am
Who: Fifty-nine year old pilot Ed Fedaj
Why: Friday, the dog Rusty was found by Parks staff Rusty survived a plane crash that killed his owner Ed Fedaj, but before the dog could be recovered, he disappeared into the park. After being looked over by a vet, Rusty, a 6 yr-old Border Collie will be returned to his family, to be adopted by Ted Fedaj.The Cessna 180 was found where it slammed into a gorge at 1,500 metres on the side of Mount King. Ed’s body was retrieved in spite of an avalanche risk.
Ed had been visiting relatives in southern Alberta and was returning home to Williams Lake when he crashed.
The B.C. Coroners Service and the Federal Transportation Safety Board are investigating.
