A Haverfield Aviation Hughes 369 outside of Midland Texas was engaged in a routine power line inspection at ten a.m. when the helicopter encountered problems.

The pilot survived. Two linemen suspended from the helicopter by cables fell two hundred feet when the cables snapped.
The helicopter made a safe landing.
OSHA and the FAA are investigating.

Pilots returned to Dallas and made a safe landing at Dallas-Fort Worth International Airport. Passengers were provided an alternative plane to St. Louis.
This is just one of numerous maintenance issues plaguing American Airlines.

According to wptv witnesses aboard the flight, “there was an explosion. Then, all of a sudden, everything stopped, and it just got totally quiet, and everyone started to panic.” Passengers took photos of the plane’s stopped propellor; and then the plane began shaking, and they could feel the weight on the left side.
Pilots returned to Portland and made a safe landing. An inspection of the plane is underway.

Update: The investigation team formed to probe into Friday’s plane crash on Monday (April 23) visited the site and collected evidence. What: Bhoja Air Boeing 737-200 en route from Karachi to Islamabad Where: Islamabad When: Apr 20th 2012 Who: 121 passengers and 6 crew Why: The plane (reported as #AP-BKC a 27 year old Boeing) took off from Karachi, impacted the ground 3-6 miles from Islamabad while on approach and burst into flames in a rural location(Hussain Abad village) near Chaklala Air Base. No survivors were found, and ten houses on the ground were affected. The plane lost contact with the control tower minutes before landing. There was no distress call. There are conflicting reports that the plane was on fire and was not on fire before it impacted the ground. Some reports say the plane was struck by lightning. There was a thunderstorm at the time of the accident. The fire/fireball after the crash does not appear to have engulfed the entire debris area, according to photos of the scene.
Debris from the crash is said to cover a large muddy area. The area has not been cordoned off; reporters and others are swarming over the crash site.
This is reported to be the first Bhorja flight on this route. The jet left at 17:00 and was due to arrive at 18:50.
The flight data recorder has been recovered along with 110 bodies (so far.)
The number of people listed as passengers varies from 127 to 130 aboard.

What: Russian Air Force Antonov An-30B Where: Čáslav Airport, Czech Republic When: 23-MAY-2012 Who: 23 aboard Why: On landing at Čáslav, the pilot lost control, and the plane skidded off the runway.
There were fourteen Russians aboard, and 8 Czechs, six of whom were seriously injured. Ten ambulances and two choppers responded to the scene. Four patients were taken to Čáslav hospital with burns. Four injured were treated on the scene. One passenger with severe burns went to Vinohrady Hospital in Prague. Seven passengers and the pilot were taken by ambulance to a Cologne hospital. The emergency was declared at 11:50, and firefighters had the fire under control by 12:25. There were no casualties of the accident.
Specialists were testing the area for radiation, which was observed in the vicinity of the plane. The plane belongs to the”Open Skies” mission. The Antonov An-30B is an aerial survey observation plane.

What: Spirit Airlines Airbus A319 en route from Los Angeles to Fort Lauderdale Where: Houston When: July 8, 2012 Who: 100+ passengers Why: After a disruption aboard the flight, the pilots diverted the Spirit Airlines flight to Houston, where they made a safe landing. Passengers who disembarked had no Spirit Airlines counter because, although flights are soon scheduled to begin landing there, currently there is no Spirit Airlines Customer Service at Houston Intercontinental Airport.
On arrival, the disruptive passenger was handed left in the custody of Houston Law Enforcement.
The disruptive passenger was an elderly blind frenchman in his eighties who had become disoriented and frightened. His son was called by authorities.
Passengers reboarded the flight and departed Houston at 6:42 PM.
