    Chartered Flight Crashes in Paraná Brazil


    What: Táxi Aéreo Hercules Piper PA-31T2 Cheyenne II XL
    Where: Almirante Tamandaré, Paraná Brazil
    When: Nov 6, 2012
    Who: 4 aboard, 4 fatalities
    Why: The Táxi Aéreo Hercules Piper left Dourados for the Bacacheri Airport, in Curitiba with the pilot, copilot and two passengers. Táxi Aéreo operates out of Bacacheri Airport. The flight was chartered for Brinks and was carrying money.

    One security personnel named Wilmar survived the crash and was taken to Hospital do Trabalhador where he later died.

    The plane crashed fifteen miles short of its destination.

    Seripa (Serviço Regional de Investigação e Prevenção de Acidentes Aeronáuticos) is investigating. Currently no cause for the accident has been released.

    Flybe Flight Returns to Southampton due to a Bee

    FlybeFlybe flight BE384 had to return and make an emergency landing in Southampton, England, on June 5.

    According to an airline spokesperson, “Flybe can confirm that flight BE384 travelling to Dublin returned from airborne to Southampton following a suspected technical issue. The aircraft landed without incident and all passengers disembarked as normal… Upon inspection, Flybe engineers did discover that the cause of the issue was a bee that had become lodged in an item of instrumentation on the outside of the aircraft… The safety of its passengers and crew is the airline’s number one priority and Flybe regrets any inconvenience experienced as a result of the delay to this flight.”

    Press Release – FAA Proposes $168,000 Civil Penalty Against Radiology Corp.

    For Immediate Release

    WASHINGTON – The Federal Aviation Administration is proposing a $168,000 civil penalty against Radiology Corporation of America, Inc., of Delray Beach, Fla., for allegedly violating Department of Transportation hazardous materials regulations.

    The FAA alleges Radiology Corp. offered a fiberboard box to Delta Airlines in Atlanta as checked baggage, Feb. 6, 2010. The box housed a soldering iron containing liquid butane fuel, a flammable gas, which is a hazardous material. The shipment was not declared to contain hazardous materials.

    Radiology Corp. allegedly offered the checked baggage for transportation by air when it was not packaged, marked, classed, described, labeled or in condition for shipment as required by regulations. Delta employees at the Atlanta airport discovered the shipment before it was loaded on an aircraft.

    Radiology Corporation of America Inc., has 30 days from receipt of the FAA’s civil penalty letter to respond to the agency.

    Helicopter and Plane Collide Mid-Air in Buckinghamshire; Four Killed

    A two-seater plane collided mid-air with a helicopter in Buckinghamshire, England, on November 17th.

    It is believed that both aircraft were flying from Wycombe Air Park when they collided near Waddesdon.

    Four people were killed in the crash.

    The Air Accidents Investigation Branch is investigating the cause of accident.

    Private Cessna Crashes en route to Council Bluffs, 3 Fatalities Reported

    What: Privately owned Cessna 401 en route from Riverside Kansas to Council Bluffs Kansas
    Where: Thomas Road between 1900 & 2000 or 9 miles west of Chanute, KS
    When: May 11, 2012 16:36
    Who: 5 aboard, 3 fatalities
    Why: The privately owned Cessna was en route to Council Bluffs when it told ATC it requested permission to descend to a lower altitude, then crashed near Chanute and caught on fire. The plane skidding 200 feet across a field before it hit trees.

    23-year-old pilot, Luke F. Sheets, Garret Coble of Tulsa and Stephen J. Luth, 22 died in the crash according to Shawnee County Coroner’s Office-Topeka.

    22-year-old Hannah N. Luce of Texas and 27-year-old Austin G. Anderson were injured. Hannah was hospitalized at KU Medical Center-Kansas City, KS and Austin at Christi St. Francis Hospital-Wichita.

    Chanute is about 90 miles northwest of Joplin, Mo.

    The plane crashed in a field east of Highway 75. Two of the five aboard survived the crash.

    Small Plane Crashes Near Camp Verde; 2 Dead

    A small plane crashed near Camp Verde, Arizona, on August 18th.

    The incident happened when the pilot was taking a friend for a short flight around Yavapai County.

    Both occupants of the plane were killed in the crash. They were identified as pilot Glenn Tenniswood, 68, and passenger Ron Walker, 78.

    The cause of crash is being investigated.
