    Papua New Guinea 28 Die in Violent Storm, Forest Crash on the Banks of the Gogol

    What: PNG Airlines de Havilland Canada DHC-8-102
    Where: 20 km (12.5 mls) S of Madang Airport (MAG) (Papua New Guinea)
    When: Oct 13, 2011
    Who: 4 crew, 28 passengers, 4 survivors
    Why: About 5.15pm, while en route from Lae to Madang, the flight went down in dense forest in bad weather. The pilots managed to send a distress call. Four survivors were pulled from the wreckage and taken to Yamad clinic. The survivors include an 64 year old Australian pilot and a New Zealand pilot and an Asian passenger who was sitting in the seventh seat, who crawled through a crack in the fuselage, and who suffered burns on 27% of his body (including back and arms). Survivors are being transported by boat to Modilon hospital in Madang.

    First responders were initially blocked when they could not cross the Guabe river, then a boat suffered engine problems, then the third attempt made it to the scene of the crash. A violent storm and dense forest hampered the rescue efforts, which were facilitated by chopper.

    Australia engaged four army Blackhawk helicopters to assist in the rescue. Rescuers on the Gogol river were impeded due to heavy rains. It took four boats to reach the crash site.

    PNG quarantined Laes Nadzab airport and grounded the Dash 8 fleet.

    Witnesses saw the plane crash on the Guabe River bank and on impact, burst into flame. Most of the passengers were local Papuan parents of Madang’s Divine Word University students. According to a resort manager who went to pick them up from the airport, six of the plane’s occupants — one passenger and five crew members – had reservations to stay at Madang Resort.

    PNG Accident Investigation Commission’s spokesman Sid O’Toole talked to the media.

    We are sad to confirm that there has been an accident involving an Airlines PNG Dash 8 aircraft near Madang late Thursday afternoon.

    Emergency services have been activated and Airlines PNG is co-operating with authorities to mount rescue and recovery efforts.

    There were 28 passengers and 4 crew members on board. There appear to be some survivors while a number of people remain unaccounted for.

    Airlines PNG is working with the emergency service authorities to confirm this information in more detail.

    A full investigation is underway by authorities and Airlines PNG as to the possible cause of the accident.

    Airlines PNG fully supports the action of local authorities at Lae who have quarantined aviation fuel at Lae airport from where the aircraft originated.

    Airlines PNG has also grounded its Dash 8 fleet of 12 aircraft until further notice.

    Our prayers and thoughts are now with all those affected by this very sad day for Papua New Guinea.

    Maintenance is Key to Aviation Safety

    Inevitably into my business life flows discussion of (aviation-oriented) sequestration, the closing of traffic control towers, and how this will inevitably lead to more aviation accidents.

    Yes, I agree with Harrison Ford’s comment that accidents are going to happen. But that prediction leaves a lot unsaid.

    Cutbacks in other places, cutbacks in maintenance budgets, in the number and quality of inspections and maintenance personnel are going to be just as lethal.

    Turn on your mind’s eye and picture the air traffic situation as you would on the ground in your car. Suspended towers are like suspended traffic lights. Picture what would happen if intersections were eliminated, forcing traffic from smaller streets to the larger intersections that are already overburdened with traffic. Into this already overburdened traffic situation, maintenance shortfalls make the problem even worse.

    You have older, poorly maintained vehicles in the flow of traffic, and they’re falling apart, causing crashes and pileups. On the ground, they cause disaster. More so, falling from the sky.

    Maintenance is a complicated thing, because even the perfect man-made thing is subject to the laws of physics.

    The most perfect plane would decay over time even if it were not flown. So of course, even the best maintained vehicles are subject to fatigue. And not everything is maintained to “perfect” standards. Believe me, I see this first-hand, as I fly.

    When the first commercial planes were built, who would have guessed planes would be required to fly for so long, so continuously and over such distances. It’s miracle enough that a machine can get people off the ground at all, much less doing it continuously for twenty years.

    As fleets age, you have rivets flying all over the place when there is metal fatigue. Especially with older planes, metal fatigue will be increasingly the cause of future plane crashes. There are two choices: 1) old planes will be automatically junked (unlikely to happen in our increasingly green society) or 2) extreme comprehensive and manditory testing must be put in to place. This testing-maintenance can not be cut back.

    I don’t mean put in to place after an event. I mean in place to prevent an event. To be able to get the plane in the air in the first place, most components of plane have been studied to the breaking point already. That is the kind of knowledge that must be applied to maintenance schedules. Get those parts replaced well before they become the weakest link.

    MAINTENANCE is where it is. You can see the decay on the inside, on the parts that don’t matter much for flight safety. The seats on a plane break apart. Window shades won’t close. They are stuck up there somewhere, and if you try and force them, they break. (Just think of what frailties develop in crucial components that the passenger can’t see.)

