    Victorian aerodrome flight safety checks

    Special flight safety checks are being carried out at 18 Victorian aerodromes next week.

    There will be a series of low-level flights at each aerodrome to make sure flight paths continue to operate safely.

    A twin-engine Cessna Conquest aircraft will be used to conduct the safety checks within an area up to 35 kilometres from each aerodrome.

    The two pilots flying the aircraft will ensure navigation aids are operational and not suffering interference, as well as looking for any new obstacles that could be a danger to aviation safety.

    The Civil Aviation Safety Authority requires these checks to be carried out every three years to maintain a high level of air safety at aerodromes.

    Low-level flying is an essential part of the safety exercise, with the aircraft down as low as several hundred feet at times.

    Local residents may notice an uncommon flying pattern, but this is to make sure obstacles are accurately marked on charts and no new obstacles exist. Obstacles can be towers, trees, masts or buildings that can be a danger to aircraft.

    The checks are scheduled to be carried out between Saturday 11 December and Sunday 19 December 2010.

    If poor weather or other factors do not allow the safety checks to go ahead on the planned days they will be carried out as soon as possible.

    CASA has contracted the operation of the special check flights out to Radiola Aerospace Pty Ltd – a company with wide international expertise in these specialist operations.

    Naracoorte aerodrome flight safety checks

    NTSB Meets

    NTSB TO MEET ON BUSINESS JET RUNWAY EXCURSION ACCIDENT THAT KILLED FOUR AND INJURED TWO IN SOUTH CAROLINA

    The National Transportation Safety Board will hold a public Board meeting on its investigation into an accident in which a chartered business jet crashed during a rejected takeoff in Columbia, South Carolina, 18 months ago.

    The purpose of the meeting will be to determine the probable cause of the accident and to consider proposed safety recommendations to reduce the likelihood of future such mishaps.

    On September 19, 2008, at 11:53 p.m. EDT, a Bombardier Learjet Model 60 (N999LJ) operated by Global Exec Aviation and destined for Van Nuys, California, overran runway 11 during a rejected takeoff at Columbia Metropolitan Airport. The captain, the first officer, and two passengers were killed; two other passengers were seriously injured.

    On February 17, 2009, the President signed into law the American Recovery and Reinvestment Act (ARRA),1 designating $1.1 billion for the Federal Aviation Administration (FAA) to invest in Airport Improvement Program (AIP) projects. These funds were intended for airport projects that could achieve several key goals, including investing in transportation infrastructure to provide long-term economic benefits, create jobs, and promote economic recovery. ARRA established tight timeframes for distributing and expending funds and emphasized preference for projects that could be completed in 2 years.

    In August 2009, we issued an advisory to the Office of the Secretary outlining our concerns with FAA’s process for awarding ARRA grants.2 We questioned the economic merit of some lower scoring projects and highlighted several ARRA recipients with grant management problems identified in prior single audit reports.3 Based on these preliminary findings, we initiated this audit to determine the extent to which FAA’s process for awarding ARRA grants complied with ARRA requirements and other associated guidance.4 We conducted this audit from September 2009 through December 2010 in accordance with government

    Air France regrets to confirm the disappearance of flight AF 447 flying from Rio de Janeiro to Paris-Charles de Gaulle, scheduled to arrive at 11:10am local time today, as announced to the press by Air France CEO, Pierre-Henri Gourgeon.

    The Airbus A330-200, registration F-GZCP, left Rio on 31 May at 7:03pm local time (12:03am in Paris).

    The aircraft hit a zone of stormy weather with strong turbulence at 2am this morning (universal time), i.e. 4am in Paris. An automatic message was received from the aircraft at 2:14am (4 :14am in Paris) indicating a failure in the electric circuit a long way from the coast.

    The Brazilian, African, Spanish and French air traffic control centres all tried to make contact with flight AF 447 but to no avail. The French military air traffic control centre tried to detect the aircraft but did not succeed.

    216 passengers were on board: 126 men, 82 women, 7 children and one infant.

    There were 12 flight crew members: 3 pilots and 9 flight attendants.

    The flight captain had a record of 11,000 flight hours and had already flown 1,700 hours on Airbus A330/A340s.

    Of the two first officers, one had flown 3,000 flight hours (800 of which on the Airbus A330/A340) and the other 6,600 (2,600 on the Airbus A330/A340).

    The aircraft was powered by General Electric CF6-80E engines.

    The aircraft had totalled 18,870 flight hours and went into service on 18 April 2005.
    Its last maintenance check in the hangar took place on 16 April 2009.

    Air France fully shares the anxiety and distress of the families concerned. The families are being taken care of in a specially reserved area of Paris-Charles de Gaulle Terminal 2.

    NTSB CITES LACK OF BIRD STRIKE RESISTANT WINDSHIELD REQUIREMENTS IN FATAL CRASH OF HELICOPTER IN LOUISIANA

    The National Transportation Safety Board today released a final report on a fatal crash involving a transport-category helicopter caused by a bird strike. The Board said the lack of requirements for bird strike-resistant windshields contributed to the crash, and called on the FAA to develop such requirements.

    On January 4, 2009, a dual-engine Sikorsky S-76C++ helicopter (N748P), registered to and operated by PHI, Inc., crashed into marshy terrain near Morgan City, Louisiana approximately 7 minutes after takeoff from Amelie, Louisiana, on a charter flight to an oil rig in the Gulf of Mexico. Both pilots and 6 of the 7 passengers were killed in the crash.

    The aircraft had reached level cruise flight at 850 feet mean sea level and 135 knots when the cockpit voice recorder recorded a loud bang, followed by sounds consistent with rushing wind and a power reduction on both engines. The aircraft crashed several seconds later. Feathers and other bird debris were collected from the canopy and windshield of the aircraft. Laboratory analysis identified the remains as coming from a female red-tailed hawk; the average weight of such a bird is 2.4 pounds.

    The investigation revealed that the impact of the bird on the canopy just above the windshield near the engine control quadrant likely jarred the fire extinguisher T-handles out of their detents and moved them aft, pushing both engine control levers into or near the flight idle position, reducing fuel to both engines. The pilots were probably disoriented from the broken windshield and rushing air and were unable to react in time to maintain control of the helicopter.

    The helicopter was originally equipped with laminated glass windshields that complied with European bird-strike resistance standards. PHI replaced the windshields with lighter-weight, aftermarket cast acrylic windshields that did not have any bird-strike resistance standards.

    The NTSB determined that the helicopter crashed because of the sudden loss of power to both engines following the bird strike and the subsequent disorientation of the crewmembers. Contributing to the accident, the Board said, were the lack of FAA regulations and guidance requiring helicopter windshields to be resistant to bird strikes, the lack of protections that would prevent the T-handles from inadvertently dislodging out of their detents, and the lack of a master warning light and audible system to alert the flight crew of a low-rotor speed condition.

    Recommendations were issued to the FAA dealing with, among other things, the design of S-76C++ fire extinguisher T- handles and engine control quadrants, and similar designs of other helicopters, and of audible low-rotor alarm systems; certification standards for helicopter windshields; and simultaneous dual-engine power loss training for helicopter pilots.
