On January 7, 2013, Chinook helicopter (registration N241CH) Pucallpa-Tarapoto was en route near the Laguna Yarinacocha in Pucallpa Peru when it came down in the Amazon.
The helicopter took off at took off at 2:57 pm, taking a load to Tarapoto.
According to reports, although the crew tossed the load while airborne, the plane broke up in mid air and caught fire. There were seven people aboard.
There were 2 Peruvians aboard: Joel Castillo and Luis Ramos.
Five Americans: Dan Immel , Ed Córdova, Jaime Pickett, Lee Bradford and Darrel Birkes.
Those aboard were employed by Petrominerales Ltd, a Canadian-based oil exploration firm.

What: Kiunga Aviation Cessna A185E Skywagon en route from Nadzab to Baindoang Where: Papua New Guinea When: Dec 30, 2009 Who: 7 aboard, 6 fatalities Why: It is conjectured that the plane developed engine trouble. (Apparently the engine caught fire.) The pilot radioed a distress call before it went down in a hilly region, scattering the wreckage on the hillside.
The passengers, four adults and two children, were all fatalities. Their burned remains were taken to Angau Memorial Hospital.
Pilot Richard Leahy survived the crash and was evacuated to Australia for medical attention.

What: Rossiya Airbus A319-100 en route from St. Petersburg to Moscow Where: St. Petersburg, Russia When: Oct 7th 2011 Why: After takeoff, the plane developed engine failure in the left engine.
The pilots shut down the left engine and returned to make a safe landing.
The flight was in the air less than twenty minutes. A replacement jet was provided within three hours.

What: Westjet Boeing 737-700 en route from Montreal to Calgary Where: Winnipeg When: Oct 6 2010 Who: 105 aboard Why: While en route, the plane emitted an alert, and subsequently made an emergency descent and landing. Although they received an emergency indicator engaged, there were no problems with the landing. Emergency services were on standby.
Maintenance determined that the problem was due to a faulty sensor, and replaced same.

What: S-Air Hawker-125 Executive Jet en route from Vnukovo airport in Moscow to the Minsk international airport. Where: Minsk When: October 26, 2009 Who: Two passengers and three crew Why: The Russian business jet crashed Monday night into a swampy area near Minsk while on approach to the airport. 5 are confirmed dead, including CEO of the S-Air airline, Marat Romashkin. A sixth passenger was supposed to aboard but missed the flight.
The plane failed it’s first landing and disappeared off radar. The wreckage was 2.5 miles from the airport.
The S-Air fleet includes Yak-42D, Tu-134, Yak-40, Hawker-125-700B and Hawker-125-800B planes.

Update: The investigation team formed to probe into Friday’s plane crash on Monday (April 23) visited the site and collected evidence. What: Bhoja Air Boeing 737-200 en route from Karachi to Islamabad Where: Islamabad When: Apr 20th 2012 Who: 121 passengers and 6 crew Why: The plane (reported as #AP-BKC a 27 year old Boeing) took off from Karachi, impacted the ground 3-6 miles from Islamabad while on approach and burst into flames in a rural location(Hussain Abad village) near Chaklala Air Base. No survivors were found, and ten houses on the ground were affected. The plane lost contact with the control tower minutes before landing. There was no distress call. There are conflicting reports that the plane was on fire and was not on fire before it impacted the ground. Some reports say the plane was struck by lightning. There was a thunderstorm at the time of the accident. The fire/fireball after the crash does not appear to have engulfed the entire debris area, according to photos of the scene.
Debris from the crash is said to cover a large muddy area. The area has not been cordoned off; reporters and others are swarming over the crash site.
This is reported to be the first Bhorja flight on this route. The jet left at 17:00 and was due to arrive at 18:50.
The flight data recorder has been recovered along with 110 bodies (so far.)
The number of people listed as passengers varies from 127 to 130 aboard.

What: small one seat experimental aircraft built and owned by Neal O. Bond Where: behind a home near the Pegasus Air Park on McIlhenny Road in Chestnuthill Township, Monroe County, about 40 miles north of Allentown PA When: August 30, 2009 Who: the pilot Why: A witness says that the plane was coming down with an unusual attitude, and part of the tail came off. A second or two later a wing came off. The plane crashed behind the airport; the pilot had been ejected from the craft, a homebuilt one seat vehicle.