    The metal on a plane degrades in the same way. (Engineers have a name and formula for it: Paris-Erdogan law.) If you sit on the wing of an airliner that you know is 20+ years old—such as the plane I was on yesterday from New York—and you encounter turbulence—as we did—any passenger stuck on that plane can’t help but look in disbelief at wings that are bobbling up and down and flexing like a preschooler’s teeter totter. Here’s the question you don’t want to ask yourself at 20,000 feet: are the wings going to stay put? Are they going to flex and flex and flex like a metal clothes hanger bending till it breaks? How do wings not come off the aging plane?

    I’m not accepting of the fact that crashes will happen. That’s too easy to say. It is pure negligence to accept oncoming disaster and do nothing to avert it. We can’t just let it ride. The aviation industry must remain proactive, no matter the cost.

    It’s like the poor horse in Central Park. The older he is, the more maintenance he requires to keep from collapsing as he pulls the buggies and sometimes heavy bodies of those in the carriage. He needs to be fed better, to have water more often, to have a pasture to enjoy, and other horses available where he can socialize. Though the needs of a living creature differ from those of a machine, it goes without saying that both will thrive better with love and care than without.

    Maintenance is key. First-class maintenance. Constant, consistent, perpetual maintenance. It is not adequate to rely on the pilot alone to do his walk around of the aircraft prior to take off. Sure, the pilot should do his visual, but his walk should be preceded by the maintenance specialist. The experts must scrutinize, inspect, examine, and put the plane through its paces.

    Especially the aging plane.

    May 31:

    22:03 — Air France flight 447 departs from Rio de Janeiro. (Brazilian Air Force says plane left at 22:30.)

    June 1:

    01:30 — Plane has last contact with Brazil air traffic control, where it is 351 miles from the city of Natal.

    01:48 — Brazilian Air Force has last radar contact. Plane appears to be flying normally.

    02:00 — Aircraft hits a zone of stormy weather with strong turbulence

    02:14 — An automatic message was received from the aircraft at indicating a failure in the electric circuit a long way from the coast.

    02:20 — Plane fails to make previously scheduled radio contact with Brazil. Brazil notifies air traffic control in Dakar, Senegal.

    05:00 – 06:00 — Brazilian, African, Spanish and French air traffic control centers try to make contact with the flight, but are not successful. The French military air traffic control center cannot detect the aircraft.

    05:30 — Brazilian Air Force starts a search and rescue mission

    07:30 — Air France sets up a crisis center

    09:15 — Flight was expected to arrive in Paris

    11:31 — Air France announces the following: “Air France regrets to announce that it has lost contact with flight AF 447 from Rio de Janeiro to Paris-Charles de Gaulle, expected to arrive this morning at 11:15 local time.”

    13:10 — Air France announces the following: “Air France regrets to confirm the disappearance of flight AF 447 flying from Rio de Janeiro to Paris-Charles de Gaulle, scheduled to arrive at 11:10am local time today, as announced to the press by Air France CEO, Pierre-Henri Gourgeon.”

    13:59 — Air France reports that it has sent all of the information it has regarding the flight to the French Accident Investigation Bureau for civil aviation (BEA) and to Airbus

    14:44 — By this time, the airplane is expected to have run out of fuel. Air France publicizes “its deepest sympathy to the relatives and friends of the passengers and crew who were on board AF flight 447 on 31 May 2009, which disappeared somewhere between Rio de Janeiro and Paris-Charles de Gaulle.”

    Airbus also announces, “Airbus regrets to confirm that an Airbus A330-200 operated by Air France has been lost about 3.5 hours after departure.”

    18:13 — Air France releases a list of the nationalities on board

    Frontier Airlines Plane Makes Emergency Landing in Las Vegas

    frontier airlinesFrontier flight 555 made an emergency landing at McCarran International Airport in Las Vegas, Nevada, on January 6.

    The A319 airbus, en-route from Denver, Colorado, to San Diego, California, was diverted due to an engine problem.

    The plane landed safely. Everyone aboard remained unhurt.

    Saudi Arabian Airlines Plane Makes Emergency Landing in Egypt

    Saudi Arabian AirlinesSaudi Arabian Airlines flight SV-321 made an emergency landing in Cairo, Egypt, on November 1st.

    The Airbus A321-200 plane flying from Dammam, Saudi Arabia, was on final approach to Cairo when the crew received a brakes fault indication and prepared for an emergency landing.

    The plane landed uneventfully. All passengers and crew members remained safe.

    Qatar Airways Flight Makes Emergency Landing in Doha

    Qatar Airways flight QR-307 had to return and make an emergency landing in Doha, Qatar, on October 21st.

    The Airbus A320-200 plane took off for Helsinki, Finland, but had to turn back due to a technical issue.

    The plane landed safely. All one hundred and fifty people aboard remained unharmed.
