The AUV device was famously designed by the Naval Oceanographic Office, the Office of Naval Research, and the Woods Hole Oceanographic Institution – WHOI, in support of deep-water autonomous operations.

The Canadian NTSB indicated that the Robinson Helicopter R44 II in July 2012 in Carcross, Yukon occurred because of the pilot’s failure to check winds affecting the landing area. On July 10, 2012, an R44 II operated by Horizon Helicopters Ltd. transported 2 Yukon Government surveyors to bear-bait sites in the Carcross area. The helicopter departed Carcross at 3 p.m. followed the north shore of Tagish Lake and approached the wildlife survey site from the west. At approximately 3:13 p.m., the Joint Rescue Coordination Centre received an emergency locator transmitter signal from the aircraft. The wreckage was found approximately 5 nautical miles east of Carcross on Nares Mountain. The pilot was fatally injured, one passenger was seriously injured, and another received minor injuries.
On Oahu, a civilian pilot and a passenger crashed in a pineapple field a mile south of Wheeler Army Airfield. Both were hospitalized. The privately owned AC 8KCAB Decathlon experienced a loss of power. The accident was originally listed as occurring in a helicopter, but it is a single engine fixed wing plane registered to J3 ENGINEERING.

Alaska Airlines flight AS-892 had to return and make an emergency landing in Honolulu, Hawaii, on April 5th.
The Boeing 737-800 plane took off for San Diego, California, but had to turn back after an unusual odor in the aft cabin caused nausea and headache to four crew members.
The plane landed back uneventfully. The affected crew members were taken to a hospital.

On August 9, 2012 at Lagos State University Teaching Hospital, two months after the June 3, 2012 crash, after that all possible DNA testing was complete (only 52 of 149 bodies identified, the rest burnt beyond testability,) the Lagos Government announced that it would release bodies of Dana crash victims to the next of kin.
Families were requested to contact the Funeral Director’s office on 01-8542254 at the teaching hospital 24 hours before they come to claim the bodies.
One family trying to claim their loved one, Mr. George Moses, found he is missing. He was #22 on the list of 29 identified names at the Lekan Ogunsola Memorial mortuary in June. Mr. Achief Olajide, a family member said that after the June 3 crash, he saw Moses intact body, his ID card and wallet. The mortuary refused to let him claim the body, and now it is lost.

What: Cessna 501 Citation I/SP en route from Venice Florida to Macon County, NC Where: Franklin-Macon County Airport, NC When: Mar 15, 2012 Who: 2 crew, 3 passengers, 5 fatalities Why: The plane owned by Bogdan Jakubowski of Venice, Florida appeared to be approaching too steeply when it attempted to land at 1:45, and when the pilot tried to level, a wing hit pavement, flipping and rolling the plane, which exploded into flames. Rescuers were on the scene within a minute but the plane was already engulfed. The names of those aboard have not been released.
